    Re: Uric Acid. I suggest you look up "PRAL - Potential Renal Acid Load" as that is a related topic and I think would make for a very interesting video series.
    Re: High Density Lipoproteins. I would say that there is more controversy here. It seems to be true that high density lipoproteins go up with exercise meaning that it can be a marker for good health, but it also seems to go up with LDL and overall cholesterols, meaning it can also be a marker for bad health. This (like "large fluffy LDL") also is likely to be an area where the "butter is back" industry scientists have muddied the water.
    The C-Reactive Protein scores look really promising. Did you see that Dr. Katcher is closely monitoring inflammation markers in his E5 work?

    Hi, regarding Allergies ... (my research and opinion)
    I discovered that Allergies or Intollerances are generally caused by Nutrician Deficiences.
    In chemistry terms - Limiting Reagents.
    The body needs the substances to complete burning out offending builups of some residue substances.
    Build ups of certain substances can cause Immune Responses, thus supplementing deficiencies will allow
    for the build ups to be processed out of the body.

    Hello Richard just received my blood work.first time my liver, kidneys ,glucose and thyroid were normal.my
    triglycerides were elevated 1st time. Been on nmn,tmg,and all the other longevity supplements for 2 months.been using a mix of donotage and renue by science but making a change to donotage

    I appreciate your information. My HDL has been 100, 99 the last few years. Glad to know that's a good level. I do not worry about elevated cholesterol, watch "The cholesterol Myth" by Dr. Zac Bush. I would like to do a coronary calcium scan since I had a bad diet and low Vit D for years. I am very interested in your experiment on when to take NMN. I do night shift and was not sure about when to take it. Thanks so much!

      Cholesterol isn't the problem, it's calcification of tissue due to lack of tetrahedrobiopterin.
      Cholesterol is a rate limiting enzyme for the utilisation of protein, and is involved in wound repair.
      I've worked out why tetrahydrpbiopterin levels drop with age, and how to correct it. Working on patents, though. It's so simple I can't resist making money on it.

      Hi Kat, thanks for sharing. Your HDL levels are looking really good. Did you do anything special to raise them to that level?
      I will watch Dr Bush. I have also watched videos from Dr Michael Lustgarten which tell a similar story with 130 - 140 LDL being associated with lowest all cause mortality.
      I will certainly be posting the results from my sleep NMN and Oura ring test when I get them.

    Thanks for sharing the blood test info. Two comments:
    1: Uric acid has some important functions in the body and so good to keep it low (ex: under 5.5 mg/dL), but not too low.
    2: You mentioned that it was your first time doing CRP (I assume it is hsCRP). FYI in case you'd like to target it: a common ideal level target I've seen is under 1.0 and even under 0.50 mg/L.

      Hi Terry, thanks for your comment. On Uric Acid, I would agree, mine is higher than I would like and something that I need to improve. On the CRP, it was just CRP, the hsCRP was more expensive and we were looking for a general understanding of whether we had high levels of inflammation, rather than being specifically concerned about CVD. Perhaps next time we should use hsCRP and get a more sensitive reading.

    Can you please help: I'm a woman 50 on HRT since a year (progesterone pill, estrogen vaginal and testosterone cream). Should I stop this before starting on NMN, since I read NMN raises estrogen and testosterone? HRT is not solving all my problems but at least I’m able to have an intimate relationship (avoiding dryness and pain). If you cant answer this I need to be recommended a doctor I can speak to. NMN is not mainstream and my doctor will likely not know what it is or recommend it. I would love it if my menopause was reverses but even if not, I would like to be as rejuvenated as possible in every way.

Based on our discussion with Dr Perlmutter I am doing most of the right things (low sugar, no sodas) but I will try taking quercetin also celery seed extract and cutting down on the alcohol.     Richard, One other item to mention. You mentioned getting your and your wife's HDL > 100.
    As I recall, there was a study that came out recently and surprisingly concluded that cardiovascular disease risk increases once HDL got above a certain level - I think 90 mg/dL. And so seems to be a sweet spot between 60 to 90 or so. So you may want to look at the study.
    My HDL is 92 mg/dL. I'm not concerned and will see what else comes out on it.

