When threading this particular Axis 114w103 one has to align the looper hole with the outer hole. Turn the handle until you see that hole then pass the wire threader through to pick up the thread .
i think your tension is too tight. you should never have to loosen he thread up below as you are doing. it could also be your needle. the trouble shooting list could go on...

Check if the needle is screwed in tight and make a turn or a few right with the handle before starting the machine, maybe it will help.
