When I was prescribed BiPAP my A:H index was 46 and I was desaturating down to 72% with arrhythmias.. When I went to get help from a home supplier they told me no machines were available and it would be six months at least before I could get one. Several suppliers were rude an in the end they left me twisting in the breeze. I bought an air curve 10 off the internet but could not get in the menu to set the pressures. This video showed me how and probably saved my life.

I’ve been on BiPAP since January 2006 and I believe it saved my life. I’ve been adjusting my pressure up and down to keep my AHI as low as possible for years, with a Respironics Dreamstation, and a System One before that. So I’m new to ResMed Vcurve Auto 10. Thanks for the info, I find it unusual that my IPAP and EPAP are so close together based on what I’ve seen here, and what my friends have for pressures. Mine are 10 and 12. That’s what I’ve been prescribed. I’ve adjusted those over time.
I’ve lost 70 pounds since April 2023 and 100 total since Sept 2021, so I really need to change my pressure. I think it’s too high for a much skinnier me.

Same! Mine were prescribed very high but only a difference of 2! I am adjusting both down in 10% increments due to morning ribcage soreness the high pressure is causing. So far so good.

I started my bipap therapy a couple years ago. I have used it on and off. I started pulling my mask off during the night so I quit using it all together. Cardiac Dr said it will worsen my afib if I don’t use it, so I went back to the lung Dr. That was a year ago. He said he would call and have my settings changed. After 6 phone calls to the Dr’s office and several to Lincare it’s still not adjusted. It will be another month or more to get back into the Drs office to get the settings changed. I am going to use this video to try to set it myself. Thank you for the help.

I'm so glad I found your channel and these great videos (even with the corny jokes! ;-)) I've fought with sleep apnea for over 15 years, through several incompetent DMEs and doctors, and, of course, the recall. When I received my AirCurve 10 six weeks ago I was so hopeful that it might finally solve my issues. Instead, I had horrible numbers with dozens of rapidly repeating CA hits. My current DME and doctor were unable, or unwilling to read the charts. So with your help I figured them out and with this video have been learning to adjust the parameters to finally get real sleep the past few nights. Thank you. Thank you. Thank you. I'm checking your other webpage to look into having a consultation in the near future.

Just the other day I found out the highest pressure the machine was giving me at night is 10. I might as well not be using anything at all. I set up the bipap with pressure support of 5 with min and max 6 & 12. My air app gave me 3.2 events not sure what i should do next. Would you please do a review on cpap pillows, not the ones that go into your snout, but the ones you rest your head on? BUCKWHEAT Pillows !! My wife and I switched about 15 years ago to the buckwheat travel pillows that curve around your neck like in car or on plane but we seldom use them that way. In bed they are very adjustable and easy to fit body/shoulder and adjust height AND most importantly to fit my full face mask when side sleeping !!! . . . After so long using them we cannot go back to the regular pillows and are baffled that people use them at all the way ones neck is bent so dramatically YUCK ! . . . Our favorite is the BUCKY pillow. About 20 dollars on amazon. Different colors available and cover is removable/washable. Good Luck to all of us !

My RT refused to tell me how to get into the machine extra settings. I figured it out anyway. I just had another sleep study because I was having over 40 episodes an hour using my new ResMed Air Curve 10 VAuto machine. I used the smartphone app to tell me the data. I have central sleep apnea so they did another sleep study to figure out my settings. Thanks for all the information about the pressure support etc. That is very useful. I was wondering what the difference in V auto was from the S therapy settings and CPAP.

Thanks for the explaining; one question is the Pressue Support(PS) level the same as chosen even during Ramp time? If so, if PS is 5 and starting during ramp(assuming starting EPAP=4), IPAP= 9, is high IPAP a concern if you want to maximize the comfort and tolerability of using the machine.

I find myself consciously breathing a lot even when awake and when I stop thinking about my breathing and my breath softens there’s a hell of a lot of resistance on the Exhale most in my nasal area So I’m trying to find the right pressure on the exhale I don’t think the inhale this is much of a problem thanks

Recently I switched to BiPAP, and found lots of the CA (clear airway) events (28). I don’t know the number when I was using CPAP, but the overall AHI increased from around 3 to around 5. My current setup is 4-13 with PS 3, I wonder if that 3 causes more central sleep apnea events. Is it worth adjusting the number a little higher? Or should I get back to CPAP?

