At the time and compared to the previous entries MGS3 was nasty and kinda mean spirited, especially with Volgin's character. Made you really wanna take the villains down. Here's hoping Phantom Pain delivers a similar set of bosses(no pun intended) that really take you in the game.
The Boss isn't part of their military hierarchy so it's whatever. Ocelot however is part of it and merely had the rank of major so calling Volgin by his plain name is utter disrespect.

There’s a belief that a story is only as good as the villain. And Volgin, as far as villains went, was pretty good
