What they wont tell you about All-On-4/Implant Dentures
Board Certified Prosthodontist and Board-Certified implant Dentist, Dr. David McFadden, discusses several things dentists don’t - or won’t - tell you when you’re consulting with them regarding full mouth dental implant treatment.
The most popular treatment protocol currently for full arch or full mouth implant treatment is the All-On-4 protocol, (where they replace a full arch of teeth on just 4 implants.) While, on the surface, this sounds terrific, and it has become very popular treatment option due to heavy marketing efforts around promoting it, there ARE several reasons it is not the BEST option.
Dr. McFadden has been doing full mouth implant restorations for over 30 years, has lectured around the globe, held many, many lectures and live surgery courses for other dentists and has performed many thousands of implant treatments, including some of the most complex and challenging ones.
He is one of only 2 dentists in the US to be double board certified in implants (by the American Board of Oral Implantology) and prosthodontics (Dental Prosthetics), "by the American Board of Prosthodontics). This extremely rare combination of credentials and 34+experience yrs of experience give him a big advantage and a unique pool of knowledge that other dentists don’t have.
In this video, he reviews many of the things that are not presented to patients when they are seeking the All-On-4 Implant denture treatment as a way to restore their mouth. He arms you with the right questions to ask when seeking and discussing full arch/full mouth treatment options to help you navigate the conversation and decipher all the dental talk that can be so confusing.
He also touches on the differences between Implant Bridges vs All-On-4 Implant Dentures for replacing a full arch of teeth or a full mouth of teeth. Many people get confused and think an All-On-4 prosthesis is a (fixed) Implant BRIDGE, but it’s not; it’s a (fixed) Implant DENTURE. He shows cases and walks you through all the factors and variables involved in each.

    Hey again Dr. McFadden! I might have complications with tooth 31(root canal treated) and I was wondering if instead of doing a retreatment and new crown, if you would ever suggest doing an implant for 31? I think most of your restorations only include 24 teeth instead of 28, and I wanted to know your opinion on keeping 28, with implants

      @@davidmcfadden7927 yes I remove but all I am saying is your animated presentation is too much presenting bone removal. I remove just enough not to show transition of real gums plus gum prosthesis when patient is doing full smile or like gnarling smile that is why pre photographs are in a full smile and or sort of doing gnarling smile as one of the guides of course ct scan etc should be taken in consideration also. How much bone removal has no definite measurement it will all depend on how you think it will all end up pre surgery. So if you plan with end in mind you can come up with FP1, FP2 and FP3.

    Interesting video. I am trying to decide between bridge implants which are 3 bridges each on 2 implants or do the all in 6 procedure. My understanding is that if one or two implants fail with the all in 6 you should be okay. If one implant fails with a bridge then you would need to replace that section but at least it is only one section. Also, bridges do not need to be removed ever year to be cleaned and I have a lower price quote to do the bridges by 6K. Looking for any advice as it is a hard decision and I plan on doing next week.

    • @davidmcfadden7927
      @davidmcfadden7927 9 วันที่ผ่านมา

      We no longer do separate bridges. There are several reasons why which I believe would only muddy the waters. I'm glad you're doing your research but I believe you are missing the biggest point about the differences between implant bridges and all on 6. You will have massive bone removal with all on 6. You will not have any bone removal with implant bridges. This is the main reason to opt for implant bridges. Your precious bone is worth far more than $6,000.

    I agree with your thoughts. I have a few teeth that are bad, and the composite they put on my front tooth came off. I asked the dentist to add more composite or bonding as I called it. Boom! I'm referred to the Implant Place to get implants for $46,000! Why bother going in to get teeth cleaning 2x a year if it's for nothing?!? I'm disgusted. They had a big 3-D Xray of my teeth and 'See where you have an abscess there? It needs out NOW' 'OK should I take antibiotics now?' 'No it's coming out' Makes no sense. Nothing makes sense. I'm not falling for this. I'll glue my own piece of tooth back on if I have to. I call B.S. Thank you for your info. We're not that stupid, although we may look like it lol. I have a month old zirconia over zirconia prostheses with long face and just realized that sometimes I grind my teeth only on my left side closest to one of my implant. I don’t want to break my new arches. Do I have to order custom made night guard or I can get one from Amazon? Appreciate your response.

      A custom one would be a better bet for a proper fit. I wish you a lifetime of success and comfort with your new prostheses 😊

    @mcfaddendentalimplantcenter Did you say that All-in-4s are removable? How long will they last?

    • @mcfaddendentalimplantcenter
      @mcfaddendentalimplantcenter 3 หลายเดือนก่อน

      No, they are screwed in and only removable by a dentist. How long they last depends on several factors. Keep watching - I have several videos on this subject. They should help you understand all the variables involved, and different options available so you know just which questions to ask and which terms to use when discussing full mouth implant treatment options.

    GREAT JOB DOC! What if by changing the MUA to straight instead of 17 0r 30 degrees your access hole is on the buccal or in the tooth?

      In cases where we change the abutment for better tissue adaption and improved aesthetics which may place the screw hole in an undesirable position, we use angulated screw channels either from the manufacturer or Dynamic Abutment.

      It's going to take a lot of consults and a lot of leg work. Start with board certified prosthodontists who don't advertise All-on-4. And, remember, we are just a four hour plane ride away.

    I wish I had not found this information, NOT!!! I’m in the first year of the A on 4 process , and not very happy, this practice got my lower arch, wrong, because it loosens up, are. As well as the most front post is painful so I can forget eating corn on the cob or a apple. So as you can imagine, I’m the fish, hamburger, chili, or all the other things that we tend to eat with teeth problems. So tomorrow I’m calling the office to put them on notice that we need to fix this. FYI , my first two temps on the bottom broke, and I’m thinking that they should have taken the hint that something was wrong with the bottom.

    This all sounds wonderful. Knowing this already costs more than an all on 4 which is already outrageous, people can't afford to lay out anymore $$. Wondering what you would charge for a person needing 20 extractions and full upper and lower bridges? Just an average ballpark figure to give those of us contemplating this surgery an idea if we could even afford to have it done. THank you