      I think that there is research supporting both low HDL and high HDL. I think that the real story is that HDL is a marker that can't be used in isolation and has to be incorporated into a larger story. You have to know *why* HDL levels are what they are to know if they are good or bad. At least that is my top guess based on the research I have seen.

      HDL, VLDL and LDL are just like taxies 🚕 that carry the passenger called cholesterol.
      HDL (high density lipoprotein), is kind of lipoprotein which is throwing your cholesterol out of the body. It ferries cholesterol from liver to intestines to be thrown out in poop.
      Whereas VLDL and LDL are types of lipoproteins which ferry cholesterol within your body to different locations, to different organs via blood.
      That's why if you have more VLDL and LDL, that means more cholesterol traffic within your body, or more problems. And if you have decent amount of HDL that means your body is healthy enough to send more cholesterol out of body.
      HDL high means more cholesterol going out of your body.
      But then if *much* more cholesterol is going out of your body, then it means it was there in your body to begin with. 🤷.
      That means logically speaking *much* more HDL than a certain amount should also mean a bad thing in my opinion.

      Hi Scott, thanks for your question. We started this focus on health and longevity about the same time that we started taking NMN. I did not get my blood markers done before then. So, it would certainly be good to be able to compare them but I don't have the numbers.

    I wonder, are you taking NMN sublingually? Renue by Science has several articles promoting the benefits of sublingual tablets vs the lower absorption rates associated with the gastrointestinal tract.

      Hi Thanks for your question. I am taking NMN sublingually at the moment, but will probably switch back to orally with liposomal NMN. I have found that some form of sublingual NMN work for me and some do not. (I like powder but not the lozenges so much). I think that the theory behind both is good but we don't have a great deal of data on the efficacy of them.

      Hi Joe, thanks for your comment. It would be good to do that kind of comparison but there are too many variables to draw any conclusions. So I think what we can say is that NMN does not seem to have any negative impact on my key markers.

    I thought Sinclair and other were taking TMG to lower homocysteine. What were your starting and ending homocysteine numbers?

      Hi Jack, thanks for your comment. One of the issues with low methyl groups is excess homocysteine so that is why I take both TMG and methyl folate. However the test I took did not have the homocysteine levels included.

      "Warning-High doses of dried parsley like 40ml/day reduced my platelets below the reference range. .5-1.5g NMN uses up methyl donors and my homocysteine went from 7 to 17. So please do this under the advice of a physician.:
      If anyone is going to supplement with NMN, Niacinamide, NR or Niacin above RDA amounts for niacin, it would seem to be prudent to get a homocysteine test. Mortality rates start increasing over 9. There is a reasonable chance you're over that if you're taking high dose NMN, even if you take methyl donor supplements.

    Hello Richard, Your videos are immensely valuable! :) I'm wondering if you take a B vitamins complex, in addition to NMN? Is it fine to take niacin in addition to NMN? I have been taking Seeking Health's B-Minus Complex which contains 50 mg niacin but I don't know if that's compatible with NMN. All best wishes ^_^

    What is the consensus optimal dose of NMN & Resveratrol each day? Also, why aren’t you taking Spermidine, part of Sinclair’s regimen?

      Hi Heimi, thanks for your question. I do not make recommendations, but Dr. Sinclair takes 1g of each I believe. I have taken 1g but because I am taking liposomal NMN I am now taking 500 mg.
      I have thought of spermidine and may well add that to my supplements but have not got around to it yet.

    AST/ALT ratio can be high due to alcohol abuse, usually >3-4. Otherwise I'm not aware of any other clinical significance. Also, you ALT and AST numbers are low so I wouldn't worry.

      Hi Maggie, thanks for the question. I was taking 1g NMN and 1g resveratrol. I am now taking about 500 mg of liposomal NMN. I have temporarily switched to pterostilbene of which I am taking 500 mg.

