How to tell if the palpitations you are getting are serious or not

  • York cardiologist, Dr Sanjay Gupta discusses palpitations and how to work out if the palpitation represent a heart rhythm problem by tapping out the different beats. www.yorkcardiologist.co.uk.
Hello, my name is Sanjay Gupta and I’m a cardiologist in York. Today I thought I’d do a little video on the subject of palpitation. Palpitations are really common symptoms that I see in my clinic and majority of palpitations are not serious. The big worry when people experience palpitation is whether they may have a heart rhythm disturbance. Even if they have a heart rhythm disturbance, most heart rhythm disturbances are not serious. So, that’s a little bit of reassurance.
The first thing that I would like to talk about is what is a palpitation? I suppose palpitation is a rareness of one’s heartbeat to the extent that it feels abnormal. So, the hart could be beating normally but us sensed abnormally. Now, there are many normal causes of palpitations, so to speak. So, for example, if you were walking down a dark alley and someone jumps out suddenly, then that would cause palpitation; that would cause the heart to beat very fast and that would be felt as a palpitation. But that doesn’t signify a problem with the heart itself; the heart is just doing what is meant to do, it is beating fast in response to the adrenaline that is circulating around the body. Similarly, anxiety and stress can cause palpitation; caffeine can cause palpitation, people who drink a lot of caffeine; alcohol can cause palpitation; exercise can, obviously, cause palpitation. So, these are the plan that of sort of normal things that can cause palpitations which don’t signify anything to be worried about.

I’m 8 years late to this, but THANK YOU SO SO MUCH. I’ve had many family members with heart and circulatory problems, and it’s given me cardio phobia for years. I strive to be healthy and work out 4-5 days a week, but every little heart flutter and instances of anxiety-driven heart rate increase gave me deep deep fear. This video made me feel so much better. Thank you again.

I usually don't comment on videos at all. But I have to thank you for taking time to do these videos. It's very comforting to have symptoms explained by a trained cardiologist. Unfortunately I've made the experience that most doctors don't have the patience to accurately explain things to their patients. So thank you very much! It helped me a lot and I'm sure others feel the same.

Over the past 9 years I’ve gone to three different doctors (at Kaiser, in California), all of whom barely looked at me, only at their computers, and seemed to be so rushed, with barely time to listen to me but just hurry on to the next patient. None of them has ever even asked about my diet, for instance. For every ailment, it’s another prescription. (I’m 73 and now on nine different prescriptions. Think how much that’s costing Medicare. And I still feel awful.) They may wish they had more time with each patient, but this is a business, after all, and time is money. I’m lucky to have access to any medical care (so many Americans don’t), but how wonderful it would be to have a doctor like this one. I’ve come across these videos by accident, but I’m now following his posts and learning so much.

Marilyn Bahoura , I had a very bad bout of Pericarditis at 21yrs old, and recovered well and no issues really for years. Then, out of nowhere, the past 18 months or so, I started having random episodes of what the doctor said may possibly be AFib.
Out of NO WHERE, my heart will randomly start beating extremely hard, but not fast, and I can't breathe in as deep as I normally could. So, basically, my heart randomly will just feel like flippy/floppy in my chest, and shallowness of breath. I can breathe, but it just feels like I can't inhale as much.
This doesn't happen everyday, and it doesn't have any real schedule. It just kinda pops up, lasts for anywhere from 5 to 25 seconds or so, then just kinda disappears.
It freaks me out terribly, like, full blown "holy crap what's happening" freaked out. lol. Due to my past with Pericarditis, I have developed SEVERE hypochondria over the past year or two, ever since I've had these heart episodes.
So, I can sympathize completely with your hypochondria. It is absolutely crippling, to the point that you can become almost reclusive or agoraphobic over it.
If anyone has any tips or any suggestions as to hire to deal with this, or any relieving tidbits of advice, I'd love to hear from you. :)

