You take the lid of and eat it first. The you either open up wide or use a pastry fork. Not to forget it's only eaten one day in February: Shrove Tuesday. That's it.

Warm milk is not common at all, never met someone eating semla with warm milk and I am 69 years old and I have eaten a few..
