Went through surgery for this. I was 67 and this surgery is very hard to go through. Helped a lot,but still have problems. Think long and hard before going through this surgery. If your having a very bad time with this hernia, then it’s worth having surgery. If not that bad don’t put yourself through this.

I had a large hiatal hernia that led to reflux, erosive esophagitis and iron deficiency anemia. This went on for years with only partial relief from proton pump inhibitors. The fundoplication surgery (reduce the hernia and wrap the stomach around the lower esophageal sphincter) gave almost 100% relief from this condition. Surgery was laparoscopic and recovery was about three months. Completely satisfied. No more acid reducing medicine needed.

I developed an ulcer from the hernia, and ended up hospitalized twice, the last time, I needed 2 pints of blood, for anemia from bleeding. For years, I was handed a pamphlet about eating, and sleeping, modifications. I was told the hernia was very large. I had hiatus hernia repair and fundiplacation surgery, about 6 years ago. All of this information was never told to me. I still go for endoscopy, because I have Barrettes Esophagus, which needs to be monitored.
I remember my father being very sick and hospitalized for an ulcer. Now I wonder if, it could be a family thing? No one every mentioned, hiatal hernias can be inherited.

    Great explanation
Forty years ago, I did an activity which forced part of my stomach up through or past the diaphragm. It was a couple months later that I suddenly started having severe stomach pains. I was initially misdiagnosed by a doctor as having ulcers. It was a couple of years later before I realized that the doctor was wrong and I had gastric reflux disease, or GERD. Sometimes GERD happens naturally without a clear cause. Normally, the esophagus sphincter and the diaphragm sphincter muscles line up and reinforce each other. If part of the stomach is pushed or pulled up through the diaphragm, the sphincters and muscles don’t line up and stomach acid can more easily leak into the esophagus, causing GERD.
I figured out a simple cure: I would drink a couple glasses of cold water first thing in the morning and jump up and down for a couple minutes to jiggle the stomach back down to its normal position. It may take a repeats on a few successive days for the esophagus to adapt to the longer length for the stomachs in its new proper position. I would repeat the process anytime I got heartburn or acid taste in my mouth. I have been trouble free for over 35 years now. I later learns of a gastroenterologist in Michigan who recommends this process also; maybe others now also.
The body needs strong hydrochloric acid in the stomach to trigger the esophageal sphincter to close, to break down bulk foods generally, and to specifically separate the amino acids in proteins to be effectively absorbed in the intestines and used by the human body. Most or all GERD medicines block the production of or neutralize the hydrochloric acid and thus interferes with protein digestion and interferes with esophageal sphincter closing. Not good.

