She's a broken, fractured mess, almost a corpse by this point and your decision makes her give up on life and die. Luckily she has other fragments of hers so she can still dial back time for you to make another choice another playthrough.
Sophia was a great climax for the story! But they shouldn't time pressure the player into a euthanasia decision. My gaming mind is used defeat the bad guys and save the princess. This one truly hit me hard... 💀💀💀
He gives that dialogue even if you give her peace but choose to lie to him by saying you released her. Because if you don't give her peace, both options are a lie, that's why Manus sees through it. You clearly didn't "release" her as she dies an agonizing death moments after your arrival and you also didn't give her the peace she explicitly asked of you.
Increasing your humanity level. Every time you lie it shows on the screen that your puppet heart is reacting, when this message shows it means that your humanity level has increased. There are other actions hidden in the game that increase your humanity.

I got the real ending on my 3rd play thru and I never thought of letting her live 💀
Devs really lost a good chance do put: 'I CAN FIX HER' option.
