Same here that happened to me but when the glitches happened, instead of going to the start like you did, the game thought I won so I got the golden hair piece.
I don't think it's a glitch but whenever I fight inner agent three and my armour is broken, unless I'm at the other side of the map, her autobombs always kill me even outside of the hitbox. It's real annoying
Alright so I got a weird glitch to. So it was on the 1st phase. So heres what happened. You know she splashdowns once? Well, I heard in the audio 3 splashdown noises. Like the sound effects for the splash down 3 times in the 1 spot. And her splash done range got bigger. It was some kinda glitch were she did 3 splash downs in 1. Yes it killed me because the range filled the entire map. I'm like wtf. Since then I havent gotten the glitch. This happened about 5 months ago.
Wonder if this ever got patched, 'cos a 1/60th frame perfect chance over a few million Octo Expansions through a few hundred thousand attempts, means it would have actually happened quite a lot of times?

Interesting! I already had the golden pin when this happened, so maybe that is why it restarted the fight

Yeah, it's really annoying because you cannot run away from it (not a glitch tho)
It's probably because agent 3s auto bombs blast radius is bigger than normal auto bombs
The auto bomb launcher is hell for me