Question, but first Thank you for doing this you might have just saved my life. I will try and be as brief as possible. I have this bipap but my sleep therapy session was with the Phillips dream and we just determined my iPap and epap which was 17 over 11. And it worked great for me my lungs are bad born with asthma and COPD in 2000. I am over 300 pounds. I was on two different recalled Phillips units for over two years both had factory recalls when given to me. Now my lungs are less then 38% i had to get this respironics talking medicaid into paying for a third one as Phillips never replaced the recalled ones. Its hard for me to leave the house so my wife picked this model up and they just gave me the same pre set numbers. But my condition had worsened since I first went on a bipap over two years ago. So my problem I'm having is , one I feel like I am not getting enough pressure and two that my breath when breathing in it isn't picking up and shoving the air in fast enough and it's not keeping it going in long enough. The meter I watch climbs and maxes out and I am still breathing in. So I changed my iPap after starting at 19 came down to 18 that felt right but kept my epap at 11. But I am having the same problem. Should I go back to original numbers 17 over 11 and raise my trigger time to high ? Please help me as I am terrified of falling asleep as I am waking up with bipap after ten mins having 7 times stopped breathing. I'm exhausted and don't trust anyone in this industry right now after Phillips and the medical supply company giving me a federally recalled unit two different times a year after recall and let me use them for two years. Thank you sorry it's long I'm tired.

thank you for making this bipap experiment possible and even almost exciting again lol.
clarifying Q- at 16:36 you mention that you like to have a static pressure, or a slight range (but not a large range). i'm guessing you're still referring specifically in the 'VAuto' setting (because you can't set a 'PS' in the 'S' mode)? ie does that still apply in 'S' mode? are you saying you like to keep the IPAP and EPAP somewhat close together in 'S' mode? in which case the suggested setting of 'IPAP' 25/ 'EPAP' 4 seems very high. or does the range of pressure automatically have open possibilities in 'S' mode so long as it's within the 'IPAP' and 'EPAP'? at the moment i'm trying 'S' mode and leaving it at that suggested setting since i don't know what is best for me yet. i hope i'm making sense. cheers!

One important difference between s-mode and vauto is that s-mode doesn’t records flow limitation if you are using Oscar to inspect your data.

I have EPAP set to 4.0 and IPAP to 12.0 - Is that too big of a variance. It seems to work. I can average 4 - 5 hours on the machine with really good nasal pillow seal and less than 4 apnea events/hr. I was diagnosed with 50 events/hr

If I'm understanding this correctly, my prescribed EPAP of 15 is what is controlling my obstructive apneas? My IPAP of 19 is what is handling my centrals and hypopneas? I'm on bilevel because I couldn't tolerate CPAP during my sleep study 5 years ago. I struggled exhaling against the pressure and didn't fully fall asleep until the tech switched me to bilevel. I recently lowered my pressures by 1 and I've noticed my high leak rate has drastically reduced, my centrals have reduced, my osa hasn't worsened, and I've never had many hypopneas. I'm thinking about reducing my pressure relief to see if I can get it to where, in the future, I can get a CPAP machine instead of a bilevel, but I thought the IPAP would be what handles my osa, not my EPAP, that's very interesting. I'm excited to try reducing my IPAP, it would be AMAZING to have my pressure be 14!

My baseline during the split study was 71 AHI at the end of the therapy phase it was lower but I had Central apneas that were not present during the baseline phase. • @DDDarray
      @DDDarray 2 ปีที่แล้ว

@@cebruthius mild OSA, but I'm tracking many nights and that sleep study was by far not the worst. I've been able to sleep a few hours cpap some nights, others it sets me into cycles of breathing too fast followed by pauses, probably due to the EPR setting causing excess CO2 loss. After many months trying that torture I no longer can sleep with it. I'll have to look for the clinician manuals for the Philips machines. They should stop prescribing CPAP altogether, because of this risk, and because the machines are all the same hardware inside anyways. Start with a good auto-bipap and then no need to upgrade, better acceptance, better results, better word of mouth for the whole industry.

Hey, thanks for the great video! It is really helpful.
I'm confused about rise time settings. Skipping to 5:30 You said that a rise time of 900 ms is really fast compared to a rise time of 150 ms. I must have things turned around somehow, because 900 ms is longer than 150 ms. Wouldn't that mean that a rise time of 900 ms is slower than a rise time of 150 ms?
I'm on BiPAP now but started on CPAP more than a dozen years ago. It didn't take me very long to stop using the ramp. I totally agree with your observations on that.
