    A young man had fractional CO2 laser treatment on his cheeks and forehead, presumably to treat acne scarring. He says that there are redness and hyperpigmentation issues still visible 4 months after the treatment. He asks what he should do, and adds he is a smoker.

    Dear doc, my lower eyelids have been resurfaced by fractional co2 Laser, 18 weeks ago. I am wondering, why there is still redness by 4,5 months post laser - but far worse: it seems like I got some scarring on my lids! Indentations making my lids looking much older and pretty bad. My surgeon is gaslighting me. Any suggestions? I am thinking about trying tretionin after 6months to see if those indentations can fill in…

    My derma suggested Combination of laser and chemical peel for my chiken pox scars. I have indian tan olive skin tone and i was not confident in doing CO2 fractional laser . When I asked about the possibility of Hyperpigmentation after co2 , they agree to it , but the responsibility lies on the patient , that we have to use sunscreen and careful about post treatment care .
    So did the ND ,YAG laser first session .

      I generally avoid aggressive lasers for darker skin types, but some lasers have more customizable settings to possibly treat darker skin. The responsibility for avoiding potential hyperpigmentation, and scarring in darker skin is with the skill, and experience of the doctor using lasers and other thermal treatments on people with darker skin, because those ultimately decide a successful outcome, or complications.     Thanks for this video, Dr Prasad. I have had two laser work done with a Korean doctor. After finding out about hyperpigmentation and fat loss on my own, I decided to stop laser for the time being. I am Asian and do not have lots facial fats. I am glad to learn all these because doctors don't tell us things and we do not know how to ask the right questions until we are educated. And such a good idea about using Retinol products after laser work.

    Hi Doctor!
    I have had CO2 laser treatment due to acne scars on my face.. It's been almost 1 year .. I only had 1 session of Co2 laser..but the dermatologist who applied the laser on my face said that i had to do 3 more session to see the remarkable result..
    1 year ago i did only one session but the redness of laser treatment still noticeable and my skin become dull and i can see the discoloration of my face
    What should i do now??
    Please reply 😢.. I am 21 years old..

      Acne scar treatment generally takes more than one treatment session, and more than one mode of treatment. I usually suggest staying with the same doctor throughout the treatment plan. Since I have not examined your skin like your doctor has, I defer to their judgment for a treatment plan for you.     How soon can I start using hydroquinone after fractional laser?

      Hydroquinone should only be used by the advice of your doctor, and should be used sparingly. As I'm unaware of your skin condition, and what issue you had treated with a fractional laser, I suggest you ask the doctor who examined your skin, and performed your laser treatment.       I hope you understand I can't say for sure as I don't recommend CO2 laser treatments for darker ethnic skin types, and I have not examined your skin, or know details about how your treatment was performed. I do need to perform a physical examination to give you an answer. I suggest you follow up with the doctor who performed your treatment about how long they expect the redness to last.     I have acne scars I am Indian with olive skin tone. My doctor suggested co2 fractional laser I am just scared reading the comments that I will get pigmentation too after the treatment. The doctor is quite confident that nothing will go wrong.

If your doctor has experience with treating darker skinned patients, then you should trust your doctor's experience.       Pimples are usually due to oil and blocked glands, which are not associated with CO2 laser or PRP. Itching is a possible indication of healing in this area. I hope this clarifies things for you.

    Hi Dr. I’m a 26 yr old woman and I’ve had co2 fractional laser (writes 36J- 2 passes on my records) 3 weeks ago . I think the overall result is progressive but I see very light grid like prints on my forehead and side of the cheeks. Looks quite reddish from close. I don’t drink or smoke and I’m worried pls help.!

As I have not seen the extent of these effects, or know the fine details of your treatment, you need to follow up with your performing doctor about where you are in the healing stage of your treatment.     Please i need your help, 7 days ago I put lutronic CO2 laser, was very high power and several times in the same place, that was on the nose to treat scars that I have in the area of The nose, the scabs have fallen and now I have many more holes than before, and I have certain white blisters, my skin looks worse, much more battered than before, please tell me what will happens and if I have the risk to have more scars than before?     I would never do this again. I have problems with my skin now that occurred slowly over many months after just one treatment, including patces of hypo and hyper pigmentation - and mild scarring and skin irritation on one side of my face. I have a friend who is a plastic surgeon who does these and has them done on herself. What I have observed is no wrinkles and smooth looking skin but oddly discolored skin and cystic acne type breakouts often. It doesn't look natural or healthy at all. I think in some cases, this treatment causes overly sensitive burn victim type skin. And to hear about experiences of fat loss, that another plastic surgeon online has explained how this happens, is another risk I am not willing to take. There are other things that are less harsh and risky.