    • @strikeforcek9149
      @strikeforcek9149 7 ปีที่แล้ว +15

Marilyn Bahoura 9 x out of 10 (at our age) it's stress induced. Hell, unseen/non-thought about stress is one of the biggest triggers to my AFIB/SVT attacks. But, getting stressed about the feeling/issue will only make it worse. So, I tend to just breathe in deep, tr my best to relax, and let whatever is going to happen, just happen.
Because in the grand scheme of things, stressing out and worrying about something will absolutely not make it any better whatsoever. It's only guaranteed to make it worse. So, try your best to just push through it and relax it away until you can get to the bottom of it. :)

I got more information from this free video than I did with a €50 consultation with my doctor who just said 'anxiety' and sent me away with medication that I didn't understand. He didn't want to monitor me at all and when he eventually did write a referral to a cardiologist, they lost my letter. Hospital said they never got it and doctor said he had sent it and the hospital lost it. It's really not taken seriously I find. I know most cases are harmless but even for the mental health of the patient I find monitoring is essential.

Hi Imogen... you are quite right... the reassurance that a doctor gives you is only useful if the patient has confidence in the doctor and a lot of doctors these days do not have the communication skills that allow a rapport and relationship of trust to develop between the patient and the doctor.

    • @dardbhariyaadein-bydrmuzaf8669

Hi Imogen. Thank you for your comments and your nice words about the video. I agree with you that just because the doctor thinks that they are not dangerous does not mean that they are not important and to my mind the doctor should do all that he can to reassure the patient because that is the only way of getting rid of them for good.best wishes

Okay so...I kind of figured if my PVCs went away when I slept, that was a good sign, and you sort of confirmed that, so thank you.
But, I just heard from my GP yesterday about my 48 hour holter monitor report. He had told me we start to worry when the total burden hits or is over 30%, so my total burden of 29%, I figured, was close enough to merit further investigation, at least. But he gets on the phone and he's so 100% certain that they're benign, I actually am confused...because what I read doesn't say anything about quality or timing, even on these videos--except maybe for today--they talk about the number and usually it's less than 30%. So it was my understanding--but what do I know--that it was sheer volume that could start to weaken the heart.
Then I hearkened back to one of Dr Gupta's analogies: If he pressed on my neck-press and release, press and release-that's not going to kill me, and I get that. In fact, I've never really worried about my pvcs because I have fatigue issues and they're kind of functional in that sense, sort of like a 16/7 fire lit under me, that constant sense that danger is slightly imminent keeps you alert, right?
But what if you have 3,360 of those press release moments in one hour?
At a minimum, might you not want to kill the person who's doing that to you?
Might it not start to leave a bruise?
I feel sometimes like doctors' focus on "will this kill you? Not today. Something will eventually, and there's nothing we can do about that, either," isn't very productive.
I have a tendency to minimize anyway, so if a doctor intimates that I should come back when I have a "real problem" that's when I'll come back, and I'm probably not going to be very happy if there were some intermediate steps that could've been taken that doctor didn't bother to tell me about, maybe because he didn't realize I usually don't come in unless I absolutely have to.
It was only after I got the report that I correlated that hour of 3,360 ectopics with how I felt. Keep in mind, I have fatigue issues, I probably have shortness of breath issues, my chest hurts a lot, but it's all so often that I've normalized it. But that day, it was like I just simply could not make myself move faster in a situation where speed kind of matters, so of course, that added stress. But usually I can step it up if I need to and it was like, no, I can't. This is what I can do right now.
And then I saw, that was right when my 1100 per hour average tripled. Why it did, I don't know. And then I deduced, probably at that point, I WAS struggling to get adequate oxygen to the parts that needed to move (my arms/hands--I was cutting hair. Not heavy/aerobic labor.) I also saw my heart rate jumped to 93 bpm, which is what I average on a brisk walk. And I broke out in a sweat, even though it wasn't warm.
I get it, it didn't kill me, I didn't have a heart attack, and I was able, albeit slower than I wished, to complete the tasks at hand.
But at what point do such things become "disabling"? At what point does it merit...commiseration, at least?
I get that my doc was trying to reassure me. I accept that these are 99.9% benign (though I am starting to notice tachycardia, and I counted at least 20 beats before it stopped (monitor said the most was 7 beats, and I didn't notice anything, not until after I was done wearing it.)
.I don't know. I also have very severe osteoporosis, fractures in my spine prexisted diagnosis, and honestly that probably is part of my stress--I know I'm going to endure some pretty significant pain in the course of my work day, and maybe my sympathetic nervous system anticipates it, but it actually wasn't hurting yet during my bad hour. Still, doctors are like, "shoot, well...old people get osteoporosis, what are you going to do?" Sometimes just a little sympathy (and acknowledgement that they missed cues, which allowed me to get worse before it was ferreted out, and WE are the ones who asked for the DEXA,) and how the fact of their missing cues, even if doctoring is hard and you can't catch everything, how that's going to affect me now...just acknowledgement, just...not acting like I'm a big baby. I don't over-react by nature; I underreact. But if I get to where I feel like you're not listening, I will possibly overreact. Just ..acknowledge, have some empathy. Imagine what it would be like if someone pressed on your neck 3300 times in one hour.
      That's all.

