Atrial fibrillation (AF) is influenced by a variety of risk factors. Among the nonmodifiable factors, aging plays a significant role, with AF being more common in older individuals, particularly those over 80. Gender also has an impact, as men are more likely to develop AF than women. Genetics can also contribute, as certain genetic mutations can increase the risk of AF.
High blood pressure is a major concern, as it increases the risk of coronary artery disease and can lead to structural changes in the heart that affect its electrical signals. Diabetes and obesity are closely linked with AF, with obesity being a particularly significant factor. As obesity rates rise, so do AF cases, since excess weight can alter the heart's structure, making AF more likely.
Obstructive sleep apnea (OSA) is another condition that independently increases the risk of AF. Additionally, imbalances in electrolytes such as potassium, sodium, calcium, and magnesium can cause irregular heartbeats. Heart problems, including coronary artery disease and heart valve disease, as well as a history of heart surgery or a heart attack, increase the likelihood of AF. Conditions like sick sinus syndrome, where the heart's natural pacemaker malfunctions, and sleep disorders such as OSA further contribute to AF risk.
Certain medications and supplements, including some prescription drugs and over-the-counter cough and cold remedies, can trigger irregular heartbeats. Thyroid disease, particularly an overactive thyroid, also raises the risk of AF. Viral infections, like those caused by chikungunya, coronavirus, and influenza, can play a role, as can inflammatory conditions such as psoriasis and rheumatoid arthritis. Lifestyle factors like caffeine intake and the use of illegal drugs, including amphetamines and cocaine, can accelerate heart rates and contribute to arrhythmias.
Exercise has a complex relationship with AF. While moderate exercise is beneficial and reduces AF risk, very intense, long-duration endurance training may increase it due to changes in heart structure and function. Smoking is a significant risk factor. The risk of AF increases with the duration of smoking, and even former smokers have a higher risk than non-smokers.
Alcohol consumption is another modifiable risk factor. Both acute and chronic alcohol use can trigger AF, with "holiday heart" syndrome describing AF occurring after heavy drinking. Consuming more than 21 drinks per week increases AF risk substantially, with women who consume more than two drinks a day seeing a 60% increased risk.
