My hospital just opened up a "short stay unit" last summer. All the patients that are ED obs patients that we don't want in the ER anymore but can't get admitted go there. They have 18 beds and it has tremendously helped us clear out our ER on those already hard days of admission holds in the ER. It's run my ER docs and PAs but like stepdown type nurses.
After ICU they send you to DOU because you need more monitoring than on the floor but less than ICU. My BP fell under 50/30 and so I was in the ICU at the University Hospital. When I went to the Catholic Hospital and my BP went 50/30 they just left me in overflow. I wasn't mad. It was the best sleep I ever had in a hospital! No one bothered me.
They can also do special drips that MS can’t do. They just can’t titrate the rate has to be fixed. We had ours for about 9 years but with Covid it disappeared and we don’t have enough nurses to staff it.
I think DOU is theoretically wonderful for all of those fragile patients that the services don’t want to clear for floor. Or the patients that are teetering on the brink of ICU vs med/surg. For instance that sick DKA’a. We end up holding them in ER for hours and hours post diagnosis until their anion gap is closed. For some it can be like 12-18 hours. But they are too much work for a floor nurse.

Interesting! I work Med-surg and feel like bottom of the nursing totem pole (based on recruiters who state that we don't have any transferable skills). I never thought there was a unit "less than" Med-surg.
D.O.U is mainly used for people who might have life threatening stuff or might rapidly get worse but at the moment they are okay, for example someone who fell from the stairs, did not broke any bones or anything but he's under anticoagulants, therefore he will usually be under observation in D.O.U for around 8 hours. That's pretty much the concept of D.O.U it's not just in ERs but in other departments too

I loved this. Mom was in DOU a few times cuz of heart. Last time was an assessment of heart failure and she she will pass away by tomorrow. That was Wed when they sent her back to AL. She passed on Sun nite at 88 yrs old. As always, truth in your humor. I miss her but glad she is no longer suffering from post polio afflictions.
I had to go into the er and eventually the hospital and surgery just over a year ago. Im still not sure why noone else thought that bloody stools were something to be concerned about. Not to mention lots of time spent in the bathroom and extreme pain. Im still in recovery. This channel helps me get through the day. Thanks Steve

All DOU nurses have pediatric life support training and many of them are nationally certified in critical care. The emergency department observation unit (EDOU) is a specialized unit designed for efficient, ongoing medical treatment, assessment, and reassessment of patients before the appropriate decision can be made to either discharge or admit.
I work in a small hospital of 400 bids. I've never heard of a DOU. In my hospital we have something called the CDU, clinical decision unit It's for patients that need looking after for less than 48 hours, freeze up space in the ED. We also have PCU, progressive care unit or IMCU, intermediate care unit.
Never heard of a DOU. Worked at several hospitals for 10+ years and that's a new one to me. Step Down was in between ICU and Med/Surg/Tele. Also called a PCU. Overflow was extra med/surg pts. DOU sounds like the name of some floor trying to sound bigger than it is.
Was a critical care RN 1976-1999, it was a thing in early 80’s. IMCU (Intermediate Medical Care Unit) or DOU. Intermediate care, so higher than med/surg, less than ICU/CCU.
My dad spent a couple months in ICU, it was great to have DOU because he simply wasn't well enough for general care. Some of our best nurses were ICU and DOU

In Australia we have SSU: Short stay unit. Basically, it's overflow from Emergency and they can stay until discharge or admission to wards.
