    Comment from a senior colleague in UK-Morning! Very interesting. We are guided by BAPM guidance on resuscitation of extreme preterm. Also in U.K. we are shifting towards use of rPAP for primary respiratory support

    Questions from a senior colleague: My comments/ suggestions
    1. Many times extreme preterm babies, may not initiate breathing, do you still do DCC in those who are not breathing that well (breathing but not that well)
    2. How do you maintain temperature during the time of DCC (apart from maintaining the room temperature)
    3. Do you use any of the temperature maintenance device in the sterile area
    4. We always need a warmer source for the plastic wrap or bag work, that creates a warm micro environment just around the baby. Can u share your view on using plastic wrap without warmer source

      1. Yes, as long as cord pulsation normal, respiratory effort doesn’t matter for DCC 2 and 3. Apart from room temperature and receiving in bag, nothing else at present but we restrict to 30 seconds in such babies 4. Agree, but it still minimizes IWL from draught and ensures baby is ready with bag while under warmer. It’s only a 20 second difference

      1. Do you drape and wear the sterile gown to enter the OBG field to put the plastic bag
      2. How do you sterilize the plastic bag?

      We can send the bags for gas sterilization, there are commercial sterile options too. The nurse receiving the baby is scrubbed and gowned