I developed a hiatal hernia when i had my fourth child. It presented as acid reflux and stomach and throat pain and tightness, then progressed over the years to sometimes a choking sensation, then actually literally choking on bread one day! It got so eating was stressful as dry foods like a sandwich, required washing down with lots of water. It was frightening!
A scope revealed pretty bad inflammation and scarring that caused the choking. The inflammation and scarring was diagnosed as progressing to precancerous. Progressing to barrettes syndrome, i believe he said? I was warned though it could develop into cancer of the esophagus if i didn’t deal with it. The surgeon showed me pictures he had taken in the base of my esophagus, i think to make me take it serious. I was put on a temporary bland diet with liquid supplements, advised not to lay down for hours after eating, to elevate the head of my bed, avoid greasy food etc. I was also prescribed proton pump inhibitors. They made me sick and digestion felt as though it was almost stopped sometimes. They were awful, I started losing weight quite suddenly, and it felt like a rock in my belly. I was told to keep trying different proton pump inhibitors, to find one that i could tolerate, which i dutifully purchased and tried, i remember the reflux symptoms would subside, but abdominal ache and heaviness returned after a couple weeks. The doctor said they were the only drug that would really work for acid reflux, long term. The surgeon said to combine the drugs with lifestyle changes, and return for another scope in i think it was two years, to make sure the scarring and inflammation wasn’t progressing towards cancer. I discontinued the proton pump inhibitors though, after a few months, and decided to seriously address the problem without them. I am so glad i followed my instincts.
I did a few things that really helped. First i elevated my bed at the head with blocks of wood. It did help somewhat. And i stacked pillows on bad nights. But it was clumsy and awkward and was affecting my sleep, constantly rearranging pillows. Eventually i actually bought one of those beds that you can elevate at the head and or foot. It wasn’t really that expensive in the scheme of things, I bought the cheapest one i could find at a local big box store. It so far has held up fine after several years. This was better as i could adjust it easily as most nights i needed less elevation, but if i was suffering from acid reflux, i could easy set it higher and that really helped, but then maybe lower it once i felt better after a couple hours. Some nights i can even have it flat now, but to be safe, i usually have it set slightly elevated, and it works great! Also, importantly, i try really hard not to eat later in the day, in fact my main meal is early, usually before 3 or 4 . If i am tired before bedtime, which happens, as i am on my feet a lot, and do physical work, i don’t lay on the couch to rest anymore, I can rest in a recliner though. I avoid fatty heavy meals, especially larger amounts or late in the day. That is the kiss of death for me, i finally got it through my head, no snacking in the evenings. Which is okay as my reward is virtually no acid reflux. I also start each day now with a large glass of water with fresh lemon juice and a splash of good kombacha. I have read reports coming from scientific studies done by reputable Universities, of the importance of gut health. I try hard to eat a large variety of vegetables lots of and unprocessed foods for the majority of my intake. There is a lot of good new information available now that i find help helpful and encouraging.
That said, i can still enjoy some treats, i noticed that i can still have french fries for lunch, or say on another day, perhaps a piece of birthday cake, early in the day, when there are lots of hours before i stop being active. Going for a walk after a meal helps digestion too. So that is nice, i still can enjoy life!
I did have gallstones that eventually required the gallbladder removal. That stopped most of the stabbing pain attacks that had been happening occasionally, but i still have digestive issues. I will get occasionally get acid reflux especially when i kind of forget to keep tally and eat too much fat at once. That is how it is when you no longer have a gallbladder. A little at a time is okay, so i can still have salad dressings, or a little butter on my veggies, and say a piece of steak, but i trim off most of the fat.
Originally though, when i quit the proton pump inhibitor experiment, that always made me feel like i had undigested food sitting like a rock in my stomach, i switched to all the above life style changes, and also quickly taking a drink of water if i have a sudden pain, and believe it or not, chewing bubble gum when i had an attack. Chewing Bubblicious bubblegum was recommended by my son who is a musician in a band, he said they chew gum before performances, and even between songs, because it creates saliva that makes them swallow, and that soothes their throats. It really helped me! Often that was all it took when i got symptoms. But i always i took Gavescon as soon as i felt any burning that i couldn’t soothe right away. That said i was able to cut back on that too as often just chewing gum for say ten minutes, soothed enough.
I went back for another scope a few years after the first one, and the surgeon was impressed and said the inflammation and scarring on my throat that caused the choking, had healed nicely and looked really good, he showed me pictures comparing the first scope and this one, he took biopsies again and they were now clear!
So for me, life style management was the easiest and probably the cheapest and least intrusive way to manage this. I occasional do the wrong things, like at Christmas with all the goodies around, LOL. But now it isn’t scary and i always realize what is happening, and correct and deal with any discomfort right away. My family doctor has always supported my lifestyle decisions, but stressed early on not to ignore any burning sensations, and to take a Gaviston pill right away if i can’t alleviate with my other tricks, to avoid further inflammation and scarring. So far, so good. This month i think i have only had to take a Gavescon once or twice. And i have been “testing” a few of the Christmas treats i bake for my family! I don’t really need bubblegum often either. My family doctor and the surgeon that did the scopes and gallbladder surgery, both had encouraged me to take the proton pump inhibitor drugs, were surprised with my progress without them. But to their credit, they both are happy with my progress and encourage me to continue with this plan of attack. I am lucky to have two doctors that took my problem seriously, but were willing to listen to my concerns and work with me together, to resolve my issues. Because although natural healing is my goal, i am not “against” modern science and surgery. For me a combination of both natural healing and surgery and modern medicine has been life changing. Good digestion equals fewer symptoms.,
Thank you for your educational videos!
I hope my experience with a hiatal hernia can help others with this condition, it took me awhile to figure out what works for me, and to experiment and adjust, but it was worth it, i feel much better now.

