    I have torn both ACL and MCL - grade 3 - both of them. My knee was completely unstable, falling out. At first I was recommended surgery for MCL , then rehabilitation , then ACL surgery. I decided to let MCL try and heal without surgery and started with rehabilitation in the first few days and now after 3 months it started to heal. It took longer than usual since it is combined injury but it did start to heal. I also changed my surgeon who recommended for us to wait and that surgery is the last option. I will see him again in about 3 months ( 6 months from the injury) and hopefully by then my MCL will be healed completely. He suggested that you can pretty much normally live without ACL, I am 41, female, go to gym , but I am no professional. So - it is possible. My knee is stable, i walk, i drive bicycle , pretty much normal life - I am careful still, since it is still not healed. So - it is important to pick a right doctor , do your rehabilitation and let time deciede what is best for you ( not talking for professional athletes of course). Good luck to everyone!

      Mine has been torn for over 9 years and my knee is still unstable. I never had it corrected but I certainly can't run. doing jumping jacks the other day I felt it wobble a little bit.

      I have similar situation like you. MCL grade2 with complete toen ACL along with bone bruises. I am seeing my orthopedics in two months. Right now healing my MCL and building up muscles on the leg. I am open for surgery. Am also 41years old female hitting CrossFit box three or four times a week pre injury. I can’t bear the idea that i have to live with worries the rest of my life. As ACL is an important part of the structure.

    One of the most comprehensive videos I’ve seen on the subject. I partially ruptured my ACL during Thai Boxing training years ago. Decided against surgery and went into rehabbing it instead. Best decision I made as I don’t have any issues with it. I don’t train in Muay Thai anymore but it was a small sacrifice to make for knee longevity.

      @@treeland5162 While sparring I went on my toes with the left to throw a right kick. At this point I felt all my supporting muscles surrounding the left knee just relax up. Felt the pop and fell on the floor like a sack of potatoes. Not too sure why it happened. Maybe I was overtrained or didn’t pivot properly on the left. One thing I wish I did looking back now was a comprehensive strength training program for the legs with lots of unilateral work. Maybe it would have helped with injury prevention. How did your injury occur?

    Begining of january this year I tore the left ACL and broke the meniscus, had a strong pain and a swollen knee for a couple of days, then I got MRI and went to see an orthopedic. I was super worried because at 59 I still work in physycal theatre and will have a lot of work during the summer. Since I had no pain at all and I recover the ROM very quickly he opted for rehab for 3 months then we would decide if surgery is a better option. I just had my first performances of the year and long mountain hiking, and my knee feels very stable and well. I still don't feel like trying lotus postures while practicing yoga, but I'm so happy about the recovery, I had no idea I could recover without surgery

    I had an ACL reconstruction surgery on my right knee done by a descent orthopedic doctor 10 months ago and my knee feels pretty stable after doing all the rehab I was assigned to do. I highly recommend you to undergo surgery if you want to go back to playing in a highly competitive side of the sport.

      I needed to see this comment I wasn’t sure if surgery would be good . I’ve heard so many stories of people’s knee not healing all the way and lifetime pain. I don’t want to wait too long . I’m only 28 and I would be upset with myself if I let it get worse. I have a torn ACL and torn meniscus. I’m a little nervous but thank you for this comment it was a sign that things will be okay ❤

      @@The-DAWG957 I worked mostly on strengthening the muscle around the knee with exercises like squats, lunges, and calf raises etc with weights to increase stability on my knees. Also gradually increasing the range of motion of the knee with squats as well. Overall, using the correct form during those exercise is cruicial when it comes to avoiding excess injury during rehab

      @@tootiefruity77 I'm glad this helped you 😊 One of my soccer teammate also suffered from ACL + meniscus tear and took surgery. She's been showing progressive recovery through rehab and dedication to return to the sport. I think most people who fail to recover from the ACL surgery are the ones who don't do the rehab consistently (which may be boring or painful) to regain full range of motion, strength and balance. Not strengthening the legs lead to bad biomechanics which you may know from there - you'll be at higher risk of injury. Other reasons could be overstretching the new graft by pushing too hard too soon or doing the exercises in an improper form, which may lead to loose graft and potentially causing arthritis which could be the source of the knee pain. As I mentioned before, the earlier is better in order to heal faster and properly. I started my rehab 2-3 days post surgery (starting at walking with crutches and wiggling my toes as part of my exercises), and it's important that you choose the right surgeon to operate your knee.     I toar my ACL 18 years ago my knee stability was never the same and as years have gone by it's now takings its toll ..I cant run anymore now but I did my own rehab to the point that I could run and jog daily and did a lot of leg exercises to build muscle strength but never got the stability which a surgery could have got me .I still walk at a fast pace knee strapped obviously but always have moderate pain on my MCL. I'm 53 now and took a chance and didn't have surgery...I cant pivot and jump down from high places or play any contact sports..so ask yourself do u want to live your rest of life with or without surgery .

