      @@BajaDeezNutz cause instinctively you're lead to believe bleeding = bad. As are all people. Once you get into medicine thats no longer the case though and you get desensitized to bleeding tissue. I will however never get use to puss, Im in med school and puss is just a big bowl of nope for me and that will never change.

    The similarities between this injury and Joe Theisman's are eerie.
    -both had compound spiral fractures
    -both happened on the same date
    -both were playing in games for Washington
    -both games had the same score
    -both happened near the 40 yard line
    -both were hit by a 3 time DPOY
    -and just to put the cherry on top, Joe was at the game when Alex got injured.

      1) spiral fractures are usually always bad fractures due to the asymmetrical shape of the break
      2) Thiesman played for the redskins so him being at the home game isnt strange at all
      3) a 3x DPOY just indicated an elite/strong player caused the injury which would be expected as a little dude wouldnt be expected to cause such an injury
      4)17-7 isnt that unusual a score in the nfl infact its about as standard as they come
      5) the date is just one of 16 game days that the nfl calendar falls on annual game wise so again its not that shocking. its a merely 1/16 shot.

      His leg looked like a cadaver's leg in an anatomy class. As if the instructor peeled away layer after layer to show the students. And some people will say it was god and a miracle. Um, actually it was an amazing medical team with several expert doctors and nurses who used years and decades (and centuries really) of accumulated knowledge. That's what saved his leg.

    Alex Smith is honestly one of my all-time favorite players. He came to my Chiefs and, along with Andy Reid, helped the Chiefs become perennial playoff teams, and I was sad when he was traded but glad he got a big deal in DC. But then, the injury. That sucked so much.

    Hey Brian. If you look at the end of the Documentary when he was running in the grass with his kids. You definitely can see in his gait he had some difficulty dorsiflexing and clearing the ground completely. I think the doctors did a good job controlling the necrotizing fasciitis from spreading because that is not easy to do my any means. Seems like they had to be aggressive in the early stages. Klay has a documentary coming up. I know you will be on that one! Good work brother!

      Do you guys have any advice for someone with chronic knee dislocations? I've dislocated my left knee 15-20 times and my right knee about 5-7 times. My ortho said it was just how my legs are built, tall and skinny. And he didn't make me feel like surgery would help much either. I pretty much have given up on sports. 25 yrs old now 6 ft tall, 150lbs, and it happens less frequently now but only because I'm less active. I did physical therapy back in high school, i dislocated them alot playing soccer and basketball back then. I can't seem to build muscle in my left quad, and when i try to work out my legs alot, they always seem to dislocate eventually regardless of how much muscle I've built up. And then the quad muscle resets so i lose any progress I've made. I'm not overweight, which is what I've found usually leads to knee dislocations. Any exercises that could help or am i just fked? Lol i do squats, lunges and wall sits. Usually body weight stuff. The recommended easier leg raise exercises on my back and flexing the quad is uncomfortable and makes me feel like the kneecap will go out.
      I know I'm late haha and yall probly won't read this novel, but any advice is appreciated.

      squanchy squanch well I don’t know your medical history or current presentation. I would first advise seeking another orthopedic doc not a surgeon. She/He could direct you with exercises themselves or maybe another PT.
      A lot of the times knee issues can be from very weak hips/low back, weak core stabilization, and mis-alignment with feet like excessive pronation/feet flat. I know some people are born with hip/knee angles that aren’t always ideal and tend to result in injuries but targeted exercises will help.
      I would look to focus on a lot hip exercises and feet/ankle strength exercises. there are tons you can even find on TH-cam.
      Another component is balance. You need your muscles to help keep proper joint alignment and improve your body’s ability to recover . Most people just think pure strength in one plain is enough(squats lunges deadlifts and so on) but you will need to focus on multiple planes of movements but also have a trained eye to watch you perform them correctly. That’s what I suggest but getting with a very reputable and trustworthy PT will help a lot. Good luck and hope this helps.

      @@JeremyGiese thanks for your reply man. I do work on my balance quite a bit as well as stretching. And yes the hip muscles are crucial in my case but i figured just walking was the best for my hips ill have to look into workouts for them! I only know of one that combines hips/balance. The left knee is just extremely loose from multiple dislocations of varying severity and i have a very difficult time building/retaining muscle in the quad. Ill be fine for months at a time and then bam out of nowhere im walking and it slips out of place. But i appreciate the advice. I think more PT or at least more targeted workouts at the gym would be beneficial.

    While I don’t expect Alex Smith to play another snap in the NFL his recovery has been nothing short of remarkable. Kudos to his doctors for being able to preserve most of his pre-injury quality of life.

      Not only did he come back, but he went 5-2 and won the division for us.This includes a 4-game win-streak and defeating the 11-0 Steelers.

    Hey Dr. Sutterer, I'm so curious how doctors control the amount of blood loss while removing so much muscle and skin from his lower leg?

      I'm obviously not Dr. Sutterer but necrotic tissue doesn't bleed like normal tissue. And that exact method is used to determine viable tissue is if it bleeds normally or not. Once you get to bleeding tissue you should stop debriding. And while doing the debridements you have cautery tools to stop any small vessel bleeding as needed. Once you stop the small bleeders the tissue will be fully functional without any further blood loss, which should be minimal if done correctly.

      They constantly cauterize all the small vessels that bleed as you approach the healthy tissue. Small vessels and capillaries are unique in that unlike large vessel they can pretty much grow back on their own. Ie: part of increasing cardiac output and VO2 max when aerobic training has to do with just increasing capillary and small vessel density(which allows for the transfer of oxygen to target tissues more rapidly and efficiently). Small vessels can grow and expand in the body to increase blood flow where as large vessels are anatomically set in their natural locations. So cauterizing all the small bleeding vessels will only temporarily halt the flow of blood before the body heals and regenerates them

    Wow, thank you for doing this breakdown. I knew Alex Smith had it bad after hearing about all the surgeries and sepsis and possibility of amputating the leg, but WOAH I did not realize how brutal limb salvage was. What an amazing recovery Alex has already come through and continual prayers for complete healing!

    Dr. Sutter, I really appreciate your detailed frame-by-frame analysis of the nature of the initial injury and the subsequent infections, treatment, prognosis, etc. Well worth watching. I prefer this to the actual ESPN documentary. Well done.

    Thanks for all this indepth work Dr. Shutteter. A. Smith is lucky he did not lose his leg. I am amazed at the complexity of this surgery to transplant muscle and skin in order to repair these tissues as much as possible. He definately wont be the same player, but i am just shocked at this man's metal strength. He made it back and is playing well. Trully unveliebable!

    He came back, played and even started. UNBELIEVABLE Retired a few days ago good for him what an accomplishment because I’m sure they were trying to talk him into cutting that leg off to save his life from that infection pure craziness

    This informative breakdown was better than the documentary itself, I didnt realize it was necrotizing fasciitis, he is lucky to have his leg, makes you realize we all are lucky to have simple abilities we surely take for granted

    Wow, medicine is fascinating they essentially made a tibialis anterior and anterior musculature from his quads and gastrocnemius. Wow that rehab had to be something else. Great video!

    Thank you for sharing your medical perspective on his injury. I recently broke my left tibia, fibula, and calcaneus. For repair my surgeon used intermedullary rod placement on the tibia & ORIF of my malleolous. So for me this was inspirational and very informative :). I'll be sure to check out the documentary too!