I used to do a lot of bending in my work, and one day I had this excruciating pain my chest. I was taken to hospital, where it was thought I'd had a heart attack, but after numerous tests it was concluded I had a hiatus hernia. This created a whole new way I thought about food. Nevertheless, I still take lansoprazole daily after all this time. If not, I'll get this weird 'lump' feeling in my throat and pain in my jaw and cheeks. Sometimes, it's just plain acid.

I have both acid reflux and a hiatus hernia, and both are treated with a proto-pump inhibitor, in my case lansoprazole, from which I never experience any side effects. As well as that, I regularly monitor my diet, reducing the intake of fats principally. This has been the situation for me for over 15 years now with continuing beneficial results. Robert, UK.

@@2eleven48 I'm sorry to inform you, but there are long term consequences of not doing anti-reflux & hiatal hernia mitigation.
Taking meds long term, you can still develop lung cilia damage, lung cysts, osteoporosis, long term progressive vitamin and mineral deficiencies (which can end up being serious), just to name a few. There are no free lunches taking medication, long term, for reflux problems. These meds are mostly used to prevent complications from developing in your esophagus and voicebox, they do not address the incoming problems down the line that I mentioned earlier.
Manometry, gastroscopy, barium/iodine swallow will get to the bottom of things.
A 270 degree toupet surgery with hiatal hernia repair and mitigation is a very worthwhile procedure.
You can get a resection and plication surgery instead, but these are reserved for people without any evidence of hiatal hernia. Chances are in time, you will develop a hiatal hernia and be having problems again, so better to get it all done in one go and then get on with your life.

That belly fat presses on my internal even when tying my shoes, so the repeated effect from working didn’t help.
There is a surgery to correct the stretched diaphragm sphincter but that specialist says that repairing it without first losing the weight, the problem will likely reoccur. (I have type 1)
Result: I’ve been on 1 tablet a day of Esomeprazole 40mg for 4 yrs now. Tried 4 different meds but this was the only one to offer consistent results.

Well, Derick, seems to me that you have to lose that weight first of all. Doing so may then not lead to having surgery, which is really quite a serious operation, as diet is fundamental to the improvement of the digestive tract. I had to explore what foods brought on reflux; usually things with fat in them, but also things like pasta (which I still eat, but sparingly). I've been on lansoprazole for quite some years now, which certainly helps. I wish you luck. Robert, UK.

Funny though, I don't experience any GERD at all. And I have to eat very small amounts throughout the day, and can only take about 3 sips of water at a time. I live with back pain every day, and it's painful to take a deep breath. The only pain med I take is Voltaren. I wish you all the best with your health.

To stop Gerd with a hiatal hernia the most important thing is to NEVER eat cold foods and drink cold beverages. No ice! The stomach produces more acid and heats up more when you eat or drink cold things. Good should be hot and mushy. Do this and drink hot tea after each meal making your stomach do less work to break down food.

It may be two months or more since I've had any problems. I can't let myself get full. I, and so many other people were brought up to eat everything on my plate. I can't always do that anymore. once I feel I might be getting full, I have to stop eating, or I will pay dearly and suffer for many hours. That pre-biotic/pro-biotic has worked wonders for me.

Before the surgery, I was doing seven to ten breathing treatments a day with my nebulizer. I hardly ever have to do more than three a day, and somedays only two. I was having to use two vials of albuterol and atrovent with every breathing treatment. Since the surgery, I've hardly ever had to use more than one vial a breathing treatment. However, there have been some downsides, too. I basically have no appetite. I hardly ever get hungry. And I can eat much at a time. But I've lost all the weight I've wanted to lose. I have a pretty healthy weight at 135 pounds at a height of four feet nine and a half inches tall. I'm kind of short. I can still eat whatever I want. I have no diet restrictions. I don't regret having the surgery at all. And I can still vomit. about half the people who have the surgery, can't vomit after the surgery. I can.

