Just a quick note re: niacinamide. When I first got into skincare a few years ago, I, like many, was introduced to The Ordinary & went out and bought a ton of their products. One of them was the 10% niacinamide serum. Was fine at first but after a couple weeks it began to cause a lot of irritation. So did the Cerave PM moisturizer so I thought I just couldn’t tolerate niacinamide as an ingredient. Long story short, I have come to find that formulation is key (TO is repeatedly noted as being an issue, as is the Cerave moisturizer. The ones it works great for, awesome. But there are many that it doesn’t). And 10% is more than most people need, I think. Oily skin with acne probably benefit the most from TO serum but no one else needs that much in one shot. I currently use several products that contain niacinamide with 2-5% in any one of them. Have never had issues when used this way (even if I use multiple products in a routine). So I just wanted to say that if TO serum was an issue, don’t write off niacinamide entirely. Try other products & see if niacinamide, in of itself, is the actual issue.

I'm 65 and never did much for my skin but after finding your channel, my skin has really improved. The mainstay of my skincare are cereve products and tretinoin for my face (for sebaceous hyperplasia and hyperpigmentation) and the advanced clinicals retinol cream and aveno lotion for my body plus, of course sunscreens, ulta md for my face and different spf 50 lotions for my body (depending on whats on sale). Thank you Dr. Dray; you're never too old to start taking care of the biggest organ of your body :-)
Blue light+red light+orange light therapy was the only thing that helped with my adult acne. I tried everything else(tretinoin,adapalene,birth control,panthothenic acid,benzoyl peroxide,azelaic acid,niacinamide,you name it). I took about half a year to see significant improvement. It's literally the best thing I did to my skin
I have been using tretinoin for about five months now and finally have gotten used to it. For a short while I couldn’t put anything on my skin. Now I have been using La Roche Posey lipikar with Trentinoin and is well tolerated.
Adore Dr Dray. The comment about not needing to use Tretinoin is absolutely fair. You don’t have to. But (in Canada) $20 gets you a 2-3 month supply and compared to pricing and value to every leading retinol product - well those are definitely NOT $20 for 60+ supply and don’t work nearly as well.
@@MsSrushh most GPs will just prescribe it unless you’re breast feeding or pregnant. If not, Felix or Maple telehealth will. $50 for the visit and you’ll get a 12 month supply. Each tube is $20 and lasts 2 months if you like to ignore the “pea sized” advice and 3 months if you listen to doctors. Guess which one I am.
I love niacinamide serum. My rosacea has gone into remission since I started using it. Good Molecules's version is $6 per ounce and last me about 6 weeks.
I discovered niaciamide thanks to dr. Dray. They do help with rebuilding skin barrier. I have rosacea and what helped quite a lot is using the PM light moisturizer from CeraVe. Another helpful product is XeraCalm lipid replenishing lotion, which I use for my face. Azelaic acid, weirdly enough, caused more problems and seemed to exacerbate my rosacea. Overall, CeraVe and Avene are two winners for me in 2022. Happy New Year! 🎉 🎊
i wish snail mucin and panthenol got the same amount of hype as hyaluronic acid and niacinamide. the latter irritates my skin and the former….. just isn’t that special to me, but snail mucin and panthenol are my holy grail!!!!!!
but also, shout out to cerave healing ointment, the only product with hyaluronic acid i use, and it is my lip care holy grail. the only product that gets my lips to stop peeling.
There are videos which discuss this ...I don't remember which ones at this time...but a few said yes...also depends on you for effective %of tretinoin.Some skins are too sensitive for the .o1% which is the stronger.I have both,the .05 and the .o1....and alternate with them...the .o5 is a cream and is nice ,the .o1% is a gel and is more harsh...but every one has a different skin type..So I hope this helps..
Topical tranexemic acid did nothing for me but what a world of difference the 650 mg Rx oral version has done over the past couple of years of being on it off and on. Has been my miracle drug to combat my melasma on my olive skin tone when my derm discourages lasers! Definitely going to ask next month about azelaic acid since I hate the drying and irritation of Differin or other retinol type products!
@@Idk-do4db I didn't experience any side effects. My derm makes me do a wash of anywhere between 1-3 months though. You're only allowed to be on it for 3 months at a time since I started it when this was a new thing 2-3 years ago, and there wasn't enough studies done on the long-term effects since it was originally developed for a different purpose. Yes, melasma just slowly fades over time. I see results at the 6-8 week mark so hate going off of it every 3 months actually and beg to only have a 1 month wash instead of 3 month wash in between cycles. It's a cumulative effect though so long-term use even off/on like that has helped me SIGNIFICANTLY. Plus it only costs $15 a month with my insurance so you can't beat that compared to the topical version that was costing me $70 a month!
I used the Paulas choice azelaic acid booster for 6 months, it did nothing. Then I found out I can buy 20% azelaic acid in pharmacy over the counter in my country in Europe 😂 it worked like magic for my hyperpigmentation.

