  • Dr. Lera talks about heart murmurs in dogs with Phoebe, a six-month-old Boston Terrier puppy.
    A heart murmur is an abnormal heart sound, usually heard by listening to the heart with a stethoscope. A heart murmur is caused by turbulent blood flow within the heart. Sometimes a murmur is determined to be 'innocent' or 'physiologic', while other times the murmur is determined to be pathologic or caused by disease. Pathologic heart murmurs can be caused by a structural problem within the heart (i.e., cardiac disease), or can be due to a problem that is 'extracardiac' (i.e., not caused by heart disease).

    My Jack Russell had a heart murmur from 6 months old. I was told she would probably only live for two years. We kept her fit and gave her a healthy diet and loved her to bits. She passed away from her murmur at 14.5 years old. Our new dog has just been diagnosed with a murmur, I think murmurs are common and many need no treatment, just a healthy lifestyle, after all they have survived with it so far. Our vet just said at some point we might like to get her tested to get a specific diagnosis. Our new dog - a rescue panics and cries if we leave her with anyone, leaving her at the vet for tests might give her a heart attack, she will stress and panic.

    Both of my dogs have a heart murmur. Ones 11 years old, and the other one is 9 years old. They say that it’s not concerning right now. My 9 year old also has, Cushings, an enlarge liver, and transitional cell carcinoma that is 2 cm in her bladder. They don’t want to do surgery because of her mild heart murmur and until her cushings is under control. She was diagnosed with all of this at the same time, although we take her to the vet 1-2 times a year. It shocks me how fast a dogs health can change so quick no matter how healthy you try to keep them

    Hi, my 12 week German Wirehaired Pointer has got a hear murmur. 2 weeks ago the vet said it even may not be a defect but a blood flow thing. However, on Monday he said it's a grade 1 murmur. The pup seems fine, running and training well. She is also a bit skinny. Is something I should be worried about in terms of her future? She comes from working / field trail dogs and I intend to run her through filed trails and tests once she is big enough. Is she doomed? I have so much future hopes for this pup. I waited for 10 years to get this breed and from this particular breeding. The breeder even said he had 3 cases in his litters and all of them are fine now. In fact, one of those pups won a master level filed trail test last year.
    Thank you.

      Not necessarily, many puppies outgrow their heart murmurs and a grade 1 is actually the lowest grade possible, out of 6. The best thing to do is to continue monitoring with your veterinarian to see if they outgrow it or if it gets worse.

    My dog a history of heart enlargement in 2019 and was prescribed pimobendan for one and a half months. Recently, the doctor conducted an ECHO and ECG test on her heart, which revealed B1 Grade Mild heart disease and MV is thickening and her heart valve is not closed. Now she has been diagnosed with ovarian remnant and has a left uterus stump with echogenic intracavitary fluid. The doctor has suggested surgery. Currently, she shows no signs of ovarian remnant syndrome. However, I am concerned about whether she will be okay with anesthesia due to a previous reaction she had when giving birth. In 2018, after undergoing surgery, she experienced muscle spasms and some fluid from her nose following anesthesia.
    Hence, I am hesitant to proceed with the surgery if she may be at risk. Please kindly response my question

    • @DrLera
      @DrLera  3 ปีที่แล้ว +1

      It depends on how much certainty and information you want about what's going on with the heart. if you want to have as much as information as possible then yes I would recommend that you go see a cardiologist. If it was my dog I would go see a cardiologist just so that I can get a baseline as to where the heart is at. They most likely will want to do an ultrasound of the heart as well so they can give you as precise information as possible. When you talking about costs it's going to vary from City to City from clinic to clinic so that's hard for me to tell you. I can't imagine that you would be spending anything less than $400 to $500 at minimum for the consultation with the cardiologist and the ultrasound

    Our dog was diagnosed with a heart murmur when it was very young, and the doctor says it is mild and does not need medication or further analysis yet (I do not know it's level on the 1-6 scale). We live in Minnesota. Would you recommend against taking our dog hiking this summer in the mountains out west? Not sure how the altitude would affect her/her heart murmur. Would the altitude affect her? Would it make her heart murmur worse in the short or long term? We took her to about 6,000-7,000 feet of altitude with a little bit of hiking last summer and did not notice any problems with her during the trip. Thanks so much!