    Great presentation. I am a 59 year old with a complete tear of the acl, bucket handle of the medial meniscus and complex posterior tear of the lateral meniscus. I am a 45 year history of working out and am shocked how much stability I do have. Nice to see the current data without geeking out on Pub Med.

      @@amansinghmtr2675 20 years ago for medial meniscus and partial acl tear. Complete acl and lateral compiles posterior horn of lateral meniscus is the last 3 months.

    If you do surgery you must do rehab in addition to pt sessions. I had PT 3 days a week and went to the gym to do rehab 3-4 in addition

    I’m 22 and I just tore my ACL. I am devastated thinking I’m never gonna be normal again. I’m gonna get X-rays and an MRI and if I need surgery I’m gonna take it. I don’t want a fucked up knee because it feels so uncomfortable. Id rather be comfortable and broke rather that uncomfortable and rich meaning I’d rather spend every last penny to make sure my body is in tip top shape

    Can you make a video on a partial acl tear please?
    I'm stuck in the endless loop of trying to rehab partial acl tear hopelessly, I keep having setbacks with pain that make me stop and start over again as muscle starts going away.
    Please make a video on how to decide if surgery is needed for partial tear, how to rehab, how long to rehab, rehab mistakes, last time I was doing 15kg squats, maybe I went too low or did too many reps, my acl knee hurts a lot and weak/clicks again.

    Hi E3 Rehab, thank you very much for great and informed content. I just wanted to ask you, I am a 36-year old after ski knee injury now waiting for my MRI, with possible ACL damage. I´ve been doing olympic weighlifting (semi-proffesionally) for a couple of years and I can´t find any meaningfull info whether and how I can potentially get into the sport back. Most information is revolving about competitive sports as soccer etc. Do you have any experience with this sport? Thank you, David

    This is the best video for this topic I have ever seen in TH-cam .
    I had a full ACL rupture while playing football ( soccer). And I was advised by my physio not to go for surgery and I did all those rehab and now I m back playing. Told by 3 ortho surgeons to go for surgery I should be thankful towards my physios for there effort in my non operative rehab and increasing my confidence after that.
    ( Note : the decision to have surgery or not totally depends on various tests physio do during the rehab process. Always consult your physio)

      So when you say rupture you mean completely sheared off right as opposed to a tear which is still connected ? How can you recover from that you basically just don’t have an acl now ?

      @@nickk707 yes it's complete tear but as per my physio some people's body are able to handle physical sports without an ACL like with other tendons, ligaments and with support of properly trained muscles. I repeat this decision should be taken by a professional physio and not by self medication and there is some conditions for this like for example u should be able to have strength in Ur leg muscles like being able to lift 2x Ur body weight ( one of the example). And I am just posting this comment so that people will be aware of this kind of things exist.( When I was injured I hadn't had any knowledge of such things I was only aware of surgery part). Hope this was helpful to you.

    I completely snapped my ACL when I was 24 by landing incorrectly from a jump. 7 months ago I had reconstructive ACL surgery and I’m currently on rehabilitation. I’m doing loads of cycling and tibialis raises. Only downside atm is I wake up with a stiff knee every morning and struggle to go downstairs with my bad knee. My knee is still swollen and I struggle to straighten it out. Other than that I’m doing ok

      Same happened to me. It will get better after 15 months or so. I no longer wake up with stiffnes

    8 months after tearing ACL and medial meniscus on my left knee from a ski accident, I feel pretty stable but it hurts because of the meniscus. Can't fold my leg without pain or walk without pain sometimes, though I feel like I don't need the ACL reconstruction.
    But if I go repair \ remove that torn meniscus then I'll just go have the ACL reconstructed.

