Most, if not every Sega Genesis cover/arrange Gecko makes is made with Deflemask, which, while it can be finicky to use, it has a fairly good interface to make Genesis music and can even export a VGM, which then you can have it play in your own Genesis provided you have the tools for it; I'm not sure if Deflemask is completely accurate to the Genesis tho (again, it's a very finicky tracker), but it definitely works well if you're willing to put the effort for it (I, myself, have made a few FM patches with it, which is not much, but hey, it's something :V) As for the lead guitar (and most of the instruments too at that), AFAIK, they are entirely custom made; Gecko is just too good and doesn't rely on many Genesis game patches from the 80s/90s, instead making most of the instruments by himself ;)
Not a VST actually, it's a tracker; it emulates countless systems other than the Genesis, but it's Genesis/Arcade (YM2151 + SegaPCM) and PC Engine emulations are the ones that are the most used in the chiptune circle; it also emulates the NES, the Gameboy and the Commodore 64, but not a lot of people bother with those as far as I've seen

Yeah it's pretty impressive what he eeks out of the PSG.
PSG is useful for percussion, vibration sound imitation, and makes a suprisingly good classic piano sound.
Also just for making SMS songs in general.
You can use psg for some kind of melody if all the fm channels are being used by other things.
