Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.
This session (December 6, 2024) provided an overview of the brand new special designation under the 2025 model: the "Health Equity Designation" which recognizes primary care clinics that have incorporated specific equity-focused standards into their practice. It covers these standards, how many a clinic must be meeting, the verification process, and other important details.
