If it's over an open flame. It's still bad. It's the juices the drip into the fire and create the smoke. Cooking on the cast-iron or something on the stove, the juices stay in the pan instead of dripping down and creating that smoke.
The solution is you set up an indirect frilling zone on your grill and cook the burgers there. At the very end you sear them on each side for a few seconds.

I don’t have an issue with flame ups when doing burgers. Not a lot of charcoal and temp at 300 degrees roughly. Cook slow and they come out juicy and that great charcoal taste. No burning

My "Grandpa Burger" recipe: -Gently separate the ground beef into small pieces ( use min 80/20 or even 73/27) -Work into thick, 8 Oz patties with a minimum of handling, just to the point it will stay together -smoke indirect until @135 -finish on the hottest fire you have *You could do something very similar with a pan in the oven followed by a smoking hot cast iron skillet, if you think that's less cancerous.
