    I worked at Office Depot 20 years ago, I was fresh out of High School, and working the warehouse. I remember my forklift trainer and certification like it was yesterday! I really enjoyed working the forks but I saw a really bad accident. Only 3-4 of use were allowed to use the forks but I ended up getting promoted out of there warehouse into Computer Repair which was what I was originally hired to do (it was a new store opening so I had to do something until the store opened). I was also a Floor Leader so if someone needed something from the back and the warehouse workers were off duty I had to get on the fork and get it. One day I was at lunch and someone needed to get a reem of paper but the warehouse forgot to leave a few pallets on the floor for the night crew. I got paged (remember those) to let me know they needed me and responded it would be 15 minutes. One of the managers so desperate to make a sale, she decided she would use the forklift and her and another girl went to the back. She got on the fork and the other girl was her spotter. She didn't realize those pallets of paper are around 800lbs and are stored at the very top. Long story, short, she raised the forks, got the pallet onto the forks, but did not angle it back. She reversed the forklift, and the entire forklift tipped over and the woman spotting her almost got crushed by the pallet. The woman in the forklift ended up needing stitches in her head as she smashed her head on the floor when the forklift tipped, no seatbelt on, no helmet on (and was fired) and the spotter broke her leg (suspended). I ended up walking into the warehouse right after it finished, they just needed to wait literally a few minutes. It was a hard thing to see honestly.

      Oh no...that would suck...glad no one was seriously hurt. People need to have a greater respect for these machines, especially the untrained!

      The driver was lucky her head was still in one piece. Easy to get pinched between forklift and ground or other obstacles when tipping without seatbelt (and training to grab the steering wheel).

    I worked at a brewery for a time years ago here in Perth, Australia. It was my first day on the job which involved mostly all fork work and I really impressed the guy who hired me. I had a lot of experience and could drive a fork lift like it was an extension of myself. I was cocky! I’ve got this!.. “Maybe I’ll be fork lift driver of the month?” I thought as I imagined my picture up on the lunch room wall.
    Cue next scene:
    In the brewery there was a special caged section that contained all the expensive spirits. All the good stuff, pallets of it. And one speed hump..
    “Just watch out for that speed hump” my new boss said, as he walked off feeling warm and fuzzy that he finally got a good ‘forky’.
    It was a simple job, I just had to move a few pallets out for picking and packing.
    So I picked up a pallet of top shelf vodka, and proceeded to take it outside the compound at my usual confident brisk pace.
    I was so busy calculating the net worth of that pallet of vodka in my head, that I forgot all else and when I hit that speed bump the whole pallet and my ego came tumbling down and shattered everywhere. There was top shelf vodka pouring down the drains like a fast moving river. It was a nightmare and the poor boss guy had to go back to his boss and explain it all.
    Needless to say I didn’t make the employee of the month but they did keep me in which was good. They ended up putting me on ‘kegs’, which one task involved bringing a 2x2 pallet stack of empty beer kegs, across a lumpy pitted asphalt parking area which a twin tine forklift and I only ever dropped one single keg. I learnt from that first disaster and humbled the f up and started really paying attention.

      With forklifts and scuba diving , if you think you are an expert you're getting cocky it's about to hit the fan :(
      30 years on the job and I was the instructor, dropped a $40,000 power hacksaw that I should have refused because the pallet was crap

    I work at Bass pro shops Cabelas and I drive a stock picker. Never turn corners to fast or jerk the steering to hard. I rammed into a barrier today and locked the machine out. Next time I will definitely take it slower even if I have alot of room. It takes one time not paying attention enough no matter how confident you are

    The clip in the soda warehouse in the beginning made me laugh. I work in one and we always celebrate someone dropping a whole pallet like that. Or when anyone drops anything.

    I'm a retired forklift technician and company owner. Many of your accidents were caused by improper equipment for the job at hand, secondly improper lift capacity not being set properly by the service provider. Forklifts should be able to handle any load within their weight envelope no matter the angle of the mast. Forks need to protrude completely through any pallet or item they are attempting to pickup. High mast forklifts are only suppose to be used on flat ground only. I had over forty years in the forklift industry. Keep up the good work.

