2025 PCPCH Model - Alternative Access (Standard 1.G)

Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.
This session (May 10, 2024) provided an overview of the updated version of PCPCH Standard 1.G - Alternative Access such as the parsing out of telehealth services into a distinct measure that includes considerations for patients with primary languages other than English, and the addition of a new 15-point measure inspired by feedback from community listening sessions around providing care in community settings.