When I heard snail mucin was getting more love, I was like FINALLY because it had its hype quite a while ago in Asia, and yes even in South Korea. As far I know, it started was a medicinal food trend due to snails being recommended for the joints in eastern traditional medicine, which was then later used for moisturzing the skin.
Why should we use living sentient beings for our skincare routine? The cosmetics industry already has enough cruelty with testing and other animal derived ingredients
@@cecegirl4327 Because they work and unless the flesh is also used, the mucins are taken without abusing them. I should say, I thought someone like you would comment on the actual eating of snails rather than the gathering of the mucins considering that the former results in their death while the latter doesn't. Personally, I am more against the use of botanical extracts and derivatives since it's not so practical ecologically speaking and plants are fundamental for the environment. Also, there are far too many animals or animal parts that could be used. Far too many go to waste in my opinion.
@msk it's inherently exploitative and to meet demand they must keep breeding them...continuing an exploitative cycle. I'm also not entirely convince no abuse happens at such a mass scale of production. That's almost impossible. I'm against eating snails as well. Dr dray used to be vegan/plant-based but it seems like she's moved away from that based on the skincare items she promotes. Which is really a pity since science and ethics can easily go hand-in-hand. Also just because something works doesn't mean we should use it. There's other things that could possible work and at the end of the day vanity is not something that should come before the lives of sentient beings.

@@cecegirl4327 I dunno man, saying the use of animal part wastes is unsustainable sounds like using petroleum jelly is unsustainable. In a way worse since the use and consumption of meat is never going to be completely replaced considering it's ingrained in our hardware due to millenia of evolution, unlike fossil fuel. As long as we consume all part of the animal, we'd be needing less amount of cattles and etc, so that ought to be the mindset we should promote than completely ceasing animal consumption. This is frankly an "original sin" of a sort for the west since the wasting of animal parts was aggravated due to the industrial revolution, colonialism and culture of not using all parts of the animal. Pork skin and starfish are good sources of collagen by the way. Pork skin derived collagen helped my eczema and acne prone skill well during my childhood, and snail mucin is great for soothing my irritation. Oat extracts are great but not moisturizing enough and centella asiatica and tea tree extracts in high dosage seem to backfire and cause irritation. So perhaps I am more biased to using animal parts and derivatives. And botanical extracts can be more sustainable, but that is so because people are trying to do it right, I say. Similarly, I stand by that animal derived products are fine as long as we do it right.
Also: game changer for my very reactive sensitive rosacea skin- my derm compounded my Tretinoin with Azalaic Acid 15%. I have -0- issues and my skin is so HAPPY now even using Tret .1% w/azalaic acid 15%
Interesting. If I remember right, when I tried an online subscription compounding service (can’t remember the name, but it’s quite popular) I think this is what they gave me. But they also put clindamycin in there and I didn’t want an antibiotic. Maybe I should try it again and just ask them to exclude that because after years of trying, tret still makes my skin a mess.

I did use Skinoren 20% but it was too much I had sever acne, but I was so irritated, and I use peroral isotretioin that didn't cause any topic irritation, it desteoyed my sebaceus glands from inside. Anyhow, that is what U have heard to make a pause.

Is it true that you don't get the anti aging benefits of niacinamide if only using it in your suncream, because suncream just sits as a film on top of the skin?
I love your content! I learn so much. I would like to see a video on your thoughts about how much importance we should hold FDA approval to. There are tons and tons of drugs and products that were and are FDA approved that lead to cancer and other issues. FDA approval doesn't have the same level of trust as it once did, and with something like skincare that's even less regulated, is there something else we can look to for safety of a product?

