    Thank you for sharing Mark. God Bless You. I hope all goes well. I had my prostate removed Feb. 2024. So far so good. Interesting you had PSA tests every 3 months. I have had PSA tests every 3 months up to this point Nov. 2024. My doctor wanted to change it to every 6 months. I said no I want to stay on every 3 months. She agreed to do it. If I have a reoccurrence I want to know sooner than later to get a handle on it.

    Hi mark. I was diagnosed with prostate cancer Gleason score 3+4. I decided to go with surgery. That was February 2023. In June 2024 after all my test results was 0.1 it then rise to 0.3 the next test 3 months later it again rise to 0.4. That was a month ago I’ve just had a pet scan and there was no traces of psa. I’ve just came from the hospital for my results of the pet scan explaining nothing was showing up on the scan. My specialist thinks two choices either go for radio therapy now and guess where it is (probably in the prostate bed ) and risk missing it with radio or wait for one more blood test and a repeat pet scan to see if it shows. Now my question is if I’ve had two rises in 6months 0.3 and now 0.4 and let’s say it goes to 0.5 or higher I would of thought it was getting aggressive and have I missed the boat. Or in other words left it a little too late. My specialist seems to think in my case it will be ok to sit tight as I’m 62 and fit and the Gleason was 3+4. Have you an opinion on this. It’s only because you stated in this vid that two rises doubling up could be quite aggressive. Thanks and will await your response. Ps love your strength on this horrible disease keep up the fight. been with you solid for the last two years now and for being so positive love from England x

    This was great! I'm 63 years old and had a proctectomy too almost 2 years ago. PI-RADS level of 5, Gleason levels of 7 and 8. As of now my PSA is at 0.08. Its possible that I may have a reoccurrence, but I'm glad I saw this video.

    Did PBT really zap all the leftover cancer and are you completely cancer-free? 5 days of PBT may be too little so you had to come back for 20 days of proton beam treatments again in a year for another recurrence?

    Now I'm two months after surgery. Still can't control my pee and still can't get any erections. The only thing I can do is having an orgasm with a non erect penis. I begin to freak out I can't have something back.

      I think he took a small area where the tumor was and ended up doing radiation if you see his last video . He will probably end up removing the whole prostate in the future and that will be a little complicated . I am going in tomorrow morning for the total removal of prostate . Wish me luck !

    Thanks Mark, I’m 58 and had prostate removed 31/6 PSA test 23/9 0.07 next PSA test in January 2025. I was advised to speak to radiology team but I chose to monitor. I’m really worried about radiation because of the long term side effects hoping next PSA doesn’t rise. I’m also concerned of the side effects of hormone therapy as it also has side effects. Tried tadalafil for ED but ended up getting really bad back and leg pains so I stopped taking them looks like I might never get over the ED. It’s a very stressful time in my life and I take my hat of to you on how positive you are with it all.

    Congrats on completing the radiation. Our stories are similar, and I'm about 2 years ahead of you. I was diagnosed in November 2010 at age 52 with a PSA of 5.0 ng/mL and a Gleason 3+3 with a palpable tumor on the digital rectal exam. I underwent a radical prostatectomy in January 2011 that went well. The prostate came out cleanly-negative margins, no lymph node involvement, no seminal vesicle involvement, no extra capsular extension, nothing-and my Gleason was upgraded to a 3+4. I went 54 months with an undetectable PSA post-surgery, and was eagerly waiting to cross the five-year threshold, but my PSA became detectable again at 0.05 ng/mL in September 2015. Of course, that sent me on an emotional roller coaster, and we continued to monitor my PSA. It bounced around between 0.04 and 0.08 for nearly 18 months before finally beginning a steady upward climb. My PSA doubling time (PSADT) was measured in years, and it took until July 2021 for it to climb to 0.2 ng/mL, the traditional definition of biochemical recurrence. At that point, I knew that salvage radiation therapy (SRT) was in my future, but I really didn't want to zap blindly so I went for a PSMA PET scan at UCLA in November 2021 shortly after they earned their FDA approval. My PSA going into the scan was 0.23 ng/mL and the scan was inconclusive. By April 2022, my PSA increases were accelerating and my PSA was now at 0.36 ng/mL, I started a six-month dose of Eligard (Lupron) on 1 May 2022 and went through 35 IMRT sessions to the prostate bed only in July and August 2022. The Eligard knocked my PSA down to 0.05 ng/mL again. In March 2023, 7 months after the SRT ended and the Eligard had worn off, my PSA was 0.13. In May 2023 it dropped slightly to 0.11, indicating the SRT may be kicking in. Unfortunately, by October 2023, my PSA was at 0.21 and, six weeks later in December 2023 after a confirmatory test, it was up to 0.33. All of this indicated that the SRT failed, too. In January 2024, my PSA rose to 0.37 ng/mL, and we did another PSMA PET scan which was also inconclusive at that PSA level. By May 2024, my PSA was 0.52, the highest it's been since surgery or radiation. The medical oncologist and urologist were in agreement to watch it for another six months-I go for another PSA test next week-and then map out a plan from there. Interestingly, the medical oncologist said we wouldn't start ADT until my PSA hit 2.0 and the urologist said we'd wait until there was evidence metastases. I believe the urologist didn't want to start the ADT too early, fearing it might accelerate ADT resistance. I know you had proton radiation and I had IMRT, so there may be some differences. About 5 months after SRT ended for me, I had a noticeable change in my bowel patterns that lasted for about 2 months and then eased up. (I went from one BM a day to 3-5 BMs a day. Two years later, I'm back to 1-3 BMs a day.) I hope your SRT "takes root" and you don't have a second recurrence and that any delayed long-term side effects are minimal. Like you, I've been sharing my story since the beginning via a blog (link in profile) because it's important for other patients to have insights into first-hand experiences. All the best.

