I'm actually pretty sure I figured out the Wormfaces. They are Guardians who, when rebirthed as Guardians were tainted by Deathblight. Guardians are the little guys with spears hanging out with Erdtree avatars. They often look like they have plants growing on them but their armor description implies that they are in fact plant people. It also outright states that they are devoted to the Erdtree and this plant person rebirth is suppose to be some kind of pact. Their armor set reads: "In accordance with an ancient pact with the Erdtree, it is said that their deaths led not to destruction, but instead to renewed, eternal life as guardians." And the rarer Full Bloom chest piece reads: "It is said that the blood-red flowers blooming on their backs mark the senescence of their ancient pact. Perhaps the guardians are part tree already." The Woodsfolk Ruins, I'm betting, was a Guardian town and as Guardians were rebirthed near Godwyn's deathroot they end up being born as Wormfaces.
interesting. no wonder the The Order was shattered, if Marika/Erdtree introduced "factual immortality" as a pact of rebirth. The lack of true death caused the outburst of death-substitute. There are many pacts and ways to "not die" in Eldern Ring, transferring souls via Glinstone is another way, Tarnished are also immortal. Speaking of transforming souls through the glinstone, maybe when the glinstone meteors fell to the Lands Between, they actually brought a "soul" of someone or something from who knows where. I personally believe that the golden residue of the Golden Order is a parasitic microbe from outer-space.

In contrast, the wormface enemies don't have wooden feet, and the feet of the Guardians also don't really resemble humanoid feet anymore, whereas this is not true for wormfaces' feet. The plants of the Guardians also seem to be growing throughout their bodies, whereas the marks on the "backs" of the wormfaces' bodies don't seem to run all the way through them, but instead stop a ways past the surface. Another key factor is that Farum Azula appears to have murals of wormfaces running above and/or below the columns of graves found on the ground in places. As such, these beings were known of during Farum Azula's construction, long before the Night of the Black Knives. The wormfaces in Farum Azula also look like they were mummified, implying that they were made from people who were already dead and buried. As such, the wormfaces would predate the Erdtree Guardians and exist due to something unrelated. Farum Azula also has murals of the Twinbird, the mother of the Deathbirds, and these Twinbird murals are prominently displayed on an exterior wall of Farum Azula, which can immediately be seen when the player first gets there from the Forge of the Fire Giants. the Twinbird murals appear throughout the rest of the city, and one such mural appears above the doorway to Maliketh's boss arena. Since the Deathbirds innately use Death Blight in their shriek attack, since it tries to fill the Death Blight meter, it's possible that the Deathbirds first created the Death Blight and that's why the victims of the Death Blight appear in Farum Azula.
@@nightscout9979 the gurdians spears have very similar engravings to to the gurdian sword spear. The chest piece they wear look very simliar to the wrappings/clothing that Wormfaces in farum azula. And the cloth on the gurdian mask is EXACTLY the same color and even have the same frills as the Wormface in the white cloth on top of all of that, there are gurdians near woodfolk ruins and gurdians turn into trees/wood. If you don't belive me go in game and look at the gurdian armor and weapon and look worm faces there clothing is very similar.

A youtube by the name shrouded hand covers it in a 13 minute video, i think this is where the idea came from. Thaeir outfits look very similar to the tree guardians, but perhaps are a different clerical rank hence the differences in their outfits.
This is a long story, started from the cursemark carved into Godwyn's body. The cursemark is only half of the full mark, taking shape of a centipede, which is the death sorcery symbol. The vague item description deliberately makes us think the cursemark is part of Destined Death and causes death. Actually it's not. The centipede cursemark's true nature is necromancy, it is deeply connected to the Ghostflame God and the Deathbirds, and its purpose is to give Life in Death. Now to further understand this concept, you must look at another Fromsoft game - Sekiro. In Sekiro, when the Dragon God shrugged off the lesser gods, the giant centipedes, to occupy the meteorite alone, their essence were dissolved into water and speard all over the lands through the water flows. The Senpou temple monks later attempted to produce the "rejuvenation water" from this essence and they were granted a form of twisted immortality by the manifestation of giant centipedes within their bodies. The concept here, found in Japanese folklore, is that impure essence can manifest within people through the consumption of such essence, and centipedes have long been regarded as an impure form of life. So, Godwyn's body has a centipede cursemark which gave it a twisted form of immortality, and from it came the impure essence - Blighted Death. We all know the essence of Blighted Death spread across the Lands Between through the intertwined root system of the Greattree and the Erd Tree, but there's still one more medium that most of us overlook - the water. From the water contaminated by the essence of Blighted Death come hideous life forms, like the Basilisks and Wormfaces. But unlike Basilisks, Wormfaces seem to be individuals of an ancient race, who died and were buried long ago, now brought back to life by the contaminated water and the impure essence of Blighted Death manifest in form of those gut wrenching worms attached to their faces. Interestingly, Blighted Death is not the only impure essence within the water flows of the Lands Between that gives rise to twisted life forms. Actually, any kind of impurity does. In areas contaminated by dirty or stagnant water, there usually are creatures like the slugs, giant crabs and giant lobsters whose drops include "white flesh", which indicates they are not naturally born creatures. Same story with Scarlet Rot and the Rot Kindred.

For what it's worth, Woodfolk in the japanese version is 森の民, the same as the Forest Folk in King's Field IV, which I only know thanks to Casitive's excellent video on that game. I don't think this contradicts any of your theory though it does seem to suggest that the name Woodfolk refers to their being of/from the forest. On an unrelated note, I found it interesting that the prayer site mentioned and shown in the video happens to be in the only location in Farum Azula with flowing water.

The wormfaces take bleed damage, unlike the Erdtree Avatars, so they're made of flesh, which their mouths also show. Addtionally, there are some minor Erdtrees on the Altus Plateau that lack any Erdtree Avatar, Tree Spirit, or boss in general, and one such tree is directly above where Godwyn's Death Blighted body is buried. As such, the wormfaces might not be versions of the Erdtree Avatars. Additionally, since the wormface boss at the Woodfolk Ruins drops the Speckled Hardtear, which is supposed to cure or otherwise heal status effects, that wormface could have tried to keep the otherwise unguarded crystal tear on its person in a desperate attempt to cure itself. Additionally, the Sacrificial Twigs could be held by wormfaces as a way to cure themselves of the Death Blight and die, but the twigs don't work on Death Blight, so the beings are holding the twigs in vain. You say that the Farum Azula wormfaces are trying to pray to the Erdtree, but they're not facing the Erdtree, or even Placidusax's tornado. In fact, with the way they hit their heads on the ground, they might be trying (and failing) to kill themselves, or to re-kill themselves. Speaking of Farum Azula, the city seems to have murals of wormfaces running along the upper (or sometimes lower) boundaries of the columns of open graves. A ring of these murals can even be found on the ground in Placidusax's arena, as well as on the wall running beneath the dragon torches in the Bestial Sanctum, though the Sanctum murals might be of beastmen turning into wormfaces. If these murals are indeed of wormfaces, then the victims of Death Blight, and the Death Blight itself, existed during the city's construction, ages before the Night of the Black Knives. As to how this could be the case, Farum Azula also has prominent murals of the Twinbird, the mother of the Deathbirds, on the outer wall of the city seen after the Forge teleportation cutscene. These Twinbird murals appear throughout the city itself, with even one such mural above the doorway to Maliketh's boss room. The only thing I don't know is how they are connected to the necroplague, perhaps as it happens with the mending runes, they acquired this form by putting their will and faith.

Guardians and Wormfaces clothing and weapons are almost exactly the same they are 100% Guardians
