2025 PCPCH Model - Health Care Cost Navigation (Standard 5.B)

Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.
This session (September 20, 2024) provided an overview of the brand new PCPCH Standard 5.B - Health Care Cost Navigation, which includes two measures focused on reducing cost concerns by informing patients of low/no-cost preventive care services, increasing transparency with regards to the cost of services, and assisting patients in navigating their payment options.