    Soooo, I Completely severed my ACL. There’s no distinction in this video that says whether the people were partial tears or a complete tear. I’m also not overweight and I didn’t tear anything else besides my ACL. I have been doing physical therapy and I’m almost back to normal. I think I’m going to bypass the surgery.

    Back in October I was diagnosed with a "complete tear" of my ACL. I tore it in a BJJ competition. It was loud and my leg moved in an unnatural way. I'm 46 and always thought something like this would end my athletic life.
    It was weird, I didn't really have any symptoms. I could run, squat, hop on the bad leg, nothing really seemed wrong except the occasional feeling of some mild instability, no buckling or anything.
    I went to the ortho more as a precaution than anything else and was shocked and depressed by the diagnosis. Anyway, in large part due to this video, I decided against surgery right away and rehabbed it for a few months.
    Soooo glad I chose this route. I'm not sure if the ligament healed, but my function is better than before the incident.
    The mental aspect of the diagnosis turned out to be the worst part of the whole ordeal. Thanks a bunch!! If I'd have gotten the surgery, I'm sure I would have been worse off.

    HI Doctor, My MRI repost after a sport related injury where i felt that my upper body moved from knee where my keen remain same and i fell down with pain ... and its been almost a month so today my MRI reporrt came
    I am 47 and the MRI report shows
    Features of near complete tear of the femoral aspect of the anterior cruciate ligament with ill defined margins and large area of breach along its femoral aspect
    Features of grade iii tear of the posterior horn of the medial meniscus|
    OTHER LEGAMENTS ARE ALL FINE
    Doctors saying need ACL surgery and he will shave some back side of meniscus
    is surgery recommended and will i feel better after surgery ... pain is too much since one month and i am taking pain killer and anti-inflammatory medicines

    I have a full thickness acl tear and horizontal tear in posterior horn of medial meniscus. But i can still walk, jog and drive (some pain) should i undergo a surgery or continue with rehab

    Hi,
    I'm a 24 year old student-athlete soccer player, and played almost my entire life soccer at the highest level in Belgium without any big injuries like this one, until recently an MRI scan showed me I torn my ACL in my right knee. If they told me the news I couldn't believe I had to do surgery, simply because I believe the body is strong and smart enough to heal from it and get back to its original function as I experienced my whole life with little injuries related to soccer.
    Currently I'm in my 4th week of rehab and I got no pain, except when I force it in a position of rotation.
    If there would be anyone with some advice related to this situation, it would be much appreciated since making a decision is not easy.

      @@takeshisendo3767 yes, I did. I'm 14 days post surgery and glad I made the choice. It's still the very beginning, but I'm taking my rehab very serious and can say there is already a big improvement. Let's see what is will bring in the long term. Btw if anybody here got good articles around forming scar tissue, feel free to send it.

      As for me, I'm here with a partial acl tear since 1 year and I just couldn't he6t proper treatment/rehab for various reasons, but since last week I have went to a new physio and hopefully I won't need a surgery.

      Hey I’m 28 I did my acl when I was 16 I didn’t even visit the hospital… 12 years later I’m still playing at a decent level but I must have a knee strap otherwise I risk injury it’s a pain but I believe you can come back even without surgery

    Are u speaking on complete acl tears or partial?
    I recently had reconstructive surgery on my complete tear (currently 6 months out). Prior to that I spoke with 3 highly regarded knee sports doctors in NY and they all said I would be unable to play basketball competitively again unless I got surgery. I was disappointed bc I believed that if I dedicated myself enough to rehab I’d be able to heal without surgery. Thus, if u are referring to complete acl tears throughout this video it would confirm what I had originally felt prior to my surgery.

      If it’s completely torn you need surgery otherwise you’ll be living without an acl and will have an unstable knee

    I tore my acl full thickness, mcl and lcl partial, median meniscus longitudinal tears. I did not get a surgery. I was sceptical initially about my recovery, as it was a year since i could walk without instability. Although i am not playing competitive sports anymore, i could rehab my way to do regular exercise, daily activities etc.
    Ps: More evidence of spontaneous healing has been in the last year. I also think more research is needed in terms of prescribing powerful NSAIDS, as they completely stop ligament tissues from responding to platelet-derived growth factors.