My dermatologist have been prescribing me steroids eversince but I seldom use them because I don't want to heavily rely on them. I used to use niacinamide but now it irritates my skin so I stopped... But I do have dark spots and acne scars... (I use tretinoin so i don't really have active acne anymore)... What product would you suggest that would help me with dark spots and won't mess with my tretinoin?
Interesting Azelaic acid Doctorly also think underrated. Next time I go to another country will try to get the Azelaic acid, right now I’m using the 10% from ordinary. And I started Tretinonin .25% for my mom and .5% for me because my mom has never used retinol. I also got the Ominilux for my mom and I to use. And we started IPL laser via groupon and my also upgraded to BLP laser which burnt her skin but after that came off her dark spots did look lighter. The IPL did nothing for me but I refused to turn up the intensity for fear of burning my skin. Next year we will try Clear and Brilliance laser also via groupon which a friend tried and thinks it was great for her. My goal is to help my mama improve her skin!
Using tretinonin I got at a pharmacy in Dubai. There can get tretinonin without prescription for .25% and .5% for less than $10. In USA I would pay $80. Saids on the tube Acretin Jamjoon Pharma

Hi Dr.Dray - I am 31 and trying to be preventive with aging but also still dealing with adult acne, I want to incorporate a LED face mask and wondering if I should do a Red only like Omnilux or a combo like Qure (although the red is at 630 nm instead of 633 nm unsure how much this matters). Do you have thoughts on which mask preforms better?
I have been using retinol for almost 2 months now, recommended by my dermatologist because of my acne scars, so my skin renews itself, but I think it's not doing anything and I'm getting like comedones and tiny little acne, like skin texture. Is this normal? It doesn't sting or peel so I think it is not irritation, I'm only using 0,03% retinol so it's okay, I think. What can I do to stop this? I used to have awful cystic acne and I went on accutane with my derm following along for almost 2 years with pauses in the summer. Now I've stoped 2 years ago and my boxscar and ice pick scars are gonna be here foverer unless I do microneedling and stuff, I'm aware, but I'm 19 so not doing that now ofc and I think since I'm so young my skin could renew some of it or I was hoping that I would at least have good skin texture and no acne, just healthy and hydrated! Please help.
You could try checking male derm channels like Dr. Mark (@dermarkologist). He does demos and product recommendations. Also Doctorly has a Q&A video where they reply how to apply products to skin/beard. They say to use water based products or take oral meds for acne if penetration through beard doesn't work; use tinted water based or oily sunscreen, remove with micellar/ordinary squalene cleanser. I'd also check out the Mixed Makeup channel's Skincare Expert playlist, for Dr. Karan Lal's night time routine. He has a full beard.
I was trying to get more info on Advanced Clinical products when I noticed their retinol face cream contains encapsulated retinol. I’ve stocked up on B of J eye cream, so I’m good for a while, but I’m wondering what you might think of the AC face cream (not sure about their retinol serum).

@@fundus5 i’m not the one you asked, but I just bought myself a LED mask. I also struggle with melasma, redness and some acne as well. Been looking into the light therapy for a few years and finally decided to get one for myself. I use green and yellow light with my skincare routine and I it is a game changer. My mask does not get any heat so it is safe in that way.

It cost 500$ Taking your time to check reviews and such is important, also if it’s FDA approved and has a CE mark. I was considering Dr Dennis Gross, but since it didn’t have green and yellow light I went for the other one.
Hey Annau, I use Timeless Vit C then put on a Timeless Q10 serum, then a great moisturiser called Solar Care B3 with 5% niacinamide (made by a company called Bernier), it’s incredible. I also use a factor 50 sunscreen. Everyone comments on how great my skin is looking and I have very sensitive skin....

