I'm pretty sure Otto knew that Peter was Spider-Man the moment he saw the torn costume. He probably was just pretending to be ignorant by saying Peter is Spider-Man's tailor or tech designer.
Peter was devastated to see Otto become a supervillain. Otto believed Peter would turn against him, but it was Otto who betrayed Peter since he cares about revenge on Norman.

@No.1 Spidey-Fan That is STILL NOT normal! For a 17 year old to loom older than a 23-24 year old! People can make up crappy excuses, saying that “His powers helps him age slower!” Or some other shit, but there IS no excuse as to how wrong it looks that Miles is Older than Peter.
You can also view all this after creating the antidote in the lab too, the game tells you to exit but you can view all of this during the end of that mission.

The lab actually is in this state just before the final fight. I believe that after you finish the story it just stays like that forever.
