    MEDICAL ANIMATION TRANSCRIPT: Opioids are drugs that are prescribed to treat moderately severe or severe pain. Examples of these drugs include hydrocodone, oxycodone, morphine, hydromorphone, and methadone. Heroin, an illegal drug, is also an opioid. In order to understand how opioids work, it’s important to know how your body feels pain. The process begins when something harmful happens to your body. Information about this harm is converted to a nerve signal. The signal passes along nerves to your spinal cord and brain. In your brain, the signal is perceived as pain. Opioid drugs affect how you feel pain. They attach to structures, called opioid receptors. These receptors are found on cells in your brain, spinal cord, and other areas of your body. Opioids act on these receptors to make you feel less pain. These drugs also have different effects in other areas. For example, opioids act on the reward pathway in your brain. This causes a release of a chemical called dopamine that results in a happy feeling, or “high.” Opioids may also cause slower breathing. And, they may cause reduced motion of your intestines, resulting in constipation. For most people, when opioids are taken as prescribed for a short time, they are fairly safe and effective. But, these drugs can be taken in ways that weren’t prescribed, such as taking too many, taking them to get high, or giving them to someone else. Misusing these drugs can raise your risk of developing drug tolerance, dependence, addiction, and overdose. Tolerance means the drug is less effective over time. This can happen when opioid receptors become less sensitive to the effects of the drug. As a result, more of the drug needs to be taken for you to get pain relief. Dependence happens if a person has symptoms when they stop using the drug. When the drug is removed, or withdrawn, you feel sick. This sickness is called withdrawal. Opioid withdrawal has flu-like symptoms that include restlessness and anxiety, muscle aches, inability to sleep, watery eyes and runny nose, nausea and vomiting, stomach cramps and diarrhea, and dilated, or widened, pupils. Opioid addiction is a brain disease where you have an overwhelming craving for the drug. You can’t stop taking the drug despite the harm it may cause you. Addiction is not the same as dependence. You can be tolerant of a drug or dependent on a drug, without being addicted to it. Finally, opioid overdose is a condition where taking too much of the drug can cause life-threatening symptoms, or even death. The symptoms of opioid overdose can include confusion, feeling very sleepy or not alert, nausea and vomiting, constricted, or small, pupils, unconsciousness, slow or stopped breathing, and death. According to the Centers for Disease Control, over one hundred Americans die each day from opioid overdoses. If you have questions about opioids, or if you or someone close to you needs help for an opioid use disorder, talk to your healthcare provider.

    I used to be addicted to hydrocodone. It was never prescribed to me. The withdrawals were sooooo bad…. I can’t explain how bad. It’s like I had to get through not sleeping for a week and a half, with the most restless feeling, and body aches, just to start to get a leg up on quitting them. I actually couldn’t do it. I started taking meth, and that made it a lot easier, and that was a lot easier to quit. No lie. Been clean and sober now for almost 7 years.

      It’s a shame doctors do not want to prescribe opioids to people that have legitimate pain issues due to addiction potential. What they don’t seem to care about or understand is that there is no other solution. I would much rather live with addiction then live with chronic pain. They’re pretty much telling patients “oh well deal with it” simply because they think addictions more harmful, it’s stupidity.

    I have been on an opioid for the past 3 years due to chronic osteoarthritis. Without it, the chronic pain would lower my quality of life until I wouldn't even want to get out of bed each day. My pain doctor trusts me because I don't abuse it. I use it before bed so I can sleep without that nagging pain. Just a few weeks ago, a good friend from my high school days killed himself because of chronic pain. I understand the need to regulate prescriptions but there are many of us who need the medication to maintain our quality of life.

      I say the same thing all the time. Chronic pain kills. And when doctors look at you like you're just an addict and try to give you the whole "try physical therapy again" or "here's some Neurontin/Gabapentin, try that instead of opioids and let me know how it works out", you have to follow their orders, and meanwhile you're dumping money on something that isn't working for you AND you are suffering the entire time. Then you have to pay more money to go back and see the doc again to ask them for help, and they just go "well you're too young to be on opioids" (I got that a lot after I got out of the army, as if you don't feel pain as strongly because of your age or something). Even when I would take my shirt off and show them my three surgical scars, "physical therapy and ibuprofen" were usually their responses.
      basically they'd tell me to kick rocks.
      Thankfully I have found a pain management doctor who takes my pain needs seriously. She saw how my life was deteriorating and put me on an incredibly low dose of methadone (pill form). I can actually sleep most nights now.

