He's 8 feet tall and built like a truck, plus he's got an angry-looking natural expression, but he's genuinely one of the nicest characters in the series. Nursing student who is also ace and likes ace attorney here! No clue what Hotti’s diagnosis is so it’s a bit difficult to say what the best course of action would be. For addressing patients with delusions tho, often the best course of action is distraction. Don’t agree with the delusion, but also flat out telling them it is wrong may be distressing.
So in this case, if he is claiming to be a hospital director, I’d give him a task to do, like a coloring page or a word search, so he gets distracted and hopefully dismisses the delusion on his own.
I would also make sure he doesn’t have access to hospital badges or lab coats, or anything that would make him look like he has authority in a hospital setting.
As for his diagnosis… it can go either way. The itching can be a sign of some sort of infection, which may cause delirium depending on the severity and cause. Or the itching is a nervous tic, or the sign of a hallucination (ie thinking you feel bugs crawling all over you).

Crazy looking at all of the Trilogy Designs btw because my favourite of the Ace Attorney games is The Great Ace Attorney Chronicles and while designs can get weird there, they are NOTHING compared to the weirdness of Trilogy Designs at times. Totally forgot some of these characters BAHSHAHAHA. TGAA has less men being built like trucks though (and it has Kazuma Asogi who would've solo'd this ranking for me I'm pathetic)
Excited to see it if you both do this again with Apollo Justice! It used to be my favourite Ace Attorney game so I hope you all have fun with it, plus it has some great designs!
As a fellow Ace Attorney fan who's also ace, I agree with 99% of your choices (the single different choice I would make would be the Lotta one, I can see the appeal, and I actually do like her character, but yeah, it's still a pass for me 😅). I'm glad I'm not the only one who would choose smash for Larry (game 1 and anime in general, he is The Sokka-like childhood Friend, and I just love Sokka from ATLA! But yeah, why did they do him so dirty, he became intolerable 💔) and pass Edgy-poo (I adore him as a character, he is my boy, but indeed, he is a HARD PASS, this man is dorky in both the best and the worst ways possible).
And yes, indeed, there's soooo many TGAA characters that are a smash without a second thought, I mean, Sithe????Herlock???? Even Yuujin???? 😳
And, of _course_, the one and only, the one we all know, as you say, that would solo this and any Ace Attorney related smash or pass ranking, Asougi Kazuma and his logic defying hotness. Seriously, a guy with a bowl cut and suspenders shouldn't be that attractive, that is not legal, wtf. One could even take it up further and count his three separate sprite designs as three different people, just like Feenie and Phoenix are counted separately, and all three of them would be the Top 1, even with the one with mask and cape. Well, Prose-Cutie Asougi would be the Super N.1, I suppose, but like, he is objectively the hottest character in all of the AA franchise, as he has the overall Best Looks™ paired with The Personality ™ and Charm™.

You are SO right about so many TGAA characters being smashable, the designs are just so good in those games (give it up to Kazuya Nuri, he is just a fantastic character designer.) Sithe is so incredibly pretty it's unbelievable.
EXACTLY how I feel about Kazuma. WHY is he hot, he shouldn't be, it's so unexpected, he's LAME, he can't say tongue twisters, he has an undying passion for Ryuunosuke that is almost embarrassing, and he's still just slaying the whole time in every fit. When he showed up in that last outfit my jaw dropped, I couldn't believe it. Truly THE number one attractive Ace Attorney character.

In a smash or pass though, I'd probably say pass? He might be fun to talk to and his design is definitely really awesome and cleverly done, but he's not my type I'd say. Super interesting character though, very silly, always moving around the way he does
