I just tried this last night and they unfortunately patched it BUT there’s another way! After killing her, have one character start the dialogue on removing her head but don’t do it Then have another character open the loot window where you’ll see her head in her inventory Then have any character revive her while another character stays removed from party and therefor the dialogue Then have that other character pickpocket karlach (somehow pickpocketing her head from her shoulders is only a DC4 check xD) Then just as before, she’ll now be headless!

My biggest gripe with BG3 is how none of the classes and some reaces don't feel *that* unique. I could be buggin' though. For example: Monks aren't much of a big deal, they literally got wiped out like the Airbenders by the Gith in Lathandar's monastery. And you do get some cool dialogue options too but that's about it.
