    1. Natural skin barrier defect with tendency of inflammation and hyperproliferation of cells at ductal lining → microcomedone → whitehead +/ blackhead
    2. Pustule formation possible from inflammatory resp to keratin as comedone surfaces
    3. Aggravating factors (drives inflammation +/ sebum production):
    ○ UV light
    ○ irritating substance (palmate from hair care, heavy makeup)
    ○ skin barrier defect (dehydration, mechanical exfoliation, harsh chemical peels)
    ○ diet: high glycemic index diet, skimmed milk

    I have so many closed comedones... it’s so annoying. I never understood my skin more than I do now that I’ve watched this video. I finally understand why my skin sucks. Simply bc it does and it’s not necessarily anything I’m doing wrong, it’s just how my skin genetically is. Thanks mom and dad... ): at least I’m using a prescription retinoid. It’s the foam one. Loved the video!

      It's more likely 'fungal acne' / malassezia. Most doctors don't know about it because they are lazy and can't be bothered to read medical research. There are many treatments with varying degrees of success (e.g. Ketoconazole, Urea, Honey, Uva Ursi, and Olive Leaf Extract, Biotin) published in the journals of medicine. It's not the fault of your parents, it's cause by a low count of good bacteria in your lower intestine - which leads to low biotin production - which causes fungal overgrowth - usually from antibiotic use.

    I use azelaic acid to help fade dark spots from formerly healed pimples that left hyperpigmentation, seems to work quite well and fairly quickly.

      Khubu Jan I usually start noticing them start to fade after a few uses, they really fade after about a month. Just depends on how deep the scar is.

      Ugh, same.. I don't seem to have issues with lipstick, but I'm going to have to skip blush from now on. I went through all my makeup and I only have a few things that don't contain any red or yellow dyes. :(
      Time for a minimalist approach to makeup I guess.

    I sooooo wish I had seen this video 10 years ago. I remember when I got my first pair of glasses, I immediately started crying when I put them on; I had NO idea I had so many closed comedones or even what they were for that matter. Things obviously got better as they tend to do, but regardless I can’t wait to start using this advice in my routine to clear up what does pop up every once in a while. Thank you, Dr. Dray!!! 💕

    Here a comedone-prone skin! Yes, sugary or fried fast food make it worse, also scrubs. Salicylic acid and AHA helps a lot.
    Funnily, my chin and my right cheekbone are the worst areas.

      Mine’s my forehead and I kid you not, Cosrx AHA made it so much worse and yeah. Curiously enough, what helped lessen its appearance is my use of moisturizer + vaseline (during night time). I still have them though 🥲 I’m going to go to a doctor soon to have a checkup. Might use tretinoin if it’s prescribed to me.

    Your description of closed comedones fits my skin to the T. I'd been suffering from closed comedones on my forehead since I was in middle school. In my effort to eradicate them using AHA, BHA, scrubs and Clay masks almost DAILY, I triggered closed comedones on the rest of my face and bonus:ECZEMA. I got rid of everything and only used micellar water and moisturizer. It took me half a year to get them under control. Never am I ever going to make the same mistake again.

      I can’t exfoliate with acids or retinoids either, my skin gets super dehydrated and produces more oil. I “wash” my face with cold water after I workout and use gentle makeup wipes to remove makeup and lightly “exfoliate”. Using cleansers and acne products freak my skin out.

      Just finished the video and wanted to provide my experience: Before I learned about the importance of the skin barrier, i was throwing EVERYTHING at my closed comedones. Treinoin, bp, aczone, bha, and aha EVERYDAY. Without moisturizer. I cringe. Of course, they continued to form and i did not understand why since I was using all these science backed ingredients! Then I read a blog post about the importance of the moisture barrier, quit all actives for a couple of weeks (during which all my cc’s disappeared...but then started to form again I guess after the initial sigh of relief from my skin lol and bc of its natural propensity to form them ) and reinitiated a simplified regimen: tretinoin every other day, salicylic acid, and aczone. Also use cerave cream in the tub which my skin loves, layering it over oils such as squalane and sealing it all with vaseline at night. The result has been far less comedones and when they do form, they leave immediately (often through mineral oil cleansing massage which i do every night followed bt vanicreams gentle skin cleanser ofc. The clogs pop out in little grits, its amazing).

      @@motongever I use a Curology mixture which contains tretinoin as well as dapsone gel (both prescriptions, although differin is an over the counter retinoid you can try), wood snow mineral oil as a pre cleanse then vanicream cleanser, and cerave moisturizing cream for moisturizer. i use the Curology at night and the dapsone gel AM & PM.