    Reminds me of when I was 10 years old, and my dad gave me a lesson on a Hyster Fork Lift at the company he worked at, where they used the fork lift to lift laundry detergent powder (very heavy) two or 3 pallets at a time to about 30 feet as the building had no lifts to the upper storage floors in the building. I did the lift perfectly (with strict supervision - (he was a FL trainer)) but remember him telling me about a guy getting fired for screwing around and nearly killing himself.

    My dad witnessed a bad accident once. The driver got a bit too far down a steep hill. The forklift lacked the power or traction to stop moving down hill. The lift built up a lot of speed before turning sideways and flipping over.
    The driver tried to jump but the counterweight landed on his head. The driver's head did not hold the forklift up at all. His head was crushed flat.

      Thats why drivers are trained not to jump. And fasten seatbelts to make sure you don't panic and jump anyway.

    You have to keep your head on swivel at all times, and you cannot let yourself be distracted for even a moment. By biggest fails have been from momentarily "spacing out" thinking about something else, losing my concentration, and forgetting to look behind me when backing up, going down and aisle and not seeing a forklift backing out of a lane, coming out of a side aisle or coming around a corner, or not watching clearances going in or out of lanes. It may seem obvious, but the human mind wanders easily and you may not even be aware of it until suddenly there is an obstacle that wasn't there a moment ago. Even drivers with years of experience can become complacent. I treat every day as a new day to challenge myself to be the best, safest driver I can be and practice everything I have learned through training and my years of experience.

    heh looks like the clip at 1:00 is at a home depot. they use the wrapped pallets. indicator is the orange scissor baracade in the left. he probably couldnt come straight back out due to how crowded the stores make their lawn & garden area

    I used to drive forklifts I work and the worst thing I've done is get one stuck in a pothole because it had no suspension whatsoever and an open differential. They brought out another forklift to push me out, and 5 minutes later I was stuck in the same pothole again.

    Something I can add that I haven't seen in the comments yet is know where your load is at all times. It's easy to be careful and watch where you're going but end up gouging one product with the corner of what you're carrying if you aren't aware of your load. I see it happen all the time with the new hires

    my biggest issue is getting the forks level when putting pallets down, sometimes i have them slightly tilted and they grab the boards of pallets, 90 degree turns are tricky in tight spots and un-even ground

    0:38 Not only did he hit the forward tilt instead of lower, he wasn't even inside the bottom pallet. He was under it.
    0:55 This guys forks were already too short for this pallet and he wasn't even all the way in back to the heel of the forks. No wonder it dumped

    Could you demonstrate how Crab steering is operated in a Terex AC-100 or AC-50 ? Can you put the wheels in any angle ? or are there predefined angles ?

    When i drove forklift for a grocery store near me our backroom would get so clogged with carts and vendor supplies that if you were traveling with a tall load say a pallet of frozen goods off of a truck which is common for me. Youd have to raise the load up above these carts to make a 180 and drop the pallet to be put into the freezer. Sketcb stuff but was a great way to learn spacial awareness on a forklift. Scared my coworkers when i did the raised 180 but i felt it was safer than driving with the tall load in front and potentially hitting someone or something.

    I run forklifts only had two mistakes: First one I was backing up our fuel & grease truck coworker was on forklift. we were both backing up and I ran into the side of him. Second story I had a spotter with me I was setting black drain pipe onto our pipe rack 25 ft high. The end wasn’t clearing he was underneath it with a bar trying to get that end up, It fell on his head. Thank god he was ok just sat out of work all day.