Met my cardiologist a few months later to discuss a few anomalies noted on the "normal" results. He said all is ok, dont worry about it. A week later I had fainting spells and chest pain and checked into the hospital with heart symptoms. Next day got a stent for a 95% blockage in my LAD "Widowmaker" artery. I tremendously respect the time you take to put videos together explaining particular heart conditions. I am sure many others have been blessed by your videos. I believe God has truly given you a gift and you are operating in your gift to help others. Like many others I have been battling with heart palpitations. My story began in August of this year. I was singing in the choir and my heart starts beating out of my chest. Before church I admit I had coffee.🤦‍♀️ Long story short, so many ER visits with results stating sinus tachycardia. I wore an event monitor for 30days with events occurring while wearing the monitor. My highest heart rate was 150 results just state sinus tachycardia. Previous echocardiogram was normal as well. I was placed on Metoprolol 12.5mg at one ER visit due to my heart rate reaching 170s(although I believe anxiety contributed to that high rate). I took it for about a month. I am currently not taking it anymore. My Cardiologist suggested a loop recorder. I was wondering if you could possibly suggest a rational for moving straight to an implant when they have not found anything nor have they done a tilt table test or a stress test. I still get heart palpitations when walking up stairs and bending down to standing position. However thankfully no fainting spells although it use to seem like I was about to faint during an attack. My symptoms were mainly short of breath and slight chest pains when my heart palpitations were at their worst. I just felt weak after an attack. I would Love hear your ideas on this. Could it possibly be just anxiety? Also could Thalessemia play a role in heart palpitations? Thank so much for the continued cardiac education! Many blessings!

I've had SVTs since I was a kid. Got officially diagnosed a few years ago. Let your GP know your symptoms. They will probably want to capture them on a monitor that you wear for a few weeks. If they capture SVT, then you will likely be referred to a cardiologist for a stress test and echo cardiogram. In my case both the stress test and echo were normal, so the cardiologist told me there was a low likelihood of SVTs causing any problem. But you should definitely get checked.

Oddly enough, this actually really helps me with my health anxiety. I've had 2 normal EKGs spaced a year apart, and an x-ray that found nothing and its comforting to know that from what you have described it sounds like an extra beat. I'm only 26 years old, but it has ruled my life for that past 10 years. I do plan on still getting a monitor just to finish putting my mind at ease, buts its good to know that these aren't anything serious.

This doctor always give me peace of mind when I watch his videos. I suffer from PACs which are extra beats. Been seeing a cardiologist for about 7 years so I pretty much take it that I’m fine. I’ve had a million ekgs, ultrasounds, numerous blood work. Just feel if there was something wrong they would of caught it by now. Also had a link monitor in for 3 years. Had an ablation but it honestly wasn’t needed. He gave me a choice to get it. Don’t notice much of a change. I also suffer from anxiety, panic and large amounts of stress which are probably the cause for my PACs.

