2025 PCPCH Model - Performance and Clinical Quality (Standard 2.A)

Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.
This session (May 24, 2024) provided an overview of the updated version of PCPCH Standard 2.A - Performance and Clinical Quality which includes a more flexible menu of quality metrics for practices to choose from and the addition of two new measures related to tracking and reducing disparities in quality measure performance.