    I’m a 15 year old female who plays semi-pro basketball. I tore my ACL fully a month ago and am having trouble deciding between doing surgery and not doing surgery. I don’t have any instability in my knee and can straighten and bend it completely. I want to return to playing as quickly as possible. What do you think I should do?

      Okay, well I don't have any information regarding your physical condition or your goals so it's pretty hard to tell but as this video already explains, a surgery is not the easy way out and it will take you at least 6 months to be able to join your training again, at least that's the average, and to heal it will take even longer. Good thing is, basketball is a non contact sports but on the other side it involves a lot of jumping. You mentioned you don't have any instability in your knee. I don't quite get how that's possible with a torn ACL since you must have some kind of instability there or how did you get the diagnosis? But apart from that I would recommend you to do strength training, focus on quads, hamstrings, abductors, to secure your knee joint, as a backup so to say. And when you have fulfilled the progression criteria include some plyometrics as well to prepare your knee for the return to sports. There are plenty of videos on this channel you can get some information. Wishing you success :)

    I am 55 and I am not getting surgery I had it done on my right knee 30 Yeats ago and it's a long recovery and now it's stable but lots of arthritis and I do not play sports and if I did the patella tendon pain would have limited that anyways

    I had a question, i had ACL in the year 2018 i think but i did not get surgery and after a year of rest from basketball, i decided to play again, until now i am playing and sometimes my knees gets pain but its tolerable... is it okay to continue playing baaketball?

      It appears it’s fine for you to play basket ball. Just ensure you also do weight based exercises twice a week to improve knee stability and aid your knee in dealing with weakened acl

    There's a shocking lack of quality evidence with respect to so many surgeries. Sham surgery studies are such an eye opener, and we know the power of placebo and a patients belief in a treatment are extremely powerful predictors of outcome. The power of sham is incredible, for example the GERAC studies for acupuncture just poked people with toothpicks or something pointy and did them both in the same patterns as acupuncture but also did them in random places, and compared to needle insertion to high level acupuncture standards and random points with all roads leading to the same outcomes. It's all in the head. I remember reading a paper about medicine globally/generally needing to embrace the power of placebo instead of only seeing it as a nuisance for research. We aren't cars, replacing parts isn't as simple as being a "fix", a surgical procedure is an injury in and of itself.

    I have a question. I am a bit in trouble making a decision. I had a knee injury back in January at combat sport training. It hurted a lot for a week but at that time I didn't go to a doctor. After the pain was less I started training more and more step by step and after 2 month I was back at even fighting trainings and ai participated at running competitions. The only problem I still have is sometimes I feel my knee when I stand up week or I cannot sit on my heels. That is why I did an MRI and consulted a doctor. It turned out that I fully torn my ACL in January. Now he suggested me to do surgery since I am only 20 and have 6 times per week training. But honestly I would like to rather avoid it and treat it only with therapy. I find it so strange that I can train with it for so long and I also don't have any kind of pain while training nowadays.
    Would it be possible to heal it without operation? Did anyone had a similar situation before?😅

    Fell from my bike...
    My MRI report shows,
    ACL appears avulsed from the proximal attachment and is seen at the intercondylar notch area
    Pls suggest me is this partial tear or complete tear of ACL

    I have a a partial ACL tear with meniscus 2 grade injury (no tear). Got it during MMA sparing (non professional)
    I feel almost no instability (~1 mm movement). I have a some degree of pain but it fells like the case of it is the menuscus.
    I'm still deciding what to do.

      @jasminesims2779 Yeah cause of injury is so important in these kinds of injuries. I was wrestling and had my leg kicked out from behind me in an awkward twisting motion, and that's how I tore mine. Take those mri results to an orthropedic that you trust. You don't want this surgery potentially done wrong, and have to do it again. Trust me

    I had ACL tear of grade 1(partial tear) with little meniscus injury 1 year ago. After that, I have completely stopped all my sports activity such as running and gone through ACL rehab protocol as doctor advised. Now I feel much stability of my knee at the time of walking and no pain is experienced. But I have to return to my running. Now I do quad exercises, hamstring exercises, squat and leg press. But I have not satisfied of my knee condition. Sometimes It gives me demotivation. I am strongly against surgery and I haven't done surgery. What should I do to make feel better?

My knee is stable as same as pre injury but I don't feel confident in doing any jerky twisting movement . I can do fast activities as I do previous but I have fear of pivot movement.