    • @DrLera
      @DrLera  2 ปีที่แล้ว +1

      It depends on what your level of concern is and how much you want to invest into confirming that the dog does not have any major heart issues. Usually I would tell people statistically speaking the heart is probably going to be fine. But if you want absolute certainty then you would need to do echocardiogram which is an ultrasound of the heart

    My 9yr old chihuahua mix had an audible heart murmur whilst under anaesthesia for dental cleaning. When awake and back to his normal self, the murmur was gone. I was quite content this was a drug-induced murmur, but my vet kept pressuring me to send off some blood for proBNP. This is an expensive test, and due to age, I can no longer get insurance for him. I feel the vet is overly cautious due to his breed and her liability, but the dog shows no signs of heart problems. No cough, no painting, no collapse, no wet chest, no infections.
    She says if the test comes back inconclusive, she'd like him to have a scan. That's another £700. I'm really not feeling this is the best option for my dog, as he finds the tests very invasive. The treatment would likely be beta blockers or other rate/ rhythm control meds, but it could equally be nothing and just observed.
    Personally I think if there is something there, it's exceptionally mild, so the outcome would be simple monitoring anyway. Meaning over £1000 and stress for my dog just on investigations for no outcome. Thoughts?

    • @DrLera
      @DrLera  2 ปีที่แล้ว

      The tests are not invasive as there is no or a very little pain involved. The only pain involved would be collecting the blood sample. The United States age is not a restriction on getting pet insurance there may be certain things that the pet insurance companies may not cover because they would be considered a pre-existing condition. You'd have to do some more research as I would also be interested to see what the final outcome is in regards to your questions about pet insurance. That's great news that the heart murmur resolved once the patient came off anesthesia. If you wanted to be proactive then performing the test would be a good idea. But if you want to take a conservative approach that's an option as well. You are correct most likely that the veterinarian wants to make sure that you know your options and what the textbook medicine would be. Hope this helps. Have a blessed day

There are possibilities that it may be benign and to help work out the differences you may want to talk to your veterinarian about doing you chest x-rays and an ultrasound of the heart

    I just took my 9 year old Chihuahua/Bull Terrier. Vet said she has stage 3 heart murmur and they prescribed her Pimobendan and Enalapril, my dog doesn't have those coughing fits but she does have labored breathing when she sleeps.. I read the side affects of these medications and I don't like the sound of it. Should I really give her these medications? I'm worried

      We had same situation with our Pomeranian. Due to the age (9 years old) we decided not to do the meds and just enjoyed the time he had left before the coughing was so bad that we had to have him put to sleep (like 6 months later). The drugs cause kidney and liver failure.

      Some dogs are born with memories and they will resolve on their own other dogs have murmurs and they don't never resolve but they also never turn into anything serious. The next step most likely would be to have chest x-rays and ultrasound done of the heart

    • @DrLera
      @DrLera  ปีที่แล้ว

      This is most likely tied to mitral and tricuspid valve disease. It varies from patient to patient some will not have any major issues related to the disease and others will go into congestive heart failure. So yes it can get worse, but not necessarily always the case. Will it shorten his life, maybe.

    Hello, can it be risky to put my 13 year old dog with a heart murmur under Anastasia in order to remove teeth tarter? What would u do

    • @DrLera
      @DrLera  ปีที่แล้ว +1

      The number by itself does not make it risky to put your dog on her anesthesia. You would have to further evaluate the murmur with chest x-rays in an ultrasound to properly evaluate. I personally would not put a dog on adrenaline to see it just to remove dental tartar. That being said there may be a decent chance that your dog has. Don't please and therefore may need more than just teeth cleaning. Depending on how calm and nice your dog is there are some services that will do a scaling and polishing without any anesthesia

    • @DrLera
      @DrLera  3 ปีที่แล้ว +2

      Dogs can live a long time with heart murmurs actually they can live a normal life with heart murmur. The question is what is causing the heart murmur and to get to the bottom of that it's gonna require you doing an ultrasound of the heart to really understand what's happening. Cavaliers are more prone to heart disease so I would recommend doing the ultrasound if you can swing it

My 9 year old also has, Cushings, an enlarge liver, and transitional cell carcinoma that is 2 cm in her bladder. They don’t want to do surgery because of her mild heart murmur and until her cushings is under control.