You can't get tretinoin from a gp in the UK it has to be px by a dermatologist which exists a very long waiting list...... I went to dermatica online dermatologists for mine 🙏and dermatica have 20% azelaic acid cream

I've used so many products for my acne including ordinary niacinamide which caused acne on my face And used BP for reducing acne,but i got super sensitive face from it Using adapalene, salicylic acid,gentle cleansers nothing worked My face is not that sensitive but using only 2.5% BP for few minutes also burned my skin After every tried and tested I've finally started using clindamycin and azelaic acid on my face I don't think azelaic acid had done anything on my acne but clindamycin had done wonders Now i don't have any pimples But my face is full of pimples marks Now i don't want to use anything further on my face @dr dray i want to know how long does it take acne marks to go away It's already been 2 months I'm little worried...😢

Avene’ thermal water spray works wonders at getting your skin used to products. Lightly mist and magically your skin will be a-ok.
My skin can't tolerate niacinamide, however snail mucin and azelaic acid are good alternatives for my skin☺

yeah the oral medication can really help, but topical is variable
did you experience any side effects with oral tranexemic acid? how does it work? Did you just start seeing lightning of your melasma?

When I heard snail mucin was getting more love, I was like FINALLY because it had its hype quite a while ago in Asia, and yes even in South Korea. As far I know, it started was a medicinal food trend due to snails being recommended for the joints in eastern traditional medicine, which was then later used for moisturzing the skin.
Why should we use living sentient beings for our skincare routine? The cosmetics industry already has enough cruelty with testing and other animal derived ingredients
@@cecegirl4327 Because they work and unless the flesh is also used, the mucins are taken without abusing them. I should say, I thought someone like you would comment on the actual eating of snails rather than the gathering of the mucins considering that the former results in their death while the latter doesn't.
Personally, I am more against the use of botanical extracts and derivatives since it's not so practical ecologically speaking and plants are fundamental for the environment. Also, there are far too many animals or animal parts that could be used. Far too many go to waste in my opinion.
@msk it's inherently exploitative and to meet demand they must keep breeding them...continuing an exploitative cycle. I'm also not entirely convince no abuse happens at such a mass scale of production. That's almost impossible. I'm against eating snails as well. Dr dray used to be vegan/plant-based but it seems like she's moved away from that based on the skincare items she promotes. Which is really a pity since science and ethics can easily go hand-in-hand. Also just because something works doesn't mean we should use it. There's other things that could possible work and at the end of the day vanity is not something that should come before the lives of sentient beings.
@msk also animal parts being a waste doesn't mean we need to use them. By buying waste or not of animal parts you still contribute to demand for those products. They are far more unsustainable than any botanical extracts. If you'd like me to provide data on this I can
@@cecegirl4327 I dunno man, saying the use of animal part wastes is unsustainable sounds like using petroleum jelly is unsustainable. In a way worse since the use and consumption of meat is never going to be completely replaced considering it's ingrained in our hardware due to millenia of evolution, unlike fossil fuel. As long as we consume all part of the animal, we'd be needing less amount of cattles and etc, so that ought to be the mindset we should promote than completely ceasing animal consumption. This is frankly an "original sin" of a sort for the west since the wasting of animal parts was aggravated due to the industrial revolution, colonialism and culture of not using all parts of the animal.
Pork skin and starfish are good sources of collagen by the way. Pork skin derived collagen helped my eczema and acne prone skill well during my childhood, and snail mucin is great for soothing my irritation. Oat extracts are great but not moisturizing enough and centella asiatica and tea tree extracts in high dosage seem to backfire and cause irritation. So perhaps I am more biased to using animal parts and derivatives. And botanical extracts can be more sustainable, but that is so because people are trying to do it right, I say. Similarly, I stand by that animal derived products are fine as long as we do it right.

Interesting. If I remember right, when I tried an online subscription compounding service (can’t remember the name, but it’s quite popular) I think this is what they gave me. But they also put clindamycin in there and I didn’t want an antibiotic. Maybe I should try it again and just ask them to exclude that because after years of trying, tret still makes my skin a mess.
I once read that azalaic acid is ment to be used for one year as max, then make a gap.

Well, I don't know, I just heard that. I will be using from tomorrow 15% and I use .1% tretinoin already. I did use Skinoren 20% but it was too much I had sever acne, but I was so irritated, and I use peroral isotretioin that didn't cause any topic irritation, it desteoyed my sebaceus glands from inside. Anyhow, that is what U have heard to make a pause.

Hard to say. You definitely get anti-aging benefit by wearing sunscreen though

I would like to see a video on your thoughts about how much importance we should hold FDA approval to. There are tons and tons of drugs and products that were and are FDA approved that lead to cancer and other issues. FDA approval doesn't have the same level of trust as it once did, and with something like skincare that's even less regulated, is there something else we can look to for safety of a product?