    THANK YOU. I have been hoping you would speak about this topic. This is my current struggle. I have been watching your channel for a while and have stopped wearing make up. I started using sunscreen and gentle cleansers, BP, Differin, SA (all brands you have recommended). But I feel like I might be going overboard and making things worse. A skincare routine for closed comedones would be wonderful and so so helpful!!!! LOVE YOUR VIDEOS. KNOWLEDGE, AND ENERGY!

      Claudia Gabrels those are a lot of actives when youre also using a retinoid which is not good for your moisture barrier. I would scale back the actives to just the retinoid and one active per day. I used to do the same thing as you and now that ive scaled everything back to the above and focused on maintaining my skin barrier (i use vanicream gentle cleanser and cerave in the tub) theyve improved way more than when I was throwing everything at them

      Having heard of washing face with only water, initially i felt that it wasn't clean having to live in an all summer and humid country singapore. However after doing just that, every single morning, the natural oils that our skin has actually naturally clears out my comedones. I double cleanse at night, and keep things very basic. And drinking water alot, helps me too.

      I have PCOS too! One great thing for me (unfortunately not for acne but a lot have had luck) is S’moo. I’ve been taking it for about 6 months and my energy has been better and I actually went off of BC (because it wasn’t helping with acne anyway and I wanted to see if my cycle would regulate itself). So far I’ve been successful with two cycles (they’re 50 days long but they’re even!). I should be hitting the time where it’s staring to regulate even better (I’ve been off of BC for about 3 months). You can always try that!

    Hi! As always, an informative and interesting video. Even if you don't have this skin problem, you still know someone you know who has an acne problem.     Ahh, I finally found that this has been the problem with my skin for the past several years! I couldn’t figure out what they were called, I just assumed they were clear pimples so thanks for making this. I have been popping them and using scrubs, but now I will try these other suggestions and stop doing that. I also use blush everyday and have wondered if that’s a part of it. Thanks!

    I suffer from closed comedones and it's really annoying to treat and to look at. I really.want to thank you for your thoughts and expertise in this area.       Something that has helped me, is keeping my skincare routine simple. I feel like we do too much. Also, using just my fingers, I cleanse for 60 seconds. It makes a huge difference. If you feel like you need more exfoliation and fingers won't cut it, the Mint Sonic cleanser tool from amazon is amazing

      The only thing they do is perscribe shampoo that has a chemical exfoliant and creams that temporarily keep the symptoms at bay, usually untill you stop using them.
      My scalp psoriasis was induced by a chemical burn 6 years ago and the best luck OTC ive had is using cerave on my scalp and the blue shampoo or mens dandruf shampoo with layers of AVACADO oil. 💞 good luck 💞

      Please see a dermatologist for an exam? If you do have psoriasis, you are at a higher risk for psoriatic arthritis, and there are medications that can help prevent joint degeneration.

    Sometimes we could be very dramatic. I mean I see a pimple and oh Lord. But when I learn how many diseases are out there that truly can affect the life of some people it breaks my heart and I realize how good I have it. Skin diseases should be taken seriously. Diseases can have a huge impact in people's mental state. Thus, adding a mental disease to the combo. I pray the day skin diseases are taken as serious as any other disease in our body instead of cosmetic, beauty drama. Feeling good about yourself is as important as having a healthy heart. Some of this diseases are not just ugly but painful.

    Once again, Dr Dray, thank you. You have just explained those bumps on my temples. I’ve been wondering for months what exactly those bumps are directly to the left of my left eye and vice versa on the right. I use sunscreen and moisturizer and do not use scrubs, but I do apply acids in addition to retinoids. I bought ZapZyt wash after discovering your channel to use once in a while, but now I’ll leave it on longer and pick up some Azelaic Acid to go along with my tube of salicylic gel.

    So helpful. I suffer from closed comedones, battle an autoimmune disease and am on immunosuppressant meds so this was very informative. Will definitely look into your suggestions to deal with those I have and prevent future ones from forming. I don't have a sweet tooth so nothing I have to change there...yay!

      Have you tried eco egg I use and it has no nasty chemicals, lasts for ages. Just takes a bit of getting used but it's been years, my washing is clean, and skin not irritated. I use an eco softern as find it doesn't much smell, only downside but otherwise really happy with it.

      I've been using Molly's Suds as well as the whitening powder from them via Amazon for several months. Our clothes don't seem to be fading nor stink despite the detergent having seemingly safe ingredients & so few of them. No kids here so I can't comment on crazy stains but that may be covered in reviews.

