Thanks Doc for speaking out.. in my job im seeing the after effects of lockdown within the community ... self harming, mental health and serious undiagnosed illnesses such as cancers.. we need the surgeries open for business.. the lockdown was supposed to be saving people yet all those I'm dealing with are now needing long terms services mainly due to isolation, mental health & lack of exercise.. this is just so wrong in so many ways

We are short several thousand GPs in this country but demand for primary care service is increasing. This was a problem even prior to the pandemic but it has been catalysed by the events of the past 18 months. Most things do not actually need a face to face appointment. Some people prefer not to have to come in for a simple prescription renewal or urine infection. From the patients perspective it is much quicker and instead of waiting in a car park/waiting room they can go about their day whilst waiting for a call. From the GPs perspective it is much quicker also, meaning they have more time for the patients that are more complex. There are of course those people that feel like they 'need to be seen' but this should really be at the doctors discretion and based on clinical need, not based on what someone 'wants' - in the same way that they wouldn't prescribe an antibiotic just because a patient felt like they needed them. But I understand how this can create some confusion that GPs are ignoring their patients. An example: a 55 year old woman calls her GP because she's complainig of bleeding from down below and she is losing weight. The GP should see this person face to face, because she might have cancer right? Wrong. This woman meets criteria for an urgent referral to the hospital (to be seen within 2 weeks) for a pelvic ultrasound to check for uterine cancer. Would bringing her in to the surgery and feeling her tummy and doing an intimate examination of her lady parts change anything? No, because either way she still needs an urgent referral to the hospital for an ultrasound. By managing problems such as this and many others remotely it enables the GP to overall be able to help MORE of their patients, in time where demand is high and there are limited resources. The number of GP consultations overall (whether that be face to face, online, phone etc) is higher now than it was pre-pandemic. If you make face to face the default it will make it take LONGER for people to be able to access their GP. Clueless politicians and journos who know nothing about medicine should stay out of telling doctors how to treat their patients.

In there it also reminded the GPs to stop checking their new patient for their eligibility re access to NZhS care. The have to register anyone who request it!! Madness - no wonder it is impossible even to have a phone consultation!!

GP receptionists are not supposed to "triage" patients, although apparently they frequently do and expect to discuss your medical condition in front of others in the queue. GPs are now run like call centres, they are "gate keepers" to the services you need. A lot of what GPs do could be replaced by an algorithm, and some of them seem to be so lacking in humanity I assumed they were secretly replaced by robots long ago.

I had this experience last week. I called the GP's surgery and the receptionist said I would need to tell her what was wrong and if it sounded serious she would get a nurse to call me back. I said its personal and I'd rather not tell her what was wrong and she replied that she couldn't get me a call back in that case. I asked "I have doctor - patient confidentiality, do I now need to have receptionist - patient confidentiality?" Short answer is you have no choice but to tell the receptionist your personal medical problem/history and hope she deems it worthy to have a nurse (not a doctor) call you back.

I was amazed when recently the NHS sent a letter to all GPs saying (amongst other matters) that should a patient turn up at the surgery without an appointment they should be shown how to get an appointment online or using an app. This shows a complete lack of appreciation of the problem. Not everyone has a mobile phone or indeed access to the internet and/or some people do not have the skills or ability to go online and book an appointment. There was a letter in the DT last week from someone saying they had 123 attempts to contact their GP surgery to book an appointment.
In any other "industry" the consumer could switch to another provider and posting on social media or platforms such as tripadvisor would cause the situation to be addressed immediately.