    You FBG is much lower than it was previously, could this be due to the liposomal NMN? Were you already on the liposomal NMN on 19/9/21?

      Hi thanks for the comment. I was pleasantly surprised by my number this time. I started Lipo NMN in July 2021, so would have been taking it on 19/9/21 for a couple of months.

      (talking about non liposomal NMN )...
      This can result in "methylation problems" where there are insufficient methyl donors that are critical for other needs and promotes age related health problems. (r)
      TMG MAY HELP RESTORE METHYL GROUPS WHEN NAM LEVELS ARE TOO HIGH
      As a result, it is common practice now for those taking NMN or NR to take TMG or other methyl donors. (r)
      That is thought to help with the problem of excessive NAM buildup from taking high dosage NMN or NR capsules, but excess TMG can also lead to problems. (r)
      LIPOSOMAL DELIVERY IS THE SOLUTION
      A far better solution is to take a form of NMN or NR that protects the active ingredient from digestion to NAM.
      Sublingual delivery of NMN delivers up to 30% of NMN to the bloodstream, so much less is digested to NAM and depletion of methyl groups is much less of a problem.
      Liposomes are even better, as they can protect NAD+ metabolites from digestion and degradation in stomach, liver, and bloodstream.
      Liposomal delivery prevents your expensive NAD+ precursors from being digested to NAM, which is a waste of money, and can actually be bad for your health.

    Richard what are your units for CRP? I'm used to seeing mg/L reported in US for high sensitivity CRP (hsCRP). With goal of staying below 0.3 mg/L.

      @@ModernHealthspan My last hsCRP was 0.24 mg/L from Labcorp here in US. Not sure of their detection limit but might be 0.2. In any case, I recall Micheal Lustgarten showing that lower the better down to < 0.3 mg/L. Detection limit difference could be due to difference in the test, CRP vs hsCRP. Being less than 0.6 mg/L is good though.

      Hi Otto, Thanks for the question. I aim for 1 to 1, but I also take methyl folate, and now that I have switched to Liposomal NMN I have reduced the dosage. Si I still aim for 1g of TMG but do not always take that every day.

    My conclusion after watching take and promote NMN was a waste of money. Sorry but diet and exercise could do better imo

    I would be more concerned about the effect that NMN may have on lowering CD38, a vital protein located on the immune system's white blood cells. I wonder if by taking NMN, you might be lowering your resistance to diseases. The only legitimate reason I can find for taking MNN is if you has asthma, which can be relieved by lowering CD38 in some cases.

      Hi Paul, thanks for your comment, though I do not follow it. I thought CD38 could consume NMN as well as NAD, it increased in old age and was one of the reasons for lower NAD levels in the body.
      In fact I take apigenin specifically to lower my CD38. I have not heard that it was important for the immune system. Could you point me to a study on this? I would be very interested to learn more. Thanks

    So I just compared your cholestrol with my own stats. You are promoting "modern healthspan" yet you rack in a total cholestrol of 215... versus my 127? Where the total Cholestrol/score is 3.10 versus your 2.76. Look specifically at LDLD you are at 127 where I am at 64, Your HDL 78 versus mine 41,..
    What does that show? You have alot more lipids going about which isn't a good thing where ApoB gives us a good indicator of the risk of plaque formation. Lower seems to be better always.
    I can conclude from this that your focus on Modern Healthspan but you are leaning too much on supplements and did not adjust the diet for it. Leading to these higher scores that could easily be lowered signifcantly by just gettting rid of some more saturated fat in the diet. You are doing yourself a disservice by not adjusting dietary patterns for better outcomes. After 2 years of awareness these numbers shouldn't be this high.
    Als for your USA "averages" you might be in the "safe zone".. For EU standard (alot better in healthspan and health promotion) You are over the top end of all these scores. That is a bad thing. Try cutting out animal saturated fats it will do wonders on these scores.
