Funny the answer to why we don't prescribe creatine to more patients is that insurance and pharma companies only pay for randomized controlled trials that will maximize their profits. Since other more expensive drugs were developed and paid for, those are what get trialed and prescribed to patients. Big $ industry

I'm 82 & every day I workout with strength, aerobic & flexibility exercises. I eat a natural diet with no processed foods whatsoever. My execise routine & diet seem to keep me in good form but I am always interested in improvement. I had a talk with the proprietor of a supplement shop specialising in bodybuilders' dietary additives & he recommended me to take what he descibed as the best creatine he carried, the product he took himself & taken by his most succesful clients. Well, I took this stuff according to directions, tried less, tried more, sought more advice, kept taking it for 6 months ---- absolutely no improvement in anything physical I was doing or increase in any of my abilities. My conclusion - BS!

The ”how does it work” slide was interesting (at least for a cell biologist like me), with all the activation and inhibition, directly or indirectly, on specific steps, including effects on transcription … But, my belief was that the main thing is as an alternative fast way to drive ATP synthesis, when oxygen and respiration cannot keep up the need, a better alternative to fermentation (which may be fast but with low yield, and lactate accumulation) … but here you present a range of other mechanisms … I need to study it seems

I’ll be 65 in October and I’ve been taking Creatine daily for 15 months. I began weight training again 30 months ago, after decades away from the gym. I’ve found that 4 grams of Creatine daily is the optimal dose for me. My strength in various lifts increased by about 10% after a month of use. I prefer 4 grams because 5 grams daily elevated my serum creatinine above the normal range.

Thanks for sharing your personal experience! You should consider that amount of creatinine in the blood is related to muscle mass (i.e. males have higher blood creatinine than females) and dietary creatine intake. Both blood and urinary creatinine may be increased by ingestion of creatine supplementation and creatine containing foods, such as meat. These transient increases in blood or urinary creatinine due to creatine supplementation are unlikely to reflect a decrease in kidney function. In fact, a new Mendelian randomization study offers compelling evidence indicating that creatine levels are not statistically associated with renal function (pubmed.ncbi.nlm.nih.gov/38874125/). These new findings alleviate concerns within the fitness and general community regarding creatine supplementation as a trigger for renal issues. In 2019, Da Souza et al. also demonstrated in a meta-analysis that creatine supplementation does not induce renal damage in the recommended amounts - 0.1 g CrM per kg of body mass daily (pubmed.ncbi.nlm.nih.gov/31375416/).
