Your number 2 (the logout by a monster "glitch") was actually introduced by a corrupt J-mod. Even when people told Jagex about it that J-mod his friends simply randed in such a topic and bump up other topics and lock the topic by other corrupt F-mods.
Just to clarify I used Mr Bug's footage on number 3 because I couldn't find any footage of the glitch itself or any other footage of people actually playing the minigame. He is NOT the one who abused it. :)

iirc, the ToB glitch always dropped your 4th slot item (Top-right), and it was happening all across twitch streams. Most streamers I saw only had combat potions or something like that in that slot, so it wasn’t really noticed until someone dropped a tbow (I think it was in Cloud Badass’s party)

The key was dropped but being it was still normal worlds there were no deadman chests to loot the bank key on. The items were still taken from the players bank but no one would ever get them.

The invincibility glitch was not a glitch. This was code that was included on purpose. This was from a jmod that was fired when they discovered what he had done. It was mod reach. It is pretty well know that this was not done on accident.

That’s not known information, and at least could’ve gave my channel a shoutout, similar to DVS did when he made a similar video as my “10 Best Achievements in OSRS” video.

In reality, it did happen. There is video evidence of it happening. A rollback doesn't mean it never happened. A rollback means the negative consequences of said glitch were removed, thus having the same effect of it never happening. Doesn't mean it didn't happen.