      Sorry to hear about your problems. It is important to realize that lasers are tools, and only as good as the experience of the doctor using them. You are right that if used improperly, they are basically causing burns to the skin and deeper layers. With laser treatment, there is commonly a high-risk, high-reward scenario. I do explain some alternatives to reducing laser exposure but still getting a collagen response, such as using PRP injections. I hope this helps you.

      I do mention at the end of the video that it is possible for pigmentation, and redness to fade over time, but it also depends on the severity.       Sunscreen is for prevention of melanin production, not for the treatment of pigmentation. You don't indicate whether your skin became darker from laser treatment, for from sun exposure. I hope this helps you.

    Hi, I have had co2 for acne scars and I am Pakistani brown skin. My skin colour went really dark which I was then prescribed hydroquinone now my cheek patch is completely white, I have stopped using this cream now can I ask will my skin go back to normal colour as it’s very obvious there has been a procedure done - thank you very much for taking time to answer

      I advise you follow up with your doctor as you may need more than one treatment to complete the treatment plan. Scars do require staying with one doctor and following the whole treatment plan.     Hii dr,i am an indian girl.i had five sessions of fractional co2 laser treatment with prp last year..i am left with grid marks and very sensitive skin which is irritated all the time.but now i am noticing that my cheeks are also getting flatter.now that dermatologist is saying that it is impossible for fractional co2 laser treatment to cause fat loss as it cannot penetrate deep to fat.my skin used to be oily but it is dry and dehydrated dehydrated after that treatment no matter how much i moisturize it.i has been 14 months but it is not improving

I would like to ask you something I have a pigmentation(dark colour ) on my face exactly on the beard area due to the usage of beard colour product. It's been 2 years I am using the same product but I noticed 2 weeks ago.I have consulted a dermatologist and he prescribed a lotion (Desonide Lotion) & vitamin c serum for two weeks.

      Laser is not always the best treatment for acne scars as the right treatment depends on type of acne scar, type of skin, skin color, and other individual factors. I could not recommend specific treatments without a physical exam, but these include subcision, laser resurfacing, chemical peels, PRP injections, cosmetic fillers, and other treatments. The best thing to do is have a treatment plan outlined by one doctor, and follow it patiently as acne scar improvement does take more than one session and more than one type of treatment to improve.     I had fractional co2 laser about 3 years ago . I am still experiencing noticeable redness and I’ve tried absolutely everything. Some dermatologist say I could have rosacea but that doesn’t run in my family nor do I experience all the symptoms. Is my face still healing or is 3 years out considered prolonged erythema and should/Can I do anything about this?

    I had my second session few days ago, high intensity and now I have pigmentation on my cheeks that also looks like blisters. Should I be worried or do these go away eventually. I am from Nepal so I have medium skin complexion.

      I don't know the details of your treatment or skin, so I really would need to perform a physical exam on you. Laser treatments have different energy settings, and exposure time by number of passes. Also, you don't indicate the time between sessions because healing is important, and exposure should be limited. Also, I need to see your skin type. Have you consulted your doctor or practitioner? They know your skin better than I would because I've never examined it.

      We do use lasers for dark spots, but lasers can cause other issues like hyperpigmentation of hypopigmentation on darker skin. I would have to perform a physical exam to determine your skin type and color, and also determine the appropriate setting for the laser, or see if another treatment may be better for your case and skin.     Hi, Doctor. I am Asian, and I have done Fractional CO2 Laser 3 weeks ago. I still have some mild redness on my upper cheeks near my under eyes, especially on my right side because it was were I felt the stinging sensation the most for almost the whole week of my healing time.
    I developed 2 tiny brown dots on both cheeks. I am anxious that my redness will turn to brownish pigmentation. I went back to my doctor, and she prescribed me melanin blocker cream, Hydrocortisone cream and Hydroquinone cream. Do you think this is just something temporary, and is normal for this redness to take time to go away?

      Depending on your individual skin conditions, I recommend you space your treatments 4-6 weeks apart to allow for healing and collagen formation, and not make them too aggressive. Alternating between microneedling and laser is also a good idea, and you can consider PRP as well for collagen induction and to improve skin quality. I actually recently did a video about this:

    My name is Talha
    I have done my co2 laser treatment just 4 weeks ago it was my 4th session all of the sessions was perfect. But on 4th session i dont know what happens might be machine was not working properly and this session was done by helper not by my doct. So i think she choose very high frequency which i also felt. But she said it will be fine.
    But now peeling of outer later of skin has been done. But the place where high intensity laser got hitted it leaves a black spot. Now i dont know what to do. And when i complained to my doct abt this he just said thats not problem it would be fine in some days.but in my first session it heal up in 1 week now 4 weeks have been passed but this spot is still there.
    Kindly help me how to recover this black spot.
    I have also done 1 session of PRP for this but spot is still there.

      If i tells about this i can say As your skin burn and after burning The outer burned layers peels off and fresh skin comes but with a patch of spot.so same is the case with me.
      Outer layer has been peeld off but fresh skin is containing black patch spot. Suggest me any cream or any treatment whixh will help to lighten this patch spot.

