I think the translation as "foster siblings" is not quite correct. As far as i know it's more like "milk siblings". They both had the same wet nurse. Ah-duos mother was the wet nurse for the then prince and now emperor.

The first that Jinshi is the son of Ah-Duo and the now Emperor, and the second that Jinshi is the son of the former Emperor, and the current Emperor's younger brother. As for Maomao's father, he was the doctor attending to the births of both children, but because he was delivering both at the same time, one died - which is why he was expelled.
