the lever you got working is the 1,2,3 position on your chart. in and out I believe is the bottom right lever like low and hi or back gear in and out. the top right is A and B
My 1966 Springfield lathe had a similar problem. Mine was just a bad shifter mechanism. I made a new one and now it runs sweet. These machines are really beefy.

Nice , I have one just like it , got it 2 months ago , today was the first day I was able to run it , had to get past the electric .