      I was prescribed tramadol for a year too for chronic gastritis pains, I didn't know much about drugs, all I knew was that the drugs are really making me feel good and completely takes the pain away, I didn't realize am developing drug resistance which will later on become dependency, my first experience of withdrawal symptoms I was out visiting a friend for a sleepover and left my pills, I hadn't taken them for about 24 hours, I felt so sick, runny stomach, unexplainable restlessness, abdominal pains, cramps in the stomach, feeling dizzy, I couldn't sleep till morning... First thing I did I got up in the morning and drove back to my place, I took 2 pills and 3 hours later everything was good, I was calm, I was okay as if I am normal. I got worried so I did a research on the drug, I found out that what I was going through was withdrawals and I had developed dependency on the drug, I tried on my own to stop by withdrawals would just be too much... Until I took a decision to seek for professional help, I went through therapy, I got hospitilized and went through detoxification process. I got my life back, it felt like heaven having to live your life without panicking when your box of tramadol is running empty... My psychiatrist said "Tramadol is the worst opiod ever that you can abuse" he said that though it's withdrawals are not deadly, but they definitely extremely terrible. After they took me off tramadol in hospital my withdrawals only last 24 hours, and then after the medication used to treat them started working in my body, and fortunately enough no organs in my body were damaged yet as a result of drug abuse. I am grateful and I so wish everyone with an addiction, dependency or resistance would one day get their lives back!

I watch another youtube channel too called 'The Life Boat' that really focuses on educating people on drugs and living happily in sobriety. Recently they have had a lot of shows about Fentanyl. They have a live stream at 5pm (Pacific Time) everyday except Wednesday and Thursday and you can call in live to talk about your experiences and ask the hosts questions. They are extremely knowledgable, helpful with no judgement and I know for a fact they have already saved quite a few lives. It's just a loving, friendly place to go, regardless of what is happening in your life and love helping people.

    I am a chronic pain patient. Honestly, I need opiates. Nothing has worked for my arthritis. I was on the same dose of Fentanyl (patch) for 15 years, so not everyone goes up and up in doses quickly. Day to day living would be impossible without an opiate. I feel NO HIGH whatsoever. That is because it all gets used up counteracting the pain. Hope this helps a few people understand why they are still such an important treatment modality for pain patients.

    • @Queen-of-Swords
      @Queen-of-Swords 2 ปีที่แล้ว +3

      @@Sweet2eatbaking Sounds familiar. I'm also in the UK and I have wondered about trying the patches you are on. After 14 years of being on the same dose my patches began to lose effect on Day 3 of use. So in the end the doc increased the dose but I still have the same problem. I also take anti-inflammatories on bad days. I wish people understood. Its an unfortunate fact of life that most pain relieving substances can be abused - but as you agree, those who really need them don't tend to experience any high.
      There are some TV shows I've seen where doctors get patients off their meds by making them swim in very cold water and so on! They don't tend to cover people having heart attacks from shock on those shows though! These alternative methods aren't realistic for the majority of chronic pain patients. And actually you will find many of the "miracle" stories are misreported by media anyway. I know personally a woman who swims in the sea for her migraine, she lives in North Wales. Her migraines have decreased in number since she started, but they haven't stopped. Still, the newspapers report that her migraines totally stopped!!!!

    I have been on an opioid for roughly18 years for chronic nerve pain(PHN). I have never had the need to increase the dose and have no side effects. I have never felt “euphoria” However, once the most effective opioid was prescribed 6 years ago, the pain had decreased from level 7 down to nearly 0 during the day and 1 at night. I cannot begin to describe what that meant to my quality of life! I have read the research on chronic nerve pain and experimental treatments (Tried many of them). I am hopeful that the knowledge gained through research will eventually lead to a medication that turns off the chronic pain response. The first paper on that was 2011. The gene which codes for the protein that controls the chronic pain “off/on switch was found. Sadly, I have heard no more, in terms of clinical trials following that amazing study. Chronic pain patients with ‘increased plasticity” in the brain,keeping the pain messages active, should stay hopeful. Our understanding of chronic pain is slowly getting to a better, more effective, solution.

This was back in 99 so the internet and information about these things were far more obscured. Thankfully I never went through major withdrawals but that was one substance that I really missed when I used them up. I didn’t realize how much I liked them until I didn’t have them, that gave me a profound realization that it was for my best interest that I didn’t pursue trying to get more illegally…it scared me.

I'm very grateful because I feel I would probably not be here today if not for the medication I take (potential chronic-pain induced suicide) and the fact that I'm not tempted to increase dosage (death by overdose). I've seen how it can destroy lives as well as prolong quality of life. My hope is that one day soon humanity will learn what makes people like myself react somehow differently to opioids and develop a pain solution that works for everyone in need without the risks of addiction and overdose.

      This person is lying. It is not 1991 anymore. You could get plutonium easier than opioids now. It is just really trendy for people to say they are easy to get because of the "opioid crisis" which is a cultural thing now and if you nod your head and say "hmmm yes i know about that" you seem wise. The people addicted are addicts who were prescribed years ago. Even if you need opioids for chronic pain doctors are unmercifully kicking these patients off them, thanks to addicts who abused the system and dealers. Cut the bs.. doctors rarely prescribe them anymore... Only for cancer and severe pain and not enough to for you to get addicted unless you abuse the prescribed dose.

