    How to treat PIGMENTATION
    If I can identify the cause of pigmentation, chances are I can treat it. This can only be achieved with careful real time examination and a history. There are over 150 causes of pigmentation including PIH from acne, chicken pox, inflammatory skin conditions, melasma, sun spots, freckles, birthmarks and many others. This video illustrates my daily laser procedures as to the most common cause of skin pigmentation I see. I will do another video on pigmentation involving the neck and body in the future, as this video only discusses facial pigmentation.
    1. Melasma - I see 6 cases a day, sometimes up to 10. This is because Brisbane, Australia has a high amount of UV radiation. 95% of melasma can be successfully managed with lasers, creams, sun protection and VEGF-1 Modulation. Melasma is a medical condition and should be treated by specialists only. Incorrect treatment will make this worse. I prefer Revlite or Nano lasers, or the new Picosure,- Picoway - Enlighten laser. Laser parameters, coupled with tyrosinase inhibitors and SPF are critical to getting a good outcome. It is more about the end point settings and wavelengths than the brand of the laser.
    2. PIH or post inflammatory hyperpigmentation is super common. PIH due to acne can be managed along the same lines as melasma. For MILD and EARLY acne scars I use pico lasers in an off-label setting. PIH due to other causes may take up to 12 months or longer before resolving. The key to treating PIH is with gentle treatments, time and absolute sun protection- especially in Brisbane.
    3. Sun damage can present as pigmentation, as well as a combination of melasma. Very important to get the diagnosis correct as if the diagnosis is wrong, treatments such as IPL and Fraxel can worsen melasma or even trigger this condition. I use everything from Fractional lasers, Pico lasers, medium depth TCA peels, all the way to CO2 and erbium lasers to treat sun damage. Reduction of sun damage can decrease the chances of skin cancer.
    4. Freckles are easy to treat- IPL, peels, Fraxel can all help, as can light laser treatments such as dermal toning combined with Vitamin A and AHA creams. Easy to treat but because it is genetic, recurrence is the rule. Sun protection is essential- especially in Brisbane.
    5. BIRTHMARKS- again diagnosis is essential before I laser any birthmark. The most common BROWN birthmark I see is a CALM, other birthmarks such as Ota, Ito, dermal melanocytosis, Hori’s can also be treated. Birthmarks that do not respond well include congenital naevi, and speckled and lentiginous naevi, additionally Becker’s Naevi as well as mosaic pigmentation do not respond predictably. Please don’t comment as to how I can help you IF you have a birthmark- as my reply would be to see a dermatologist for a diagnosis. Thanks for your understanding as technology is not quite there for virtual laser - possibly in another 50 years time.

    Thank you for making this video!! I recently got a laser treatment done for redness/rosacea and I was left with severe burns, intense swelling and hyperpigmentation and hypopigmentation! That clinic is now trying to “fix” the damage they’ve done and have recommended the PICO laser. So it was great to get some info! Thank you, I wish you were here in Melbourne so I could see you instead :)

    New subscriber here from CT and I’d like to preface by thanking you. You’re a tower of knowledge and have truly taught me sooo much!
    I suffer from Melasma and I also have some sun damage. At first I thought your go to was the Q Switch Laser, but then read in the description, that you prefer the Revlite, Nano or Picosure. So I’m just a bit confused. I understand that certain lasers work for sun damage and certain work for melasma. However, do you have a go to laser that treats both? Or most importantly the Melasma?

    I was misdiagnosed with melasma a few years ago and found out yesterday (from a different dermatologist) that is is only sun damage/hyperpigmentation. The derm recommended a gentle laser to remove them but I’m a bit nervous just based on all the terrible reviews I read about lasers. What do you recommend? We don’t have pico sure in my area.     Thank you for taking your time to educate the public in detail, but only about the subject. Many channels have too much chit chat or bring guests in and have play time. I am here to learn. Your curriculum is on point! Forgive my cosmetics term. I am an esthetician who wants to be able to avoid pricey classes but needs to know my own scope and when to refer to a derm.     You are a brilliant doctor! I have pro-longed PIH from minor scratches, cuts, friction to my skin, affecting my entire body. My skin was burned/blotched during laser hair removal administered by a nurse who I am confident now wasn't remotely in your orbit. Too many non-physicians are allowed to perform procedures I believe require physician level expertise but are hard to find. Thanks for the very informative video!

    So happy & so lucky to have found and subscribed to your very informative channel! Question for you: It's my 2nd week today of using Zo Multi-Therapy Hydroquinone System, so am now on the peeling stage. But I've been getting allergic reactions of this treatment everytime I applied them on my neck and chest area. Do you know why? Also, I work during the week, so what would you recommend for me to apply on my face without showing the peeling-off skin tissues which is quite embarrassing? Lastly, the reason I'm using this Multi-Therapy is because I was asked by the Med Spa Clinic that this is a pre-treatment before they can perform a Fractional CO2 on my face. Thank you kindly, truly appreciated.