My dr. told me to wear loose clothes, nothing tight around my waist and for several months I couldn’t wear a bra. I got bra extensions to keep it really loose. In January one day I went to take my usual walk and couldn’t breath. Luckily there are benches. Went from one to another. A bunch of tests later to find why I had a breathing problem and finally sent me to a bowel dr. , I was constantly burping, diagnosed with Gerd, that was in June. The surgeon I have sent me for another test they aborted because the sensor they wanted to put in the stomach wouldn’t go in. From more CTs you could see the esophagus looks like a multiple switch back mountain road. She was going originally expecting give me a whole hug but because of the shape the esophagus is in she will have to do a half hug, after I loose another 20 lbs. I have had this hiatal hernia as long as I can remember but was a systematical until January and it turned everything upside down. I started the Gerd diet. A month later I had lost 30 lbs, but the breathing was still an issue for another month or so, now it’s good and I can walk like I used to. But the Gerd still kicks butt if I eat anything wrong.

@@lilliehalumi8770 , I really don’t understand it exactly. The hernia will be wrapped around the esophagus which will go through the middle of a hug into the stomach ( my stomach is sitting on top of the hernia) . The hernia would be wrapped totally around the esophagus. In my case, I have had this hernia since I was in late teens (74 yrs old now) as the hernia got bigger my stomach ended up on top of hernia and the esophagus started kinking so it looks like a really winding road, so apparently I can’t have a whole hug as the dr. said then I wouldn’t be able to swallow anything. Exactly how a half a hug is done I’m not exactly sure, I just know I have to loose another 10 lbs. to be able to have the surgery and they will put my stomach back where it belongs.

So if I don’t go in there right away and throw it up I can’t breathe. What kind of hernia is this. I was supposed to go in and and get it fixed but I happen to have a psychopathic nurse Who would ignore me when I was in great pain. I was afraid to get the surgery and be at her mercy so I still have it. I just have to be really careful about chewing my food up really good or else it gets stuck and I am really miserable. Hiatal hernias affect gallbladder function, digestion, stress and sleep not to mention it can damage the vagus nerve which can cause chronic diarrhea, SIBO, erectile dysfunction and dysphasia. PPI’s do more harm than good! There are water exercises that when utilized properly and consistently can correct a hiatal hernia by pulling the stomach back down out of the diaphragm. DO NOT USE PPI’s! H2 agonists are a much safer option for the SHORT term until the hernia is corrected. It’s AMAZING to me how little American doctors know about the actual CAUSE of most digestive disorders. Now as to what can CAUSE a hiatal hernia can range from poor posture putting pressure on various nerves in the upper spine, poor eating habits, eating while stressed, food intolerances, masturbation soon after eating a meal (that can weaken the diaphragm) low magnesium levels in the body etc. digestive enzymes are a MUST.

This sounds good in theory, but for large hiatal hernias, fundoplication is the solution. I took omeprazole for years. Now off of PPIs and can even lie down after eating. The surgery is now routinely done laparoscopically. No lifting for three months, but then back to normal.

Very good explanation. I developed a hietal hernia when my daughter was born. 26 hours in dry labor with a 3 week late baby tears up your inwards. Three years later I was in major surgery. My Dr found my diaphram tore during labor and my stomach went up into my chest cavity. My splean and other organs were also included in addition to bowl and vigina damage. The hernia was strangulated by te healed diaphragm wound. My spleen ruptured when he tried to pull everything back out of my chest and a simple surgery nearly killed me and doctor. 4 1/2 hours later it was over. Now that old surgery is failing and my new doctor turned white when I described the severe mid chest pain I am having on a daily basis. I am scared to death. At 75 I don't think I can do this again.. Are there any alternatives to major surgery now??? LGB

Hi, sorry I am not native speaking English, do you by "laryngeal spasm" mean the actual breathing tube is causing spasm? I am asking, because I think I have a Hiatus, and really felt when it went up, due to me causing a pressure while laying down(felt sort of like when you blow a baloon and it's a bit hard, but suddenly it expands..) after that I had this strange lump in my throat and then suddenly I got spasm in my breathing tube, as if having asthma, which I do not have, very frightening. So, that's why I was wondering if having spasm in breathing tube is what you meant and it could be due to a Hiatus.