I have seen some dermatologists who told me that because I am acne-prone (and I get a lot of closed comodones), I don't need moisturizer because my skin already develops a lot o oil.     Omg, I went to a lot of dermatologies during the last 5 years of my life and none of them told me that my kind of acne was a comedonal one, AHH! I'm so frustrated my mom really has spent a lot of money on a lot of treatments for my acne and none of those worked. Now that I know what type of acne I have I would do more research about it and find a skin routine that hopefully works for me T.T.

    What a brilliant presentation! Thank you! So much useful info here.. I think many of us have mistakenly believed that over enthusiastic exfoliation and sonic brushes were the answer to closed comedones and pimples generally. I’m convinced that such practices, combined with inadequate sun protection brought about my rosacea when the results collided with menopause. My skin was ok till I started to use a Clarisonic.. within two weeks I was flushing uncontrollably and the rosacea had began. Now, three years down the line, gentle skin care is making the nightmare way more manageable. I have to add that I’ve been taking Low Dose Naltrexone for the past 12 months and that seems to be helping too. It helps to stabilise the immune system...I’m wondering, based on what you’ve said here about the role of cytokines (which I believe are over produced in rosacea) if the LDN might be calming the over reactive immune response in my rosacea skin? I’d really value your thoughts on the role of LDN in certain skin conditions, as no dermatologist or Doctor I’ve ever seen is able to offer any thoughts about it at all.

    Thank you once again for your advice. Every time I am watching a skin care program or looking at skin care, they are always saying you must exfoliate .. every day if possible. I think that is so harsh and especially in the winter time there is no way I can do that. So am glad to hear that you don't advise exfoliating.

    I have always had skin prone to acne and comedones. Since I' ve started watching you (about a month ago), I stopped using scrubs and my Foreo brush. I started using a different mosturiser (one recomended by you) and a suncream and I started noticing acne going away. Thanks for all your advice. ♡♡♡

      @@cloudsinmycoffeeeee Sure! I started using the Neutrogena Hydro Boost. It's supposed to lock the moisture in your skin and seems quite effective in doing so. I usually follow up with a day/night cream by Bioliq. I have not seen that one in her recomendations, however, I purchased it before finding Dr. Dray's channel.

    Hi Dr. Dray! Update on my fight against closed comedones at 59. What a battle. I discovered CosRx Whitehead Power Liquid. It really works for me! My plan is to use it every other night and Retin-A the other nights. After using it for only 2 nights I am starting to see improvement. It doesn’t dry out my skin and is pretty gentle. It stings when first applied but feels okay after 5 minutes or so. I hope this helps other folks out there who are frustrated with these stubborn whiteheads. I should add that I’ve been looking for an effective treatment for 4 years.

    Brilliant video, and I identified with so much of what you say. My whole problem with closed comedones became apparent after having microdermabrasion a couple of years ago and it ruined my skin. It took several months to fix the damage and thankfully now I'm using the right skincare routine I can manage it but lesson learnt the hard way! It's such a careful balance between keeping my skin hydrated with the right moisturiser but not using anything too heavy/rich that exacerbates the clogging! Retinoids and salicylic acid have both been fantastic for improving my skin in combination with a basic routine to support the health of my skin barrier.

      @@veggiesaurus5834 I use my own brand of rinse off cleansing oil (oil to milk), for moisturiser I love Eucerin 5% urea face cream over my barrier support serum to help tackle dehydration and keep my skin barrier healthy. I wear bare minerals complexion rescue most days as it's really light/sheer and hydrating. I don't enjoy the feel of foundations on my face so only wear them occasionally on nights out (in that case NARS radiant tinted moisturiser or sheer glow). Hope that helps!

      So the gel helped with my comedones! I don’t remember the last time my face felt so smooth. But, the purging period was a little intense for me... especially around my cheek area. I had a hard time not picking at my skin during the purge and that left me with some intense post inflammatory erythema... however, I got some azelaic acid from the ordinary about a month ago and that has helped tremendously with the redness as well as other acne that arises. I think another month in and my skin will look good as new!
      I’m glad I started Differin but it definitely made my skin worse before it got better. Wearing masks all the time helped me get through the purging period but it was tough. I definitely recommend looking into an over the counter azelaic acid once you’ve used Differin for like a month.

    Love this video! Thank you! Yeah, I hate testosterone 🤦🏻‍♀️ I’ve been on Sprironolactone for almost 2 years. Acne is a bit under control, but I still breakout. I even wash my pillow cases every other day with All detergent. No fragrance and no dye. I switched my diet to low glycemic index food about 3 years ago, but I noticed quite a difference 6 months ago when I cut dairy out. I love cheese, but I limit dairy because I have noticed I will get acne. I recently switched to coconut yogurt rather than Greek yogurt; unsweetened coconut yogurt was difficult to find around here. I love yogurt and I hadn’t been consuming it for a long time. I was happy when I found coconut yogurt. Lol! Have a nice weekend!!!!

