Do you mean a tapered belt where it's narrower in the front than the back? Part of the function of the belt is to brace and push your abs against the belt, so a narrower front will make that less effective in my opinion. There are very small lifters wearing regular 4 inch powerlifting belts.
They are very different, specifically in the mechanism. The SBD will be able to be worn tighter more easily. However the Wahlander can still get quite tight with a bit of practice.

For heavy lifting of any type I would recommend a leather belt like the Inzer or SBD. The Lever is the most popular style belt.
