One thing I find that has helped me hold power outdoors is to focus on my cadence rather than shifting or gearing (particularly on flat-ish terrain or a steady climb). Hold the cadence steady long enough to see if you're on target (around 10 seconds will give you three readings for 3s power) and adjust cadence up or down to make minor power adjustments as needed. For bigger adjustments (more than 20 watts), shifting may be a better choice. The more you play around with it outside, the more you'll start to recognize how your cadence or shifting will change the power. There's a definite learning curve when you first start trying to hold workout power targets outdoors if you spend your indoor time on ERG and you sound like you're on the right path already!
