2025 PCPCH Model - Hospital Setting Transitions (Standard 4.E)

Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.
This session (August 23, 2024) provided an overview of the updated version of PCPCH Standard 4.E - Hospital Setting Transitions, which includes a new optional measure focused specifically on post-discharge follow-up procedures (4.E.2), and more flexible documentation requirements for the must-pass measure (4.E.0).
