Hi Jack. It's one of many possible causes. Sometimes that hip thrusting towards the ball can drop the club in behind and have the heel moving in to out towards the ball. But the same can be true from an over the top move also.
As you suggest, it's really important not to throw the hips forward - this restricts the space to swing the club creating all sorts of problems. Better swingers keep their butt back almost as if it were pressed against a wall. I don't know about you but my chest and shoulders are pretty much attached but I think I know what you mean!

Yes, that's a good one. You'll be less likely to see the early extension characteristic during the swings of most elite players.
Early extension, if only I could cure that. Think I need an "electric steel bowl" just above the top of my head to zap me into submission !!!