      That's kinda odd to me. Normally, a doctor would (and should) prescribe that kind of drugs in a form of "dosage plan" or schedule, starting with the minimum necessary dose for a couple of days, then increasing it in small amounts until achieving the desired effect and keeping that dose as low as possible, then as the cause of pain goes away (in your case, the wound healing) also the dose should be decreased to prevent the patient withdrawal effects.
      So in a *very, very* hypothetic example instead of taking a full dose every day for 14 days you actually take 1/4 for 4 days, then 1/2 for 4 days, then 3/4 another 4 days, full dose 2 days (or as long as the medic deems necessary for a peak dose), then decrease the amounts by 1/4 every day or two until you no longer need the medication. Opioids are extremely addictive so healthcare personnel would usually have to be very careful with them.
      It really sucks to know that was not the case with you 😟 But it's good to know you ended up alright 🙂

    I’ve been on high opioids for 20 years. Such a shame they haven’t figured out a way to make a pain killer minus the addiction issues. Oxy I done in my opinion is the best pain killer out there, but it has a dark side u get addicted very quickly then it starts a hell cycle of trying to stay ahead of withdrawl symptoms that make u wanna die. It’s almost like the ppl who make these drugs don’t wanna look into that 🤔 no addiction less money made

    • @vinnieboy3142
      @vinnieboy3142 ปีที่แล้ว +2

      It's actually impossible. Opioids - full or partial agonists - mainly bind to mu receptors to exert the pain-killing process, but alas, the same receptor is responsible for the pleasant effects, also for the partial agonists /antagonist buprenorphine. So there's no analgesia without( at least some) euphoria. The ideal opioid does not exist. You can try with Tapentadol, a stronger form of Tramadol. But they're not opioids properly. They have the same backbone of venlafaxine, an antidepressant, modified to target opioid receptors. But it's too weak if compared to Hydrocodone or Oxycodone. I'm an ex heroin addict, and after a long methadone treatment, I was put on oxycodone since 2013 because of chronic pain. I have have never abused it. I live in Italy and the doctors are more willing to prescribe 5mg or 10mg OxyContin. Pills sold in the street? There's no such thing here.

I got my grandpa back❤

    Ive been off drugs now for 16 yrs this month. (11-2006 to now 11-2022) I wouldn’t wish my worst enemy on dope, (aka heroin). I was turned onto heroin from using prescription drugs legally. I was a straight up, no nonsense guy. Never broke rules let alone break the law, until I turned to drugs. I was into boxing as a teen, and wrestled in middle school. One day at the gym, I broke my wrist, and the Dr prescribed a strong opioid. The feeling was amazing (at first). Then after a few weeks, to couple months, it turned into maintenance, just to stay functional. The high was gone, and the goal was not to allow my body to get sick, (aka go through withdraws).
    I wanted to do special forces, then law enforcement, then be a detective, I dreamed it every day from the time I was a child. All my dreams went out the window the day I started drugs. I ended up getting a 5 year sentence for breaking the law to supply my drug habit. I did 3 1/2 years on that 5 yr sentence, and haven’t did drugs since my release in 2006, Im 42 now and not a day goes by I dont regret making the choice to switch from pills to heroin.
    Im telling this story to try and persuade anyone from using drugs, DONT DO IT!! Its not worth it, no drug is.

      It does stop the pain. Substance p and glutamate can't be released from the presynaptic terminal when the opioid receptor is activated. Lack of substance p and glutamate creates a strong analgesic effect.

    • @bradleyj365

      I would also like to know, because if you are dependent on something aren’t you by definition addicted to it?

    • @666Hellbells
      @666Hellbells 2 ปีที่แล้ว

      I would imagine the emotional body being out of balance, pain, trauma, grief, anger etc, unresolved will remain in the body. Left unprocessed it will continue to feed; (low self worth, self harm, etc) addiction happens when you use opioids to block this pain rather than face it full on. To be dependent on a drug because it blocks the pain is understandable however, pain has a voice and if you listen very carefully it will lead you to where you have to go to heal...

    • @kenziealex3868
      @kenziealex3868 2 ปีที่แล้ว +5

      I believe the difference is that dependency is being dependent on a substance to continue living normally but addiction is when you are dependent and taking a substance for pleasure instead of the intended purpose. Like let’s say someone has to take a opioid to control their pain and be able to go on with life things like getting out of bed, going to work, engaging with friends but then if that person doesn’t take the opioids they can’t do those things because they hurt to much they are stuck in bed, don’t go to work, stop talking to friends that is dependence. Another scenario let’s say a person is taking a opioid that was originally given for the treatment of pain but now their pain has gotten better, but they are still taking it not for pain relief, but for euphoria and entertainment and if they don’t take the opioid they can’t function not because of being in pain but because of other reasons that’s addiction. At least that’s my understanding of it.

    • @rotex
      @rotex 2 ปีที่แล้ว +3

      Addiction is the loss of control of limiting the use. Dependence is when it's needed, but you have the ability to control how much you need and no more. I have an intrathecal pump that has a constant amount of Fentanyl and bupivacaine given to me. I also have a bolus device (a remote control) that I can use to administer medication every hour if I need to. If I use it every hour, my body will want it every hour, and it will cause me to go through withdrawals. I only use the bolus when I can no longer deal with the pain. If your Doctor prescribes you so many pain pills for the month, and you use them up the first week, you're addicted. If you can make them last the whole month, your dependence.
