Great advice! Some additional tips:
1. When stealing from a regular house, only men and male teenagers will attack you if they catch you stealing, the rest will just flee.
2. If you’re caught stealing, just leave and re-enter the house. This resets the aggression meter.
3. When playing as Haruspex, hearts are the most valuable organ, so if you don’t have enough space for harvesting every organ from a body, prioritise those.
4. Still Haruspex; if you don’t mind meta-gaming, trade away all white whip and blood twyre, hold onto swevery, brown twyre, and black twyre. 1 black, 1 brown, 2 swevery makes a ridiculously effective immunity booster, and if combined with an infected organ, makes the best antibiotic. Only the Haruspex can consume them, but it means all other antibiotics and immunity boosters can be safely sold or traded as needed (excepting those blasted yellow pills)

Nice tips. Pathologic 1 is secretly very easy, it's just obtuse about its mechanics so it can be punishing to new players. If you've already played P2 as I had when I first played P1, you will find the economy trivial to exploit and the survival gameplay is mostly just grinding and busywork you do on the way between NPC dialogues. After Day 2 it's not at all difficult to build a growing stack of food, drugs, money, and bullets as long as you know what you're doing. Loot trash bins and buy all the barter junk you can find in shops, trade with kids for medicine and ammo, sell anything you don't need (most of it) for copious piles of cash, repeat for 20-30 hours. Break into people's houses to steal their belongings and kill them if you feel like it, as you can trivially renew your reputation by killing bandits at night.
Not to mention that the Bachelor route pulls its punches like crazy; you can acquire up to 3 free revolvers by Day 2, affluent citizens will donate money to help you complete quests just because you're the big city doctor, and you rarely have to get your hands dirty to get things done. I was allowing myself to get infected (going into plague houses and not reloading my save if a cloud hit me) just for a semblance of challenge. The Haruspex route is a little tougher due to your reputation problems at the start, but that's easily solved as you mentioned. You can get screwed by quests like Taya's demand for 50 alpha tablets if you're not planning ahead, though.
I currently have a mod on the backburner in development which is the first of its kind to rebalance P1's gameplay and make it more hardcore and engaging (it changes shop prices, barter values, loot, item effects, reputation sources, and more). For example, killing criminals no longer restores reputation, so you'll need to perform your role as a doctor if it's taken a hit. On the other hand, the bandit throwing knife doesn't deal 90 damage anymore, as that hardly suited the kind of attrition gameplay Pathologic is going for. To anyone having trouble with the knife throw instakills, just remember to run perpendicular to any bandit who's doing their windup throw animation as they don't lead their throw.

Artemy's route wasn't a challenge for me either in that regard. You can get a good gun and a handful of bullets from an "unofficial" quest from Grief on Day 1, and your reputation should be normal by the end of the day anyway, even if you've made... questionable choices in the side quests
I really like there being a short and well explained guide like this to the game. I'd bet this video is going to help a fair few people not just bounce off Pathologic in the first few hours. All were good tips for making a playthrough smoother. I did not know about the barrel glitch in the warehouse or that plague clouds could not go up ledges, that's kinda funny as well.
I think the main way in which Pathologic makes itself difficult is the lack of information. NPC's lie to you, quests are unclear, and, depending on the translation you are using, some of the dialogue in the letters and objectives is just actually just incorrect (I'm looking at you building names!)
A lot of people came into Pathologic from more modern survival/RPG games. Games with objective markers and clear quest lines (and clear survival mechanics). And it's a shock to the system (no pun intended) to come across a game that holds its cards so close to its chest and gives you so little direction. It almost feels like Pathologic doesn't care if you know what's going on, or if you like it or even if you play it. Its a fairly unique feeling of desperation, frustration and bleakness that few other games have managed to (or even tried to) capture.
I don’t think the NPCs lying made it more difficult, at least for me. More annoying and confusing absolutely but to be fair it is one of the few things the game warns you about right from the start.
Absolutely agree on the unclear quests though, super annoying to be on a clock with no idea where you are meant to go.
ALSO ALSO ALSO not all food is equal. day 1 you may feel the need to buy all the food but you only really need to buy three 3 kinds of food. Meat, fish, bread. buy these 3 as much as you can day one. if your good about it and only eat when you really need to you can make the food you buy day 1 as the bachelor last you pretty much through the entire game.
true! Fresh meat is a ripoff though, it's marked up a huge amount and only restores 1% more hunger than a Bread. Smoked meat is useful tho :)
That tip about storing Artemy´s itens in the box is so useful! The first time i played his route i lost the shotgun and some ammo . Great work!!
You did not mentioned the Clothing shop near the Cemetery, on day 2 you can sell items there for fat money, ez 100k on day 2 if you are prepared

Good tips! I definitely undervalued the painkillers. The rest of it I figured out on my own by just playing the game and observing, which shows that this game isn't as hard as people make it to be (Pathologic 2 experience probably helped a lot though)
Btw the barrel cheese is not so straight-forward: I wasn't able to just jump onto it, it only worked when I got hit mid-air by one of the muggers.
You could do the same for P2 just explain that those cabinets in the Crude Sprawl respawn every midnight. I always find at least a free ring or a bracelet in every run there.
