Our lecturer was adamant about this as well... and all the textbooks seem to be unanimous in their guidance that you should start at the right iliac fossa for the palpation of the spleen as well as the liver. Sorry about the marks you lost. I'm slightly taken aback by this 'Error' from Macloeds'.
Many many mistakes: 1- skipped inspection (flat/distended, caput, visible peristalsis and wall veins, bulges, diastasis) 2- auscultation is done before palpation since palpation may elicit bowel sounds 3- included testing for the kidneys and bladder even though they’re part of the GU exam 4- did not mention testing for pallor, mucosal telangiectasis, aortic bruits, renal artery bruits 5- did not mention the rectal exam 6- should hv included more details in the inspection of the tongue Internist

The wrong thing that this candidate had done is that he put his elbow on the side of the patient face instead of putting it on the opposite direction while he examining for ascites , what a fatal mistake.
This is the most comprehensive exam with explanation of abnormalities I've ever seen. Thanks so much!
