13:00 IIRC, there is actually a loading tooltip in ME2 that says you can continue on in ME3 _if you survive_

ME3 won't import from a savegame where Shepherd is dead. (Shepherd dies if there aren't at least 2 teammembers alive)
This game did not recruited Kasumi, but she was the one who would had died on the approach instead of Legion. (There is an order in which teammates are considered for it which will be influenced for example by who you take with you to fight the Oculus. The people with you there are safe)
You can actually start ME3 with a new character (ME2 too), it's just that we love to keep the continuity so we import.
Normally anyone disloyal if selected, will either die or cause someone to die. The one exception being Miranda as the diversion team leader during the biotic bubble approach.
The deciding factor for the final team's death is indeed disloyalty. That's also the only reason why the character who escorts the crew and Chakvas can die.

Shepard: gathers all badass in the galaxy in one team. And then gets them all killed anyway. Perhaps Saren was right- at this point joining the Reapers seems like a logical choice...

When I was doing a bad playthrough in ME2 I had it where almost everyone unloyal so when I sent unloyal Garrus to lead the second wave his death was so sad and what he says is even worse.
“Sorry Shepard. Wont be with you till the end, snipe one for me will you?”

If shepard’s crew isn’t there at the end to pull him up… the game ends. You can’t carry the saved game over to 3.

I just left this note on the Arrival episode, but just in case it raises its visibility for you, here it is again.
A quick note for ME3 because Arrival had it happen and it will happen on a few occasions in ME3 (including the opening):
When the game gives you a new companion in the middle of a mission, PAUSE and take the time to assign the new companion's skill points, even if it is just to hit the "auto level up" button. If you don't, the new character will only have a single skill point assigned and therefore be far less combat effective than they otherwise could be.

I did a pure chance suicide mission. So randomly selecting squad mates to do the roles. Left with 3 members and Shepard alive. Garrues was a one army holding off the collectors while we fought the reaper embryo.
It was an early plan to be able to import a ME2 save with Shepard dead with you playing as a different character. But they didn't use the tech for that type of variety back then

    I........
I have never watched this ending before this. I never wanted to acknowledge it as real. Right now I'm feeling extremely nauseous and I legitimately cried at many points.
The very first time I played these games about ten years ago, if my memory serves me right, I lost Kasumi during the first door closing cutscene (like Jacob was lost in this one) and also Mordin during the last part of the mission when they were holding the Collectors back. This was the complete trilogy box on PS3 and it didn't include all the DLCs of the series, which means I didn't have Zaeed with me. My first experience with him was with the Legendary Edition.
After my first playthrough, I assumed one singular playstyle from which I have not strayed ever since. No matter how many times I play through these games, I will always have the same outcome, with only minor trials with dialogue options which won't affect any greater scenes. I am not interested in experimenting with any other outcomes of the games possibilities. So it's interesting to watch these possible outcomes through other people's playthroughs, although often many of them mirror my own 😅

If you don't have a save that you can import, you can still start a new game in ME3. Start the game from scratch, and it will present you with an interactive comic which tells the story of ME1 and ME2. The comic allows you to choose the main decisions from those two games. Some options are not open to you (for example, I believe it forces you to choose between Jack and Miranda when they have their confrontation on the Normandy), but it has all the main ones.
Can't really say more than that without giving away spoilers. Suffice it to say that ME3 is a lot better if you DO import a save from ME2.

If you don't do a DLC, the events will still have happened, but you just weren't there to help/change the situation. Like with the Arrival DLC, if you didn't do that one, the Alpha Relay and Batarian colony would still be destroyed. But it wasn't you who did it, the blame will be on an Alliance's Division. They do things like that with most DLC's, but the outcomes are usually the more bad ones. So the good outcomes can only happen when Shepard is there to help

Dunno if anyone has mentioned it yet, but there's a Mass Effect anime called Paragon Lost that takes place at the same time as ME2 and gives some extra back story for ME3 if you're interested.

So if Shepard dies and you try to open Mass Effect 3, you cannot transfer that campaign. Shepard's story died at the end of Mass Effect 2. You would have to get a variant "bad" ending where at least Shepard lives to transfer over to Mass Effect 3.