    Could you do a review of some “primer” sunscreens? Such as the La Roche Posay Anthelios 50 Daily Face Primer with spf 50. I’m wondering if they would be good options even without makeup

      Rebecca Nas Rebecca Nas thank you for your recommendation, but this simply isn’t true. A better way to word it would be that most people will not be applying spf makeup (namely foundation) in the proper amount and coverage needed to get to the labeled SPF of the product. They are not technically “less effective”, just not applied correctly for sun protection. Primer is a different situation though, which is what I asked about. I suppose you could say it’s makeup, but it is generally applied in a manner more fully, like a moisturizer or sunscreen would be, so one would think this may be an acceptable and “effective” option for the face even without any other makeup, like I said in my comment. So my question to Dr. Dray still stands!

    Thank you SO MUCH for doing this video. I’m so happy you informed us about it. I have been struggling with closed comodones for a long time and it took over my self confidence it was horrible. I was able to get tretinoin for a little with curology so I know it works well but I couldn’t afford to pay for the monthly subscription so I finally got health insurance again and plan to go to my dermatologist to figure out if I can get a prescription of tretinoin again. Before I did that though your videos SAVED my skin, especially since from you I figured out my skin was not getting enough moisture so I started to use a humectant while my face is wet and ALSO moisturize and also use SPF everyday so little by little you have helped me so much and I just can’t wait to finally go to a dermatologist and figure out everything. Thank you for giving us great information that really helps. It would be awesome as well if you did a skin care routine for someone who has closed comodones so we know what to use when and all that. Also separate them from oily to dry skin? If you possibly can. Thank you for being you Dr.Dray it’s so helpful ❤️❤️❤️❤️❤️❤️❤️❤️

    Extractions helped me dramatically with closed comedomes. They wouldn't go away even after I solved my acne breakout problems. Turns out, all they needed was a clean extraction and my skin texture became 90% flatter over the course of just a week.

      @@iqrabhat645 No, that's a waste of money. Buy an extraction kit and do it yourself, and just maintain sanitary procedures. Also make sure you use proper technique to prevent skin trauma.

    Help Dr. Dray! You have been a life saver for me. I’m in my 30s and I’m breaking out for the first time. I was overdoing it with scrubs and masks, but I’ve been trying things you recommend and it’s helped! After watching your videos I realize I have super sensitive skin and my skin barrier isn’t great. Is there a vitamin supplement or some specific kind of lotion I should use? I bought the CeraVe PM and it tingles a little. Is that normal? Thank you SO much for all your helpful videos.       Same. It happened when I hit 25. My doctor said that this is commonly seen for some reason and he prescribed adapalene which helped SO much. BHA and AHA helped initially but it wasn't until I went to the doc that it really cleared my skin.

    dr dray, i just looked at the ingredients for cerave moisturizing cream and was surprised to see petrolatum. the official site lists the product as oil free but petrolatum is mineral oil. more importantly, i couldnt find anything on their site that lists their petrolatums refinement history to show its purity, thus proving its truly non toxic with polycyclic aromatic hydrocarbons. in the eu products cant contain petrolatum for this reason unless the refinement history is known. i know the us doesnt require this.

    I’ve been waiting for this video for soooooooo long! I have these nasty little buggers on my cheeks and around my jaws. Tretinoin really helps keep them at bay but there are some really stubborn ones on my cheeks that have been there for literally years that just don’t go away! I use Tretinoin, salicylic acid, Sulfur, etc. nothing works! I also drink 3.5 litres of water a day, take zinc supplements. Should I just go to a dermatologist to get them extracted? They seem really deep in the skin and they don’t appear white at all. Just skin coloured bumps. 😫

      Mine are similar, but I'm not sure they're CC. As yours are, mine are very deep in the skin. So much so that sometimes it probably isn't even noticeable to others unless I point it out. Mostly I can *feel* them when I drag my fingers across those areas. Furthermore, I only have a few. Like a couple of the cheek, 1 or 2 on the chin, 3 or 4 on my forehead (near the hairline). I just hate feeling them every time I wash or moisturize.

    Long time Benzoyl Peroxide hater here. I recently found a BP wash that has been absolutely transformation to my skin. If you're one those individuals that has had a bad experience with Benzoyl Peroxide, I would still highly recommend a 5% BP wash. So, so much less irritating to my skin, but still effective.