      I had one mistake (not really my mistake but I should have been more careful) and its a mistake I'll never forget. I actually dream about it still even though I haven't worked a forklift in 19 years! I was pulling heavy pallets from the truck to the warehouse to be store. We pulled the pallets out with the heavy duty forklifts and then stacked them with a mid sized Dae-Woo Forklift. These bigger forklifts could crab so it made getting into the warehouse very easy. We had just finished pulling the pallets out of the truck when someone told us there was one more pallet in the truck that belonged to us. I said I'll go get it and asked for my buddy to spot me. So we went out to the truck and I got the pallet loaded in and was having some issues with how the pallet was stacked so I needed my friend to help. So he came in to help guide me and brought a flash light to help me. Well we didn't realize it but the Store Manager finished signing the load manifest and I guess didn't hear the forklift and he dropped the loading bay extension (its a metal piece that goes from the loading bay into the truck). My spotter was walking backwards helping me out and I was reversing, and all the sudden I hear, "Oh f___k" and I felt like I just fell through a hole. What ended up happened he walked straight backwards off the truck onto the ground because he was expecting the extension to be there and I backed right into the hole. Luckily for us both he wasn't hurt and the only just my back wheels went off before I pushed the forks forward (I did so to put some weight forward to keep me balanced from tipping back). The manager ran back to us and was very apologetic to us but at that moment I was a scared shitless 17 year old!

    Picked up 2 bundles of steel on the Toyota today thinking it could handle it but as soon as I dropped the forks gravity just sucked the bundles to the ground. I grabbed the roll cage and held on for the ride. It was not that bad but I’m glad only the truck driver saw it. We both laughed.

    Ive done a few moments of forklift driving. not an impressive amount of hours. but enough here and there. But when my friend who was a rookie forkie get bumped and dropped a bloody load of pallets on a thin crate of gaskets.
    I laughed so hard and another guy was as well. friend didn't laugh but he immediately turned the forkie off. suddenly one of the supervisors walked along and, in a super serious manner, said who is going to pay for that and that he doesn't take excuses. All i did was calmly explain my friend's goof up and that he only got on the forkie the third time now. visor was looking at my poor friend the whole time. I explained the crates were supposed to be moved somewhere else because its already in a bad spot. something the supervisors were in charge of.
    not many liked our guy anyways but i didnt mind. after we cleaned up and got someone else on the forkie i explained to my friend that your gonna drop a whole lot of mistakes no matter what. just learn to pick em up and carry it with you. everyone at our workplace for the time was super nice. no serious arseholes. exept for that guy. Noone should ever be mean and rude to people who are learning new things on the job and make mistakes. that takes away their ability to learn and focus.

    Military, driving an off road 10k, I was off loading patriots off a truck. The guide up on the truck said I was good, confidently lifted immediate yelling, waving and signaling me to drop it. Turns out the tip of my forks was just under true opposite patriot and had picked up and almost off the truck lol

    American pallets and racking leave something to be desired... We use mostly big hardwood pallets in Australia (Chep) and sometimes big Pine ones (Loscam) Both the pallets and the racking are very sturdy and designed to lock in together. I had to handle American pallets at a small company once... Never again!

    1st Video... It's not the ground he is driving on, he should be driving in reverse so you can see the uneven ground ahead and see if there are any hazards!
    2nd Video... That was not the correct travelling position. The forks should be positioned lower leg height between ancle and knee tilted backwards.
    3rd Video... Yes straight out was required but the load was far out from the guard. Not only the centre of gravity was way to far forward on the forks but it would reduce the lift capacity of the truck which could also be dangerous.
    4th Video... Three things, firstly again the forks are not fully under the pallet. The pallet the load is on are notorious to not be especially robust, a blue pallet would proberly take that extra weight at the end from the forks being not inserted properly provided it was undamaged. Also there was no wrap on the products, when you wrap it it (to a point) makes it one mass and the shrink wrap could stop product falling off.
    Lastly though, although the pallet feel and people laughed everyone that I could see knew of the situation and was standing back in a safe location. It wouldn't be possible to safely get a person up there to restack so in some ways this was the safest thing to do. The forklift driver would get some stick for this afterwards though.
    5th Video... Agreed with the roll cage. Do not jump from a forklift ever, there should be a roll cage and a roof. If it goes over you should have your seatbelt on which will hold the lower part of your body and use the palm of your hand to push up on the cage to hold the top part of your body.
    6th Video... This is not even a forklift, its a pallet truck to be used on a completely flat floor. This was asking for trouble.
    7th Video... Thats a forklift that will attach to the back of a truck. Yeah they knew what was going to happen.
    8th Video... He jumped out... he was lucky he wasn't crushed however if he hadnt notice the forks on the forklift behind could have impailed him. Not good.
    9th Video... Two people on the back is not going to make any difference.
    10th Video... Forklift should have been reversing so they could see where they were going! Imagine if that was a person!
    11th Video... Don't stand near a moving forklift at anytime!
    12th Video... Providing it wouldn't have been overweight then carefully take the top 3 off in this instance. Its the bottom boxes that have failed! Otherwise grab another forklift to stop beside the stack with the forks at full height to rest the pallets on the mast while the other forklifter lifts them down. (Dependant to weight and type of load)
    13th Video... Agreed
    14th Video... Not sure if I mentioned before but why is he balancing the load on the end of the forks?!
    15th Video... Agreed
    16th Video... Agreed
    17th Video... Yep, need a proper forklift with longer forks, not a pallet truck for this.
    18th Video... What was the guy thinking?! He didn't even try to save it!

