  • Episode 262: Welcome to our enlightening session, "Decrease Urine Protein and Preserve Kidney Function," a must-watch for anyone looking to slow the progression of Chronic Kidney Disease (CKD). We are honored to have Dr. Rosansky, a distinguished nephrologist and kidney disease expert, sharing his valuable insights and practical strategies to help you manage CKD more effectively.
In this video, we delve deep into the importance of decreasing urine protein to preserve kidney function. Dr. Rosansky discusses how certain strategies and treatments can help reduce proteinuria, thus aiding in slowing down CKD progression, prolonging life, and possibly delaying the need for dialysis. This video is an essential resource for those living with CKD and those wanting to understand more about kidney health.
    Main Discussion
    1. Importance of Proteinuria:
    * Proteinuria is the best predictor of future kidney function decline and should be the main focus of treatment.
    * Reducing protein in the urine is more significant than minor changes in eGFR for slowing kidney disease progression.
    2. Understanding Proteinuria:
    * Protein in the urine, particularly albumin, indicates glomerular damage.
    * Measuring urine protein accurately is crucial. Dipsticks are a simple way to monitor it, and specific gravity helps understand urine concentration.
    3. Measuring Protein in Urine:
    * The most accurate method involves sending a urine sample to the lab to measure albumin and creatinine levels.
    * Persistent protein levels above 200-300 mg/day are worrisome and indicate a higher risk of kidney disease progression.
    4. Factors Affecting Proteinuria:
    * Certain conditions like fever, intense exercise, severe dehydration, and emotional stress can cause temporary proteinuria.
    * Orthostatic proteinuria (protein in the urine when standing) is benign and common among young individuals.
    5. Impact of Proteinuria on Kidneys:
    * Protein in the urine is likely directly toxic to the kidneys, causing scarring and glomeruli damage.
    * Reducing urine protein correlates with slower kidney function decline and lower risk of cardiovascular diseases.
    6. Treatment Strategies:
    * ACE inhibitors (ending in 'pril') and ARBs (ending in 'tan') are effective in reducing proteinuria and slowing kidney disease progression.
    * These medications can decrease the risk of kidney failure by up to 50%.
    * Other measures include lowering salt intake, taking statins, controlling diabetes, and quitting smoking.

Hi James have question please. Mom 73 with diabetes type 2 and Asthma and she does not have kidney disease. Tested her and gfr was 52 and creatine was 92. We repreated the test again thank God her Gfr became normal again at 92 and creatine 51. Also, urine protiean/creatine test was normal. My question for you is, what can caused that fasly low gfr dehydration maybe? We were concerned before we see her gfr rturn to normal.

I start the day w-16oz lemon water, next 16oz celery juice, next an apple sauce, next a decaf black espresso, next a soup or stew. later a Chicory coffee. Some herbal teas, like rosemary etc. I *never* drink plain water and believe me, I get more than enough liquid

yeah-great job, congrats. The dietary information in this country sucks= refined sugar, refined foods, processed foods are all garbage. There is NO NUTRITIONAL VALUE IN THEM AT ALL. Only empty carbs-calories. The body requires vitamins/minerals to function. And on top, there are genetic predispositions that make matters even worse. Doctors suck-they don't tell you anything at all = except what chemicals to take. Do your own research is best.

So happy with this video. Constantly stressed about my markers for over 5 years. Unsuccessful at getting appropriate explanations from my PCP who diagnosed my score at 3.4. I believe this was based almost strictly on a GFR of 44. I have recently been prescribed Farxiga and that number was improved to 51. My Creatinine is high at 1.5. Albumin slightly lower than normal. One test have not heard mention was a Renal Function Panel. I had one done recently came back in normal ranges.
Anyway thank you for lowering my stress level. {which I just learned from this video can affect my markers😊)

I came across your site, incredible information. I’ve have had a transplant for close to 40 years. Just recently in the last year my protein level has skyrocketed from 11 mg/dl to 446 mg/dl, BP has gone up also a little and Albumin in the urine from 10.8 mcg/mg to 5276.1 mcg/mg (normal 29.9 mcg/mg) definitely something is going on! Will follow your guidance and your information to improve current situation!

I also have high albumin protein in my urine. Are you taking supplements? This doctor said not to take supplements if you have high protein in your urine, but at my last appointment, my nephrologist told me to keep taking them. Confusing on what to do when there is so much conflicting information out there. What meds does your nephrologist have you on?

Very nice discussion. Thank you very much for your humanitarian efforts to inform the public. Most foks cannot get this kind of information because,
    1. They can't ask their doctors to much because the doctors are limited on timing schedules between appointments.
    2. They don't know what to ask their doctors.
    3. Some doctors just keep them in the dark about this disease as they treat high blood pressure and diabetes.
I'm getting the impression that everyone living on Western diets have high protein in the Urine. I assume it may be because of so much sodium in our processed foods used as preservatives. Heck, it could even be caused by the other wild preservatives you see on the labels.
    Anyway, thanks again.