Very nicely presented and informative video. Thank you. However, I'm a tad confused in that among the remedies suggested is the avoidance of the types of food and drink which tend to relax the oesophageal sphincter and elevate stomach acidity (i.e. lower p.h).
But ... numerous doctors on YT say that GERD is the result of too LITTLE stomach acid, or, stomach acid which is not SUFFICIENTLY acidIc (i.e the PH is too high). Rather than the PH of 1 to 3 required to digest food, the PH may be over 3 (i.e. too alkali).
They suggest the 'Burp Test', which goes as follows:-
    - on an empty stomach mix one quarter teaspoon of sodium bicarb (Arm & Hammer, some such) in a glass of water.
    - Drink.
    - time how long it takes for the first 'burp'.
    - if less than 3 minutes the stomach PH is between 1 and 3. ✅
    - if more than 3 minutes chances are the PH is too high or there's insufficient acid. ❌
Obviously this is a very rough and ready test. Quite why the stomach acid should should be too alkali is not explored.
    There are two things:
    1. A PH of 1 to 3 is needed to fully digest food. If the PH is too high (i.e. insufficiently acidic) there is incomplete digestion leading to gas, belching, heartburn and other symptoms.
    2. Apparently the oesophageal sphincter needs a low PH of 1 to 3 to close fully, preventing reflux. If the stomach acid is continually too alkaline the sphincter is insufficiently stimulated, fails to fully close, and reflux occurs.
So, IF THIS IS CORRECT (IF), the paradox here is that heartburn is caused by too little acidity, rather than too much.
Ergo - taking drugs (Tums, sodium bicarb and other meds) to REDUCE acidity is counter-productive. While they reduce symptoms by helping neutralising acidity in the oesophageal tract, in the long run they make the condition worse by reducing the acidity of the stomach..
Many of these online experts recommend Apple Cider Vinegar, taken in water to INCREASE stomach acidity and thus increase the efficiency of the oesophageal sphincter, thus reducing or eliminating GERD.
I am my own worst enemy at times. I'm 71 and have been performing a series of stretch-band exercises - using the isometric method of holding the stretch. As part of my 'explorations' into better health 😂 I devised a way of placing the band around my rib cage and holding it there, under tension (do you see where I'm going with this? 😂😂). After some months of this my body grew much stronger, but apparently so did the discomfort I experienced with almost continuous indigestion (something I've never suffered from before).
My conclusion is that in doing this static band exercise I've put pressure on my diaphragm and the upper part of the stomach where it passes into my oesophageal tract. I'm a great believer that if you're going to do something you should do it THOROUGHLY. 😂😂
Currently I'm not doing my self-invented exercise - or the many sit-ups I used to do because potentially that might have the same effect by compressing the abdomen against the rib cage. As recommended by the various YT doctors I'm taking ACV in water a few times a day. My symptoms have abated a lot, but I'll continue to monitor what happens. It may be that the YT 'experts' have got it wrong, but I'll make a judgement on that if my symptoms continue to abate by taking the ACV and leaving the stretch band exercise and sit-ups alone. Thanks. 👍🇬🇧

And are saying it's hiatal hernia now. Can they mistaken that? They said when I turn to my left something is pulling my intestine inside itself, which is why they said intussiscention. Which is also why they did the small bowel follow through. I still feel I have no answers other then small bowel of now saying hernia

All PPIs are evil. They lower stomach acidity, which leads to indigestion, mineral and nutrient defficiency, bacteria overgrowth (because acid sterilize food) and other problems. They can soften your symptoms in beginning but it will be worse on the long run.

I’ve been on Pantoprazole since 2012. In addition I stay away from high acidity foods like Pineapple, tomato sauces. Thirdly I don’t shovel in I’m stomach and I rarely will get dish up seconds.

It could be a sliding one, then it slides up and down. When you have had one, it almost never goes away, think of it like a damage on the inner part of a wheel on a bicycle, if pressure is up (air pumped in), then the damage part will bulge out.