Now I'll be having adt + radiation although the PET scans showed nothing! They're gonna go for the "prostate bed" area. Cheers + Happy Continuations...

    My main concern is dealing with incontinence over having erection at this stage. It's been just over two months since my prostatectomy. I'm taking Viagra for increased blood flow and hope to have no! Urine leaks or dribble as prior to the operation. I simply don't want that worry in my life. After that is under full control then Ill deal with the erectile dysfunction. Most research says if you haven't regained full erectile function after 18 to 24 months then that's the level it stays at. I'm just grateful that the cancer was discovered before it got out of control and escaped out from the prostate. I'm having blood tests every three months just to make sure its all ok. So far the results are great with 0.01 being as my PSA level. Thanks Mark for posting this useful information as it's crucial for men's mental health and giving them/us hope for a more hopeful future. I'm looking forward hopefully to having a woody again, but it's just baby steps at this time. I'm just glad they got the cancer in time.

    I wish I saw this podcast before my surgery, or immediately after. It's been eight months now since I had a robotic prostate surgery, and I'm not able to have erection at all. Six months after the surgery my surgeon prescribed 20mg of sildenafil, and referred me to cancer survivor clinic where I was asked to discontinue the use of the medication. But up till now I'm not able to have erection, though I have arousal. Is there any hope for me?

    I'm 73 and I am 8 Days out of surgery in the UK catheter was removed after 6 days I was a Gleason 8 T3A I was absolutely dreading incontinence I was amazed when I found out that I was dry the only leak I get is when I try to clear my throat it's only 2 days so I am still very nervous I think it must be because I did pelvic floor exercises for two months before surgery my heart goes out to you guys who are suffering I think the pelvic floor thing should be really drummed into anyone awaiting surgery

    Did your Psma scan show some new oligomer spots after these courses of proton beam radiation? You got radiated again one year later after this treatment with 40 days radiated !

    I just had my surgery yesterday. I"m a healthy 73 years old and on no meds. I had my single port robotic prostatectomy at 7:30 am and discharged by 2:30pm. I was able to do fine yesterday and today with the expected discomfort easily treated with Advil Liquigels. I also am a retired family physician and have diagnosed prostate cancer and followed by patients post operatively as well. I researched the benefits and risks of different treatments and also consulted for proton radiation treatment as well. I felt the best treatment for me as a healthy 73 year old or anyone younger with the same diagnostic findings (Gleason 8 and no spread) would be the prostatectomy only after the PSMA cat scan (that specifically looks for prostate cancer in any other area including lymph nodes) confirms the cancer has not spread and is consistent with your MRI findings, surgery is the best course in my opinion. Using the Da Vinci robot with a single port procedure is an excellent method, but only with an experienced surgeon. Do your research online and personal and professional reviews of urologists in your area before proceeding. Best of luck and hope your post op PSAs are always 0.0.

      I am 5 days post op. Discomfort but no real pain. Replace 73 with 70. Same story.

    Thank you Mark, I am dealing with reoccurrence after 3 years post radical prostatectomy. My PSA is elevated to 0.027 so I am early on in the process.

    Thanks for sharing, some really useful information and guidance. Like you, I had a radical prostatectomy in 2014, aged 53. In 2021, my PSA started to creep up and I've had 2 PSMA PET scans that haven't found anything - yet ! My PSA is currently 0.67 and rising slowly and I'm due to have my 3rd PSMA PET in a couple of weeks. I really want to find where this pesky cancer is !! I'm not convinced that ADT is the way forward when the cancer cells cannot be identified and shooting radiotherapy 'blindly' at the prostate bed seems a bit risky. I would prefer to wait for my PSA to reach a level where the PSMA PET can 'light up' so the cancer cells can properly targeted with radiation. I know everyone is different and I will be interested in your journey back from cancer and full health. 👍

    Just had my procedure with the Davinchi system, but all of your excellent you never mention antibiotics as part of the remedy. Was also wondering if it’s normal to have some level of sour in my inner working area?Thanks.

    I had a prostectomy in 2004. Cancer WILL RETURN after 5 years, it returns in the prostate bed. I'm grateful my Urologist did the nerve sparing technique, due to my age of 52, at the time. I'm 72 now, single with no sex life except for an occasional masturbation event. Cancer DID RETURN in 2009, & after Radiation treatments, the cancer has been nondetectable. After 15 years of that, my erections are challenging, at best, however, I CAN have an orgasm with a half hard on, which is nice. I've noticed my penis is shorter though. I continue enjoying my weekly masturbation sessions.

    After you stop the medication you must start taking vitamin D3 30.000 IU with Magnesium 420 mgs daily. if you want take saw palmetto 550 mg because it will reduce DHT doing this it will stop the recurrence of the cancer.

    Thanks for your video my biopsy just came back shows I have cancer haven't talk to my urologist yet to know how bad it is just preparing myself when I meet when him it won't be for another 18 days Thanks again