    One of mine accidents wasn’t really an accident, it was more of not knowing what would happen in the situation. I was on a big Hyster and the area that I was dropping the load had big timbers where I could set the load down safely and back out from under it. The dirt had been blown away by the winds so basically I was driving into a deep wide trench. I set my load down fine but I didn’t realize that I had dropped my forks too low. Because of this trench, my forks went down below the point of no return and as I was backing out I left the entire fork assembly in the dirt! I realized something was wrong right away and stopped. All of the hoses were still intact and where they needed to be. I was sure scared to go talk to my boss! It all turned out OK!

    So many accidents can be attributed to improper use or poor training . Some guys just rush or get complacent.

    The kid at :37 didn't have the pallets locked in. If he had it through the slats the bounce wouldn't have made it tip over. It would have stayed. Lost center of gravity. Or...am I wrong lok

    The most important thing in the cab is, that little flip book, just to the right of the steering wheel. If you dont see and understand it .You fail your precheck, get fucking out.

    The last one should have boomed in and down at the same time but it wouldn't have been easy. Theat Manitou would have had solenoid activated hydraulics(on /off). JCB all day long.

    I wish you had done more than just laugh at that last video, there is lots to learn from it. Apart from "what could have been done to avoid flipping backwards", I would also like to point out (unfortunately that part was cutoff) that after the loader was lying on its back (basically the whole machine on just the boom), people were walking around next to it, who might have been crushed if the machine had then tipped over sideways.

    Telehandler at the end should have deployed their outriggers before lifting up that high.That should never have happened. Source 19 years behind the boom.

    Prevention of these accidents begins with selection of a competent operator. I could train someone for 3 weeks and they'll still go and fail and I could never train another and they would never fail. You just can not train someone to be "intelligent".
    Training someone with low intelligence is difficult at best and the company better have its insurance current.

    Fork lifts like any other tool, should be treated with respect. It can help but, it can also kill. Loading or unloading a truck on an inclined and uneven surface in uncovered wet weather, means you mentally measure every action, every second in three dimensions plus the incline and uneven surface, then factor in your speed and torque and the next pothole, remembering the tight space you have, and some idiot co-worker who doesn't remember what the lock out chains are for, slips under the chain and I am glad I am looking in at least 5 or more dimensions at any given time.

    I lost an I-beam roof post once, behind the mast of a Cat ride on 6000lb lift... hit it full tilt.. about 12mph... that hurt like a bitch... bent the tip of my fork up like an elf shoe...lol knocked a fist sized divot of concrete out of the pole protecting cement post around the pole... That was 20 years ago! The concrete is still there, I still drive past it sometimes, I still remember how high the back of my hoist came off the ground! It actually bent the frame of the truck, literally shifted it

    It seems you two are not too familiar with driving forklifts or the safety aspects. The kid, with the pallets in the air, dropped them because he was traveling with his load in the air, not because, "he hit the wrong button." If he was properly trained he'd pick up the load, back up enough to clear and lower it till just above the pavement then proceed to where ever he was heading. Then even if he did tilt forward by mistake it would not likely have been a problem. You may be trainers, but that was a very important miss neither of you caught.