I used to use niacinamide but now it irritates my skin so I stopped... But I do have dark spots and acne scars... (I use tretinoin so i don't really have active acne anymore)... What product would you suggest that would help me with dark spots and won't mess with my tretinoin?
Interesting Azelaic acid Doctorly also think underrated. Next time I go to another country will try to get the Azelaic acid, right now I’m using the 10% from ordinary. And I started Tretinonin .25% for my mom and .5% for me because my mom has never used retinol.
I also got the Ominilux for my mom and I to use. And we started IPL laser via groupon and my also upgraded to BLP laser which burnt her skin but after that came off her dark spots did look lighter. The IPL did nothing for me but I refused to turn up the intensity for fear of burning my skin.
Next year we will try Clear and Brilliance laser also via groupon which a friend tried and thinks it was great for her.
My goal is to help my mama improve her skin!

Hi Dr.Dray - I am 31 and trying to be preventive with aging but also still dealing with adult acne, I want to incorporate a LED face mask and wondering if I should do a Red only like Omnilux or a combo like Qure (although the red is at 630 nm instead of 633 nm unsure how much this matters). Do you have thoughts on which mask preforms better?

I have been using retinol for almost 2 months now, recommended by my dermatologist because of my acne scars, so my skin renews itself, but I think it's not doing anything and I'm getting like comedones and tiny little acne, like skin texture. Is this normal? It doesn't sting or peel so I think it is not irritation, I'm only using 0,03% retinol so it's okay, I think. What can I do to stop this? I used to have awful cystic acne and I went on accutane with my derm following along for almost 2 years with pauses in the summer. Now I've stoped 2 years ago and my boxscar and ice pick scars are gonna be here foverer unless I do microneedling and stuff, I'm aware, but I'm 19 so not doing that now ofc and I think since I'm so young my skin could renew some of it or I was hoping that I would at least have good skin texture and no acne, just healthy and hydrated! Please help.

You could try checking male derm channels like Dr. Mark (@dermarkologist). He does demos and product recommendations. Also Doctorly has a Q&A video where they reply how to apply products to skin/beard. They say to use water based products or take oral meds for acne if penetration through beard doesn't work; use tinted water based or oily sunscreen, remove with micellar/ordinary squalene cleanser. I'd also check out the Mixed Makeup channel's Skincare Expert playlist, for Dr. Karan Lal's night time routine. He has a full beard.
I was trying to get more info on Advanced Clinical products when I noticed their retinol face cream contains encapsulated retinol. I’ve stocked up on B of J eye cream, so I’m good for a while, but I’m wondering what you might think of the AC face cream (not sure about their retinol serum).

@@fundus5 i’m not the one you asked, but I just bought myself a LED mask. I also struggle with melasma, redness and some acne as well. Been looking into the light therapy for a few years and finally decided to get one for myself. I use green and yellow light with my skincare routine and I it is a game changer. My mask does not get any heat so it is safe in that way.

@@fundus5 I have only used my mask for a few weeks and I do see some difference. I live in Norway so the mask I bought is from a Norwegian company called Silkemyk. It cost 500$ Taking your time to check reviews and such is important, also if it’s FDA approved and has a CE mark. I was considering Dr Dennis Gross, but since it didn’t have green and yellow light I went for the other one.

Hey Annau, I use Timeless Vit C then put on a Timeless Q10 serum, then a great moisturiser called Solar Care B3 with 5% niacinamide (made by a company called Bernier), it’s incredible. I also use a factor 50 sunscreen. Everyone comments on how great my skin is looking and I have very sensitive skin....
Niacinamide is so irritating for me

I've used so many products for my acne including ordinary niacinamide which caused acne on my face
And used BP for reducing acne,but i got super sensitive face from it
Using adapalene, salicylic acid,gentle cleansers nothing worked
My face is not that sensitive but using only 2.5% BP for few minutes also burned my skin
After every tried and tested I've finally started using clindamycin and azelaic acid on my face
I don't think azelaic acid had done anything on my acne but clindamycin had done wonders
Now i don't have any pimples
But my face is full of pimples marks
Now i don't want to use anything further on my face