    Hallo doctor,thank you so much for answering question- I had pigmentation on my chin for first time of my life and I am 33yrs old and Asian .I used sunscreen 50 and always protect myself from sun.I don’t smoke and take healthy diet. I did laser on my pigmentation hoping it wil go away but I had opposite reaction-it first appear burn ofcourse,than skin start peeling off and I didn’t touch anything. It appear red and now it’s kind of 80% more darker Dan before.I start using retinol now.What should I do more?? I am getting so self conscious now and it is putting my self confidence down.

    I too had CO2 laser and am of Italian decent. I do not smoke and take great care of myself. My twin sister and I had the Active FX C0 2 done on the same day one month ago and have grid lines all over our faces. They are still very apparent, even with make up....Our skin turned dark.....not red. We have had NO SUN! The doc that performed it, shrugs it off and says it could take months to go away.....Help! I have SO much on the calendar for this summer. I am sick at heart...money NOT well spent...scared! I really look freaky!

I often do not perform fractional CO2 on Mediterranean or olive skin tones due to the problem of hyperpigmentation, which sounds like what happened to you. I would need to examine your skin to see if anything can be done to improve it. For now, you can wait it out, and make sure you are applying sunscreen regularly to prevent the problem from getting worse. In the future, you can consider fractional Erbium or ND:Yag laser as they can work on some darker skin tones. I hope this helps you.

      Hi Fiza Naeeem,
      I'm not sure what you had treated with CO2 laser, but it's best to follow up first with your doctor as they had the benefit of physically examining your skin and situation, as well as details about your procedure. What did your doctor say about your current condition? It may take more than one treatment for best results. I hope this helps you.

    I had co2 laser on my face and I’m dark tone Mexican American. My face was treated with co2 and my face has gotten darker tone within a week. Can I send you pictures of my face . I need help.

      Sorry to hear about your problem. I would have to examine your skin tone before recommending treatment, which I can't do with just pictures alone. I suggest you follow up with the doctor who performed your treatment first as they can examine your skin for possible causes, and recommend treatment based on the examination of your skin. If you would like a proper second opinion from me, I would have to physically examine your skin. I hope this clarifies things for you.

      @@AmiyaPrasadMD Thank you for your time and response. I have an appointment with a dermatologist on July 13 and I had the co2 done on June 1st of this month. During the time I’m waiting for my appointment do you know if it can still be treated ? I’m going to a dermatologist that didn’t do the co2 . Where are you located?

      @@AmiyaPrasadMD I also see the black tone shedding of my skin slowly hoping it doesn’t come back . I been out the sun for the past 2 weeks and I return to work on Monday. Ima cover my face and use sunscreen cause I drive a lot. I also want to thank you a lot for this video . I saw this video before I did the procedure I should of listened and I was still hesitant. I did the procedure in TJ Mexico . At this point I don’t care as long as it goes away by time . Thank you for your time once again .

    I'm from India .5 months ago I had CO2 fractional laser to remove my cholestrol bumps but that specific area turns on hypopigmentation .
    Now what should I do ?
    Will it spread?

      Sorry to hear about your problem. Hypopigmentation should not spread, and should be limited to the direct treatment area. I would have to examine your skin before making specific recommendations for you. I generally avoid using CO2 lasers on darker skin types like mine. I hope this helps you.

      I can't recommend laser treatment without examining your skin, especially if you have dark skin where laser treatment can make hyperpigmentation worse. There is no best treatment that applies to everyone, as skin color and other factors differ with individuals. I hope this clarifies things for you.

    I would really like to know your opinion regarding the effects of laser CO2 on skin thickness. I am 20 years old, with thick and light skin, and I have some acne scars. I am interested in getting an ablative CO2 laser, as I have tried 2 non-ablative fraxel and also 1 discovery pico, and they didn't do anything in my case. In some articles, I have read that CO2 laser can cause thinning of the skin and in some other articles I have read that CO2 is good for skin thickness and quality. So, the main question is if, besides hyperpigmentation, can an ablative CO2 laser also "harm" the skin thickness that is actually beneficial for the skin?

      If you didn't get the results you wanted from previous treatment for acne scars, I suggest you stay with same doctor, instead of changing doctors and devices if you don't get the results you want. Acne scar treatment usually takes several treatment sessions, and several different modes of treatment before you see improvement. If you keep changing doctors, and trying different devices with another doctor, then the likelihood of damaging and thinning your skin increases. It's not the device itself that causes the skin to thin, but the experience and expertise of the doctor using it, as well as not being informed about details of your previous treatments.       I don't recommend any aggressive treatments until at least a year after you stop taking isotretinoin as the drug does leave your skin very fragile. I hope this clarifies things for you.
