How Coughing Causes Low Back Pain (and what to do about it)

  • Is coughing low back pain taking your cold from annoying to downright painful? I’ll teach you why the two are connected and how to avoid the pain.
    There are 3 major categories of back issues - flexion dysfunction, extension dysfunction, and compression dysfunction.
    If you’ve got flexion dysfunction, movements that involve flexing or bending forward will aggravate your low back pain symptoms.
    If you’ve got extension dysfunction, movements that involve extending through the spine will be problematic for you.
    In the last type, compression dysfunction, activities that cause weight to bear down through your spine will aggravate your symptoms.
    When people cough, we tend to go into flexion. That’s because when you cough, your abdominal muscles engage. These muscles are flexors of the spine.
    Because of this, it’s not surprising that coughing low back pain is more likely to be an issue for people with flexion dysfunction.
    It’s also a big issue for folks with compression dysfunction because any strong muscular contraction in the torso will probably add to the compression forces and cause symptom aggravation.
    The strategies will differ based on what type of low back pain you’ve got, so it’s important you take a look at your symptoms and understand the root cause.
    If you have flexion dysfunction, your strategy will be to avoid letting a cough pull you forward into flexion.
    If compression dysfunction is giving you trouble, you want to avoid strong abdominal contractions when you cough.
    The simple postural adjustments taught in this video will help prevent that irritating cough from causing big setbacks in your low back pain progress.
    That way, the next time you’ve got a cough, you can avoid your back symptoms flaring up. You need to focus on getting over your cough - not dealing with a new low back pain episode!

    Thank you so much.. This was the first time, I ever felt the pain due to coughing. I got really really sick with the mega flu and it was horrifying. Thank you for these exercises they helped amazingly!

    Thank you so much. I'm experiencing low back/spine pain (for the past 2 weeks) with pain in both hips and upper butt area. I didn't know that coughing could cause this pain. Have had a bad cold with cough since early December. I will follow your suggestion especially for the "flexion" exposure, which I believe is my situation.

      @@PrecisionMovementCoach Thanks again. I've been dealing with low back/hip pain and didn't know what could be causing it. I have been coughing much in the past 2 months with a bad chest cold (finally got my doc to send me a good cough syrup with codeine) which led to bronchitis. Still have a dry cough but working on it with more meds. My coughing method was bad and now I see what is aggravating my back/hips. Hopefully I'll get over this thanks to your help. Keep up the good work and stay well.

    My abs are so sore, I feel like every time I cough someone stabs me. I can't stop coughing cause I am sick, I can try to control my cough and do small coughs but it doesn't get rid of it, so I am force to make a big strong cough.

    I find your video very informative, I get a bad sharp pain in both legs when I cough , I have prolapse disks and presume that's the cause of it

    Do you have a routine or exercises that we can do to help heal these dysfunctions? I think mine sound like a compression and maybe a little flexion. Don't know if that's possible. I cant walk straight up for too long, I lean a little foward. But when I cough I have to lean back to help reduce the feeling of the disc slipping and the pain. 😫

    Wish I had seen this video before goin to hospital. Had dry cough for 3wks, my lower back started to hurt for 2 days straight. Got one of those cough attacks and tore my muscle and tissue FROM COUGHING!! I dodnt' know this was a thing.

    Coach E - when I cough, my back doesn’t hurt, but rather my posterior hip/butt around the glute medius. Sciatic like symptoms - extends to calf. Uncomfortable to drive and worst pain is standing up getting out of the car (pain in same spot). When I sneeze or cough, the pain is sharp as hell but then goes away.
    After conducting each test, laying on my stomach brought pain for about 10 seconds then eased away. The only other test that brought pain was the dumbbell raise while in extension. The test result came back as a flex ion dysfunction though.
    Can you recommend anything? P.s. I have a lot of external rotation - feet point out all the time. I’ve been going through this hip trouble for 3 months now - visited a chiro and massage therapist with no luck.

      @@PrecisionMovementCoach Thanks a ton Eric.
      The beginner piriformis stretch you have demonstrated used to be my favorite stretch before this injury, I performed it nearly everyday. I can’t even get into that position anymore without pain in the same spot - posterior hip/buttock
      Do you suggest staying on the standing piriformis stretch until I can properly get into that figure four position? What’s your opinion on foam rolling/massage ball on that spot for someone who can’t get into a figure four? Roll it up daily or leave it alone? It’s hard to tell what’s aggravating my nerve anymore.
      Important to note that anything with internal rotation seems to relieve the pain temporarily, and I was told by my chiropractor that my rectus femoris is tight. I’ve tried TFL and PSOAS compression, all bring slight pain relief. But my posterior hip/butt is what is horribly inflamed.
      My assessment returned as flexion syndrome and I signed up for the emails, I’ll pay attention to those.

      Definitely do some rolling/releasing before attempting the exercises. It can help to restore the quality of the tissue before you mobilize and activate the area.
      Keep in mind that you can do too much rolling/releasing so keep your sessions to 60 sec or less. Daily should be ok.
      Continue to work on the progressions that you are able to manage before attempting more challenging ones.

      Understood. Final question - with 60 second rolling sessions - how often can I roll? 60 seconds - relax for 10 secs - then repeat for a few more sets? Or should it be more tame keeping it at one session every hour or so?

      Do the 60 sec rolling sessions once daily. Because it is a passive modality no amount of rolling is going to help you to address the root cause of your issue. It may feel good in the moment but the tissue will just return to it's provoked state after a while.

    Hi i just want to ask what to do to my back yesterday i just pick somthing in the floor then somthing pop in my lowerback i think then after that i have pain but not too much and after 40mins pain got reduced but that night im lying down in bed belly down suddenly i sneeze and i hurts my i i feel that pop again in my back now my pain is worse any suggestions?     I know to video is old but I have a question, In april 2022 I start lifting weight in school. After a very hard week overall, I sneeze and I suddenly have a small pain in the left side of my back like a dot or a ball.
    Its been a few month and now the pain is still here and now in the middle of my back.
    I did go to the hospital and they Say its nothing. I hope you have an Idea of what it could be but if you dont, Thank you anyway

    in the middle of my sleep i suddenly sneeze ,then my lowback ache and cannot go back to normal position cause of severe pain, and this pain cannot cure immediate of any medicines i was stock on my bed for a couple of weeks,it was hard for me to stand. so much struggling when the times i had to go to comfort room,feels like i was dissabled person..

    It’s weird when I take a deep breath my mid to upper back hurts. When I exhale exaggerated my lower back hurts. 🤦‍♂️