But one night I awakened un able to get air into my lungs. The ambulance came and took me to the ER. DR. IN THE ER CALLED RESP. THERAPY. THESE GUYS HOOKED ME UP TO A CPAP MACHINE. IT WAS LIKE THIS MACHINE FORCED AIR INTO MY LUNGS NOT UNTIL THEN DID I TAKE A BREATH. I was put on bloodthinner medicine and a fluid pill. And then my Cardiologist told me I was having congestive heart failure what's going on?

Well, thank you for this! What you describe from 7:30 is the kind of heart palpitation I've been getting a lot of since I had my 3rd child (he is 4 months old now). I've been driving myself nuts trying to work out if it's just stress, sleep deprivation, something to do with the low carb/keto diet (and possible electrolyte imbalance) I've been on since having gestational diabetes, perhaps anemia (I needed an iron transfusion in late pregnancy), my thyroid, a lack of estrogen due to breastfeeding, too much caffeine, or something more sinister... goodness me, the list is endless! But what I experience is the most common one and it's more obvious to me at night and on days where I have been stuck in the house and breastfeeding more (so when I am exercising less). So it sounds like I should just go ahead and ignore it and try to enjoy life more instead of worrying. Thanks again! :)

Thank you so much for posting this. I’ve been experiencing what I think are PVC’s or extra beats. They happen exactly how you described them. All of a sudden started happening multiple times per day. I believe I pinpointed the cause to a dietary supplement I was taking that contains aspartame. I’ve read about people’s experience with heart palpitations and aspartame. I never drink diet sodas or use artificial sweeteners, but suddenly had a significant intake of this sweetener. Now I’m just waiting to see if they go away now that I have stopped use.

I woke up 2 nights ago with a racing heart which made me nauseous, very hot and sweaty, and then I threw up a couple of times. Nausea continued for about an hour. I had severe burning pain across my chest and into my right arm for about 5 minutes....and then I was able to go back to sleep . So weird, miserable, and scary.
Also I've had what has been diagnosed as PVCs/PACs which happens randomly (at rest and during exercise) and bradycardia at times. I've been a nervous wreck most of my life because of worrying about heart irregularities. I'm a 51 YO woman in relatively great health. This is a very good video indeed, I have been having palps for 3-4 years, heart structure all normal, my heart palpitations were caused by severe vitamin B12 deficiency, I still get them from time to time but have learned to manage them even though at times they can be very frightening, I never have chest pain or shortness of breath so I know its OK.
Once you learn how to manage them they become just annoying like hiccups really but when I first had them and did not know what was happening I had a full on panic attack which of course makes it a lot worse, I find getting my heart rate up by walking and also talking at the same time can quickly make them go away, sometimes bearing down like you are trying to poo also works as does splashing cold water in your face, I normally try all 3 and find jogging on the spot for a few mins and staying calm works just fine.
Knowledge is power in controlling palpitations, lots of rest also helps, my palps are always worse after a late night.
    Thanks for this great video!

York Cardiology I am a 31-year-old Caucasian male6 feet tall 330 pounds. I was drinking a lot of heavy heart liquor for about four years straight. About five months ago I had one of my last few drinks. I was eating a few burritos that were greasy and playing a video game with suddenly right in the center of my sternum I felt a shock. A sudden tug at my heart almost as if it pulled my heart downwords an this lasted for 1.5 seconds. It made me jump up out of my seat and throw my controller on the floor and went to my wife and sit on the bed and told her something strange just happened. My heart rate jumped to 122 and my blood pressure was at something like 180/101. I tried to go get a vitamin I think it was a Cayanne pepper vitamin to help my blood circulation and when I went to grab the bottle my hand started twitching and I could not squeeze the bottle. I stopped drinking for like a month and then I have another drink and I would randomly throughout the day Get the shark feeling right now in my heart that lasts one second but it is absolutely scary. I have now not drink for about two months straight and I rarely get these shock feelings where it feels like my heart jumps for 1 to 2 seconds and that it in fact it feels like a shorter time than that. Feels like I will be laying down and just randomly out of the blue my heart will flip around and then suddenly im paranoid for an hour. And during this paranoia it won’t happen again even if I get really anxious and worried it only happens randomly. Usually I have to smoke marijuana or eat a lot of food for it to happen or just randomly be playing a video game sitting or laying straight down. I need to know what this is or what it sounds like. I have no doctor or an option to go to one. I can answer any and all questions pretty much I like to give all the information upfront. I don’t know of any known heart diseases in my family and I’ve never experienced anything like this in my life until just recently. Sometimes when I am laying down in bed it will randomly happen it will feel like one second my heart flips up and down and that’s all it takes for me to get worried. My heart beats per second can be anywhere between 49 and 59 when I just wake up in the morning or when I lay down for a few minutes during the day but most the time it stays between 60 and 80 it definitely gets up in the 90s and sometimes 103 when I’m really worried or anxious. And my blood pressure has been around one 140/85

