On plasma exchange: Formal TPE is not practical for most people, they can't afford etc. I've done plasma donation at my local blood bank since beginning of 2022 and over course of the year lost donated 11 liters of plasma which is between 3 and 4 plasma volumes. These donations are free and help people in the process. Donation is about 25% of volume each time and I do every 4 weeks. At 68, my Levine Phenoage is in low 50s, but it was good prior. With the new Conboy paper, I will be tracking some of the markers noted there, CD4:CD8, etc noted in the paper along with normal blood makers. 2021 paper by Rob Flickenger assessing plasma donation in a small group of people suggests changes similar ways that larger volume TPE does. Conboy recently said that she does not know minimal volume needed and frequency of donation and would be interested in assessing plasma donation. I would steer away from anyone selling this process unless they can provide a scientific basis for why routine FREE plasma donation does not have a similar effect, unless you have money to burn.
$4,500 per treatment, if correct, is a crazy price for TPE, I suspect he meant the $4,500 was for a pair of treatments, which is still crazy. I have been doing whole blood donations 4x/year for 8 years (12% of volume/donation). It is not clear to me that conventional plasma donation actually filters out most of the "bad" components in the plasma. I will probably increase my whole blood donations to 8x/year, which is a violation of the rules, but a creative person can get around that.
His claim that his Uric Acid dropped from about 7 to about 2.1 only makes sense if he did the UA test shortly after the TPE. If he had a UA of 7 for months despite efforts to lower it, he has a large amount of Monosodium Urate crystals deposited in his tissues, his blood serum with slowly return to equilibrium with the deposited Urate.