    I usually use Q switched for pigmentation but I get great results from microneedling with dermapen too... Visible after just one procedure... Thank you for sharing your knowledge and experience with us!

      Nadia Lachhab It's a type of chemical peel. I use a low strength one at home with good results (13% just 1 or 2 layers). Deep peels are usually done at doctors offices. xoxo

    Having the DIAGNOSIS before treatment is essential in improving this common condition. In Brisbane, Australia, the most common cause I see is MELASMA. Remember, if some one has not got a diagnosis do not have pigmentation treated. This is the key to success- a careful examination and a good history, this is the basis of good medicine.

      I don't know - WHAT IS THE DIAGNOSIS??? Like I said, Pico is not the only answer, a laser is only as good as the operator behind it. It is not about the laser, it is about the settings, passes, diagnosis, adjunctive therapy. Be guided by your dermatologist. All the best, Dr Davin Lim

      Dark Mist The Aerolase brand of lasers are. They may be the only ones out there that are safe. If you check out their website you’ll see lots of info on treating darker skin types.

      No idea. Bumps could be comedones, adnexal tumours, plane warts, seb. hyperplasia.... diagnosis before treatment.

    Melasma creeped up on my cheekbones when I was pregnant with my second child. My doctor prescribed tri-Luma which worked amazing. She advised me to use the product 2 months on and two months off and continue to repeat. The problem is within a week of being off, the melasma was back as if I never treated it. My dermatologist has not been helpful in advising me what to use during the off months of tri-luma. I’ve researched all of the potential skin lighteners and read studies (kojic acid, licorice, ellagic acid, hydroquinone). I wanted to try hydroquinone but have read that you need breaks from this and it’s already in tri-luma so if this is true, I should not use this. I’ve been using a cream with ellagic acid along with my tretinoin and it has done nothing. Any suggestions? I went to a cosmetic dermatologist and they advised against lasers.

      It's trial & error. I used rentin A , hydroquinone with rentin A mix, that my dr have the pharmacy mix for me. I used the mix for 4 months straight, nothing , took 2 months off. But used rentin A only for 2 months then back to the mix cream & it's almost gone. It's been 2 years but finally. Rentin A helps I think, it's like having a microdermabrasion & makes the mix cream work better. Hope this helps. I got melasma on ny cheeks from pregnancy also.

      Depends - blending in is the most important. Its easy to ablate a spot, but the colour change may not be natural. It really depends on the background skin. ...Davin Lim

      SunnyKip Denny my nanna did the same! Even in summer she'd wear light long sleeves & a sun hat whenever she went out the back to hang washing or potter around her garden... but then again, we are a very freckled family & it comes from her line so maybe her mother passed that down to her :}

    Very informative video Dr. Davin...I've yet to find any information addressing red blotches...that look like small broken capillaries...its seems like I get new one one my chest and arms every year!!! I don't know what causes them or how to treat them...i suspect UV damage..would you do a video about the cause and treament? I use Retin A daily and had great results for dark spots but not the red blotches. Thanks Doc😊

    Great information... I have monthly Tri-Beam laser treatments for the last five years. I also apply a compound cream of; Kojac, Tretonin, HQ 6% and hydro cort cream. I never leave the house without SPF (elta md or color science) under my makeup. I also wear a hat if outdoors. I live in Upland, CA and the summer heat is finally here. Last month I vacationed to Cancun and was so paranoid of being exposed to the sun with my Melasma resurfing. What a way to live... But ths works for me.

      I went for a consultation and I was recommended fraxel to remove my acne scars - being a fan of yours, I know that I am very likely going to get hyperpigmentation (Asian skin... I am Chinese). When I ask about this, dr. dermatologist told me that he will put hydroquinone on my skin before the fraxel and this will completely avoid the risk of hyperpigmentation... how true is this??? I was also told that he is going to use the Fraxel Duo which they can make adjustment to the wavelength to totally avoid hyperpigmentation.... I don't know how much of this is true....

    Dr Lim, what about glutathione iv to suppress melanin followed by Dermapen ? I don't have a skilled laser dermatologist in South Africa, Cape Town that can use lasers to treat pigmentation

      Hi Claire Marshel, I am also in SA but in Johannesburg. It seems most derms are recommending the glutathione IV and pills/creams along with TCA peels. I have been searching for months for someone who is a laser specialist but no one seems as knowledgeable as Dr Lim.
      I will be having a look at LCM skin and laser clinic in Pretoria next. Hope it goes well.

    Great info. A question about CALMs. You mention that the q switch laser is adequate but will the pico be as effective? Which one is preferred if you have the choice of either? I did see your other video on birthmarks.
