Letting him live turns out to be a much harsher punishment. He ultimately goes blind and lives out the remainder of his life in a gutter with kids throwing rocks at him and everyone else spitting on him as they walk by. He loses everything and will eventually die in complete squalor and misery.
By the time Geralt got to that point I was pretty emotionally invested and infuriated and disgustipated by Whoreson's vicious perversions. It reminded me the GoT scenes with Joffery when Uncle Tyrion bought him Littlefinger's ladies. Took me a brew and a bit to unwind after I choosing Jr.'s fate. "Cause she's like a daughter to me and that's why I just can't let this go." Sorry to get heavy but Geralt's "emotion" in the scene and what happens later makes it compilation worthy in my book.