I feel like I get it when my electrolytes are unbalanced. I do not eat sugar or grains so my body does not retain water and this flushes out electrolytes. If I get them I balance my sodium (broth) potassium (avocado) and magnesium (supplement) and it works. This is likely why caffeine, exercise etc causes them as you are flushing out electrolytes.

Thanks for your video. I have extra beats, although it is pretty much happening all the time. I'm 57 and when I was 30 my doctor said my rhythm was "regularly irregular". I start having palpatations almost a year and a half ago. They're present on EKG, I've had an echocardiogram that showed I have mild pulmonary regurgitation and my problem is coming from the upper part of the heart, they said not to worry. They also did a holter monitor. My concern is that having this irregular rhythm will eventually cause more issues.

guys apparently most of the time it is not much to worry about, just a PVC or less. Happens more to ones that have irregular sleeping schedule, consumes caffeine, nicotine, and other stimulants. I've recently got a 24 hour holter testing and they've found nothing. My symptoms started over 3 years ago and they usually come and go time to time. Doing sudden fitness tends to provoke those symptoms. However, I wouldn't be 100% sure until more advanced tests are done, such as while running or over long periods of screening.

I've experienced the third type of palpitation you did with the normal beat, then the sudden stop..then the two beats then normal rhythm but it usually happens after intense workouts. The first time I experienced it I thought I was about to have a heart attack. I have a history of panic attacks and high anxiety and i believe it may have a lot to do with that. I just started experiencing this as of last year around march.

You should'nt workout everyday. Your body needs time to recover. I heard the other day, athletes are 3 times more likely to have a cardiac arrest. What do they do. They push their bodies to hard.

Thank you for this video. I've been having heart palpitations since Friday (today being Monday). I experienced them all day Friday with my heart beat feeling irregular, occasionally "missing" a beat which caused me to feel quite panicked each time. They also come with tension (pressure build up) headaches or mild head rush sensations. That evening my boyfriend took my blood pressure reading - the first was 165/135 with a bpm of 55. I know that's incredibly high, but didn't know if it was an inaccurate reading so after another 10 minutes I did another, then another 10 minutes after that, both averaging 140/80 ish and bpm 77. Still high, I know, but I'm a sweep it under the carpet kind of person. :/
I had a nap and on waking I didn't feel the palpitations until I was fully concious, then they started again. Then Saturday and Sunday I had them on and off throughout the day. Today being Monday I've had them all day again. I'm not a big caffeine drinker. There is a lot in my life to cause me stress right now, but I'm quite laid back and don't let stress show. If these palpitations are caused by some deep-seated stress, should they be lasting for days? I struggled to get through to my GP to make an appointment today, but will keep trying tomorrow. I never go to the doctors and I wouldn't want to take up their time unnecessarily. Any advice?

This is what's been happening with me the past 5 days idk why they're there but the highest my blood pressure was at was 181 which scared me into going to the hospital. They told me something is there just not noticeable on the ekg. It bc of the fact that I had other symptoms with the palpitations and they feel very uncomfortable and draining I see my heart doctor today so I hope I get some answers at some point. I do hope u still see your doctor just to be safe
