Well assuming the 6k years was a slip saying that it's our earth and not specifying when leaves continental drift on the table so trying to overlay it onto the current earth is short sighted
I think middle earth is pangea the super continent before the split that made the 7 continents because its called "Middle Earth" which might mean that its in the middle of the water that surrounds the earth

Mountain ranges grew or shrank by a few feet at most. Some river's courses might have changed, but we didnt gain or lose any major continents or bodies of water. Tolkien was only inspired. He hated using allegory. He created an Anglo- Celtic mythology.
